The pars plana approach to cataract extraction in the pediatric age has several advantages. We report of 9 eyes with congenital cataract and discuss the advantages of the pars plana technic. In all cases a single surgical approach was sufficient. There were no complications postoperatively which made either surgical or medical treatment necessary. Implantation of an artificial lens after extracapsular cataract extraction with perforation of the anterior and posterior lens capsules is only possible under special circumstances. In only one out of 5 cases could a posterior chamber lens be implanted. This kind of injury can easily lead to a vitreous-lens mixture in the anterior chamber, which then has to be removed by a vitrector. In the cases described here the unilateral aphakia was corrected with a contact lens. In order to prevent future complications neither an anterior chamber lens nor an iris clip lens were implanted.

After the indications of the various contact prostheses are briefly reviewed, the interest of continuous wearing of optical hydrophilic soft lenses is emphasized. A 55-year-old woman presented with sudden diminution of vision in her left eye following trauma with her own finger. Examination revealed a firm subconjunctival swelling superiorly with aphakia and hypotony. Lens spike was absent on B-scan. Ultrasonic biomicroscopy showed thickened conjunctiva, a subconjunctival mass lesion with an underlying uveoscleral discontinuity. We have studied our results of 15 epikeratophakias for aphakia, 13 for myopia, 1 for a keratoconus and 1 for a bullous keratopathy. We have about the same anatomic and functional results as they have in the Nation Wide Study published by Mc Donald in 1987. We have 86% anatomic success and 14% failure for the myopes and the aphakics; 87% of the aphakic patients achieved within 1 or 2 lines of their preoperative corrected visual acuity at one year post-op; 91% of the myopes achieved their corrected visual acuity at three months.

A case of traumatic ocular perforation and aphakia resulting in enucleation after six weeks is used to demonstrate histopathological details of an early stage of a typical foveal involvement with grossly visible petaloid swelling of the central retina, separation of the inner limiting membrane with some exudate, massive swelling and early cystoid degeneration of Henle's fiber layer and a vertical retinal fold of the outer retinal layers extending downward from the foveola. This demonstration is important in view of its possible relationship to the clinical entity of cystoid macular edema.

In order to assess the applicability and safety of cellulose acetate buterate (CAB) contact lenses for extended wear in aphakia, 139 lenses were fitted in 102 aphakic patients. These lenses were removed periodically for cleaning only. There has been an average follow-up of 24.75 months on 99 patients (135 lenses), with a success rate of 80%. No serious complications or corneal vascularization have occurred. The major problems encountered were due to lack of tint, increased tendency to accumulate mucus, and the need for periodic insertion and removal.

The development of soft lenses is recent compared to the history of hard lenses. In cooperation with chemists an appropriate material was developed which was found more suitable than all previously used hard material for close contact with the cornea. The development of the soft lens from the modest start up to the most advanced soft lens is shown in tables demonstrating the therapeutic results in young children with unilateral aphakia. During the development the scepticism of many experts had to be overcome. To day the soft lens is a contact optical and thereapeutic aid of great significance.

Of 94 patients (100 eyes) with giant retinal tears, 71.4% of eyes with nontraumatic breaks were myopic and severe retinal pathologic findings were present in 57.3% of fellow eyes. Fifty-five (58%) of 95 treated eyes were successfully reattached; retinal incarceration, attempted in 19 cases, was successful in five. Serious surgical complications were responsible for failure in nine cases (22.5%). The worst prognostic factor were extension of the tear for over two quadrants, myopia of -10 diopters or more, and the presence of aphakia.

PURPOSE: To report the long term visual outcomes and complications with use of the novel CM-T Flex scleral fixated intraocular lens (CMT-SFIOL).
MATERIALS AND METHODS: 116 eyes that underwent CMT-SFIOL were reviewed and 57 eyes with CMT-SFIOL that completed a 2-year follow-up were included. METHODS: Records of 36 children (36 eyes) with RGP CLs to unilateral aphakia between 2014 and 2018 were evaluated.
