In order to study the relationship between sleep-induced periodic breathing and the development of occlusive sleep apneas, 6 patients with hypersomnia-sleep apnea syndrome were studied during nocturnal sleep before and approximately 1 month after therapeutic tracheostomy. Post-tracheostomy studies were performed in open and closed tracheostomy states. Sleep-induced periodic breathing resembling the pattern of Cheyne-Stokes breathing was observed in all patients before and after tracheostomy, even when tracheostomy was left open. When tracheostomy was closed all patients developed periodic hypo…

Periodic breathing (PB) during sleep was investigated in two groups of full-term infants with histories of apnea that were terminated by resuscitation. One group of infants had been reported to be asleep whereas the other group had been reported to be awake when apnea was noted. Electrophysiologic sleep recordings were made after the apneic incident. The infants with histories of prolonged apnea while asleep exhibited more PB during recorded sleep than infants with histories of apnea while awake. Increased PB during sleep in full-term infants, therefore, may not be associated with all prolonge…

Peripheral Sleep Apnea syndrome has been associated with enlarged tonsils and adenoids as well as other abnormalities which may cause upper airway obstruction in children. A multidisciplinary approach is used at the Step Disorder Center of Cincinnati General Hospital to evaluate the role of tonsils and adenoids in sleep apnea. Polysomnographic techniques combined with cine sleep studies of the upper airway document the degree, site and type of obstruction. This presentation highlights the entity of obstructive sleep apnea and provides a rational clinical approach in the management of these pat…

The efficacy of theophylline in preventing severe apnea was evaluated in 17 low-birthweight infants (mean weight, 1,400 gm). Apnea was detected and accurately quantified by 13-hour pneumogram recordings and correlated with serum theophylline levels. Nursing observations coupled with on-line alarm systems detected only 39% of severe apneic episodes as compared to the pneumogram recording technique. Theophylline in six hourly oral doses(1.5 to 4.0 mg/kg) yielded two-hour serum concentrations of 6.6 to 11.0 mug/ml which completely controlled apneic spells exceeding 20 seconds in duration and mark…

INTRODUCTION: Obstructive sleep apnea (OSA) is marked by recurrent upper airway collapse during sleep and is associated with upper airway muscle dysfunction. Oropharyngeal rehabilitation (OPR) targets this dysfunction through structured muscle training. While supervised OPR improves adherence, its time and logistical demands may hinder access and increase dropout.
OBJECTIVE: This study evaluated the effectiveness of a multilevel OPR program delivered via video conferencing on muscle function and the apnea-hypopnea index (AHI) in individuals with OSA.
METHODS: This quasi-experimental study in…

PURPOSE: This study examined the association of insomnia, sleep apnea, and comorbid insomnia and sleep apnea (COMISA) with all-cause mortality in a large Veteran sleep clinic cohort.
METHODS: We retrospectively identified Veterans with insomnia and/or SA using ICD-9/10 codes, requiring two diagnoses within 13 months (≥ 30 days apart). Insomnia cases also required relevant prescriptions and/or cognitive behavioral therapy for insomnia (CBT-I); SA cases required positive airway pressure prescriptions within three years. Patients were classified as insomnia only (INS), SA only, or COMISA. Adjust…

Background: Obstructive sleep apnea (OSA) is common in individuals with obesity, largely due to increased soft tissue causing upper airway narrowing. However, mechanisms of OSA improvement following weight loss are incompletely understood, particularly in Asian population, where craniofacial and soft tissue characteristics differ. This study aimed to evaluate changes in upper airway anatomy before and six months after bariatric surgery. Methods: We prospectively evaluated Indian obese patients with OSA undergoing bariatric surgery. Magnetic resonance imaging (MRI) and polysomnography were perf…

OBJECTIVE: Sleep apnea-specific hypoxic burden (SASHB) is independently associated with cardiovascular disease (CVD) risk in patients with obstructive sleep apnea (OSA), and cardiac autonomic dysfunction (a well-established marker of CVD risk) is also linked to OSA. However, no previous studies have demonstrated an independent association between SASHB and cardiac autonomic dysfunction in patients with OSA.
METHODS: A total of 2270 patients with suspected OSA were recruited between January 2021 and December 2024. We performed separate calculations for the SASHB and 19 heart rate variability (…

BACKGROUND: Shitali respiratory or pranayamic yogic training might help to improve OSA symptoms but no study investigated the response of OSA to Shitali respiratory training (SRT).
AIM: This pranayama study aimed to investigate the response of OSA patients to SRT.
METHODS: Forty from-both-sex OSA patients with polysomnography-based apnea-hypopnea-index (AHI) evidence were included. Patients - aged 35-58 years old - with mild and moderate OSA (5 < AHI <30 events/hour) were included. The closed envelope procedure was used to randomize and assign the OSA patients into the SRT group (received a …

BACKGROUND: The symptomatic response to continuous positive airway pressure (CPAP) therapy in COPD-obstructive sleep apnea overlap syndrome (OVS) compared to OSA syndrome (OSA) alone has not been well studied so far. The aim of this study is to explore main differences in the clinical response to CPAP treatment in OVS compared to OSA alone.
STUDY DESIGN AND METHODS: Using prospective data from the French National Sleep Apnea Registry, we conducted an observational study among 6320 patients with moderate-to-severe OSA, available spirometry, and at least one follow-up visit under CPAP therapy.

Excessive daytime sleepiness (EDS) is a symptom of obstructive sleep apnea (OSA) that resolves under treatment with continuous positive airway pressure (CPAP). In some patients, sleepiness persists despite CPAP treatment. We retrospectively analyzed data on subjective residual EDS, assessed as an Epworth Sleepiness Scale score (ESS) >10, in patients from the European Sleep Apnea Database (n = 4,853, mean age ± SD 54.8 ± 11.8 years, 26.1% females), at baseline and at the first visit (median follow-up: 5 months, interquartile range 3-13). An ESS > 10 occurred in 56% of patients at baseline and i…

BACKGROUND: Obstructive sleep apnea (OSA) patients are at increased risk for pulmonary and cardiovascular complications; perioperative mortality risk is unclear. This report analyzes cases submitted to the OSA Death and Near Miss Registry, focusing on factors associated with poor outcomes after an OSA-related event. We hypothesized that more severe outcomes would be associated with OSA severity, less intense monitoring, and higher cumulative opioid doses.
METHODS: Inclusion criteria were age ≥18 years, OSA diagnosed or suspected, event related to OSA, and event occurrence 1992 or later and <3…

In this 71-year-old man diagnosed as obstructive sleep apnea syndrome initially, the apnea-hypopnea index in polysomnography was 31.3/hour. He started auto-adjusted continuous positive airway pressure (auto-CPAP) treatment in July 2005 but developed congestive heart failure in December 2007. Pulmonary arterial pressure (PAP), estimated by echocardiography, was 71 mmHg. In January 2008, during simplified sleep examination with a breath-movement sensor under auto-CPAP, many central-type apneas were recognized. After replacing auto-CPAP with adaptive servo-ventilation (ASV), the apnea-hypopnea in…

OBJECTIVE: To evaluate the performance of the No-Apnea score, a simplified screening instrument for obstructive sleep apnea (OSA), by gender.
METHODS: This was a cross-sectional study including adults undergoing full polysomnography. The No-Apnea model comprises two items (neck circumference and age) with a total score of 0 to 9. The severity of OSA was categorized, on the basis of the apnea-hypopnea index, as any (≥ 5 events/h), moderate-to-severe (≥ 15 events/h), or severe (≥ 30 events/h). The performance of the No-Apnea instrument was assessed by determining the area under the (ROC) curve …
