Prophylactic antibiotics to reduce morbidity and mortality in neonates with umbilical artery catheters.

BACKGROUND: Umbilical artery catheters are often used in unwell neonates. Infection related to the use of these catheters may cause significant morbidity and mortality. The use of prophylactic antibiotics has been advocated for all newborns with umbilical artery catheters in order to reduce the risk of colonisation and acquired infection. Countering this is the possibility that harm may outweigh benefit. OBJECTIVES: The primary objective was to assess whether prophylactic antibiotics, in neonates with umbilical artery catheters, reduce mortality and morbidity. In separate comparisons, we plan

Antiviral prophylaxis and isolation for the control of pandemic influenza.

Before effective vaccines become available, antiviral drugs are considered as the major control strategies for a pandemic influenza. However, perhaps such control strategies can be severely hindered by the low-efficacy of antiviral drugs. For this reason, using antiviral drugs and an isolation strategy is included in our study. A compartmental model that allows for imported exposed individuals and asymptomatic cases is used to evaluate the effectiveness of control strategies via antiviral prophylaxis and isolation. Simulations show that isolation strategy plays a prominent role in containing t

Antibiotic Use in Dental Implant Procedures: A Systematic Review and Meta-Analysis.

Background and Objectives: This project was developed from anecdotal evidence of varied practices around antibiotic prescribing in dental procedures. The aim of the study was to ascertain if there is evidence to support whether antibiotic (AB) use can effectively reduce postoperative infections after dental implant placements (DIPs). Materials and Methods: Following PRISMA-P© methodology, a systematic review of randomised controlled clinical trials was designed and registered on the PROSPERO© database. Searches were performed using PubMed®, Science Direct® and the Cochrane© Database, plus the

Asplenia and spleen hypofunction.

Asplenia (the congenital or acquired absence of the spleen) and hyposplenism (defective spleen function) are common causes of morbidity and … asplenia are warranted to prevent overwhelming post-splenectomy infections through vaccination and antibiotic prophylaxis. Although recommendations are available, the implementation of vaccination strategies, including more effective and immunogenic vaccines, is needed. Additionally, screening programmes for early detection of hyposplenism in high-risk patients and imp …

[Prophylaxis and Therapy of Malaria: Current Recommendations].

Malaria incidence is decreasing on a global scale, while the number of imported cases has remained at a high level in Germany. To decrease this number, counselling of travellers to malaria-endemic regions is important. Patients to high risk countries need regular chemoprophylaxis. The alternative of stand-by emergency treatment for travellers to regions with low or medium risk has been met with growing criticism. For the treatment of all malaria cases, artemisinins are a mainstay of treatment. Regular follow-up is warranted to prevent relapses. After treatment with intravenous artesunate for c

Urinary tract infection in children: recurrent infections.

INTRODUCTION: Up to 11.3% of girls and 3.6% of boys will have had a urinary tract infection (UTI) by the age of 16 years, and recurrence of infection is common. Vesicoureteric reflux is identified in up to 40% of children being investigated for a first UTI, and is a risk factor for, but weak predictor of, renal parenchymal defects. METHODS AND OUTCOMES: We conducted a systematic review and aimed to answer the following clinical questions: What are the effects of treatment of acute urinary tract infection in children? What are the effects of interventions to prevent recurrence? We searched: Me

Fetal blood sampling.

OBJECTIVE: We sought to review indications, technical aspects, risks, and recommendations for fetal blood sampling (FBS). METHODS: A systematic review was performed using MEDLINE, PubMed, EMBASE, and Cochrane Library using the terms "fetal blood sampling," "percutaneous umbilical blood sampling," and "cordocentesis." The search was restricted to English-language articles published from 1966 through July 2012. Priority was given to articles reporting original research, in particular randomized controlled trials, although review articles and commentaries also were consulted. Abstracts of resear

[The prevention and treatment of the gastroduodenal ulcer bleedings].

The results of the complex prophylaxis and treatment of 33 458 patients with gastric or duodenal ulcer were analyzed. Of the analyzed group, 1002 (2,99%) patients experienced ulcer bleeding. The complex out-patient and stationary treatment of the ulcer disease allowed of bleeding frequency from 8,6 to 2,1%; the decrease of recurrent bleedings from 13,0% to 0; allowed to avoid major surgery (the previous frequency was as high as 14,5%) and decreased lethality rate from 9 to 1%.

Antimicrobial prophylaxis in oral surgery and dental procedures.

Transient bacteraemia is a known risk factor following oral surgery and invasive dental procedures in patients with altered immune system response and those with a susceptible site of infection (patients with heart valve prostheses or recent joint replacements, etc.) The most commonly isolated aerobic bacteria in postoperative bacteraemia are Streptococcus Viridans. However, other periodontal pathogenic anaerobic bacteria are found in up to 64% in blood cultures (mixed bacteria or anaerobic bacteria alone). Dental pathogenic bacteria do not appear to be covered by standard amoxicillin or clind

Prophylactic strategies before solid-organ transplantation.

PURPOSE OF REVIEW: Prophylactic measures prior to organ transplantation are evolving based on recent reports of emerging infectious diseases, as well as an expanded understanding of the epidemiology of familiar infections. This review will highlight developments with potential impact on donor and recipient screening and pretransplant management. RECENT FINDINGS: Key findings regarding bacterial infections include the lack of utility of mupirocin intranasal decolonization for prevention of Staphylococcus aureus infections after liver transplantation, and the description of transmission of Pseu

Bacteraemia during direct laryngoscopy and endotracheal intubation: a study using a multiple culture, large volume technique.

Bacteraemia secondary to orotracheal intubation has been reported to occur in 0-5.3% of patients. Bacteraemia detection is dependent upon several factors including the volume of blood per culture and the number of cultures. Prior studies used small volumes of blood and one or two cultures, and may therefore have underestimated the incidence of bacteraemia. Sixty-two adult patients who underwent direct laryngoscopy and endotracheal intubation were studied. Baseline blood cultures were sterile in all patients. After intubation, four blood cultures were obtained in ten minutes, with 10 ml being e

Antibiotics for incomplete abortion.

EDITORIAL NOTE: This review has been superseded by a more recent Cochrane review https://doi.org/10.1002/14651858.CD014844.pub2 BACKGROUND: Unsafe abortions result not only in costs for acute care but may also be responsible for longer-term complications such as pelvic inflammatory disease, damage to reproductive organs, and secondary infertility. If effective, antibiotic prophylaxis at the time of the procedure can potentially prevent these adverse consequences. OBJECTIVES: The value of routine antibiotics before surgical evacuation of the uterus in women with incomplete abortion is controv…

Increased Risk of Periprosthetic Joint Infection Following Aseptic Revision Hip and Knee Arthroplasty With the Use of Non-Cefazolin Alternative Antibiotics.

BACKGROUND: Although perioperative cefazolin antibiotic prophylaxis is associated with a reduced risk of periprosthetic joint infection (PJI) in primary total joint arthroplasty (TJA) compared to non-cefazolin alternatives, its relative efficacy in the aseptic revision setting is less defined. Furthermore, the roles of postoperative oral antibiotic prophylaxis and the management of unexpected positive cultures (UPC) remain controversial. Therefore, we examined PJI in aseptic revision TJA based on (1) perioperative antibiotic prophylaxis, (2) postoperative oral antibiotic regimen, and (3) UPC s…
