Differentiating Kingella kingae septic arthritis of the hip from transient synovitis in young children.

OBJECTIVE: To conduct a retrospective multicenter study to assess the ability of a predictive algorithm to differentiate between children with Kingella kingae infection of the hip and those with transient synovitis. STUDY DESIGN: Medical charts of 25 Israeli and 9 Spanish children aged 6-27 months with culture-proven K kingae arthritis of the hip were reviewed, and information on the 4 variables included in the commonly used Kocher prediction algorithm (body temperature, refusal to bear weight, leukocytosis, and erythrocyte sedimentation rate) was gathered. RESULTS: Patients with K kingae ar

Impact of a pediatric infectious disease consultation service on timely step-down to oral antibiotic treatment for bone and joint infections.

PURPOSE: In recent years an earlier step down to oral antibiotic therapy has been advocated for numerous infections. Trained infectious disease specialists regularly consulting their colleagues may speed up the implementation of such recommendations into clinical practice and thus may improve treatment. METHODS: We retrospectively analyzed bone and joint infections in children admitted to the University Hospital of Cologne between 2010 and 2021. We assessed clinical, imaging, and microbiological findings and treatment modalities. Additionally, we assessed both the impact of a newly implemente

Bilateral septic arthritis of the sternoclavicular joint complicating infective endocarditis: a case report.

BACKGROUND: Septic arthritis is an infectious disease that commonly affects weight-bearing or proximal joints such as the knee and the hip. The sternoclavicular joint is an unusual site of this entity. It usually occurs in patients with diabetes mellitus, intravenous drug abusers, or those with rheumatoid arthritis. Analysis of the previous literature showed few articles and these described essentially cases of unilateral presentation. CASE PRESENTATION: We report a rare case of a bilateral septic arthritis of the sternoclavicular joint sustained by a 71-year-old Tunisian woman whose medical

Adult-onset Still's Disease Presenting with Prolonged Fever, Polyarthralgia, and Hyperferritinemia: A Case Report.

Adult-onset still's disease (AOSD) is a rare systemic inflammatory disorder characterized by quotidian fever, inflammatory arthritis, rash, and markedly elevated serum ferritin levels. Due to its nonspecific presentation and the need to exclude infectious, autoimmune, and malignant etiologies, diagnosis remains challenging. We report a case of a middle-aged patient presenting with prolonged high-grade fever, diffuse abdominal pain, polyarthralgia, and generalized pruritus. Extensive investigations ruled out infectious and autoimmune causes. Markedly elevated inflammatory markers and hyperferri

Narrative review: diseases that masquerade as infectious cellulitis.

For cellulitis that does not respond to conventional antimicrobial treatment, clinicians should consider, among other explanations, several noninfectious disorders that might masquerade as infectious cellulitis. Diseases that commonly masquerade as this condition include thrombophlebitis, contact dermatitis, insect stings, drug reactions, eosinophilic cellulitis (the Wells syndrome), gouty arthritis, carcinoma erysipelatoides, familial Mediterranean fever, and foreign-body reactions. Diseases that uncommonly masquerade as infectious cellulitis include urticaria, lymphedema, lupus erythematosus

Corrigendum to 'quantitative determination of 64Cu-liposome accumulation at inflammatory and infectious sites: Potential for future theranostic system': [journal of controlled release 327 (2020) 737-746].

BACKGROUND: Therapeutic interventions for infectious and inflammatory diseases are becoming increasingly challenging in terms of therapeutic resistance and side-effects. Theranostic systems to ameliorate diagnosis and therapy are therefore highly warranted. The pathophysiological changes in inflammatory lesions provide an attractive basis for extravasation and accumulation of PEGylated liposomes. The objective of this study was to provide direct quantitative information on the theranostic potential of radiolabeled liposome for accumulation in inflammatory models using position emission tomogra

Clinical and microbiological follow-up of an outbreak of Yersinia pseudotuberculosis serotype Ib.

Yersinia pseudotuberculosis, a food-borne pathogen, causes infection that commonly presents as gastroenteritis and mesenteric lymphadenitis. Post-infectious complications include erythema nodosum, reactive arthritis and, less commonly, uveitis and nephritis. Six serotypes of Y. pseudotuberculosis have been identified, and post-infectious complications have been identified following infection with some, but not all, serotypes. The first recognized outbreak of Y. pseudotuberculosis serotype lb occurred in British Columbia in November 1998. We documented the incidence of post-infectious complicat

Medical management of septic arthritis of sternoclavicular joint: A case report.

RATIONALE: Sternoclavicular joint septic arthritis is an unusual disease in healthy adults, and Staphylococcus aureus is the most common causative pathogen. The current treatment of choice is surgery with sternoclavicular joint resection and pectoralis flap closure, especially when the disease is complicated by osteomyelitis and abscess. PATIENT CONCERNS: Here, we report a 76-year-old woman without risk factors who visited our hospital for pain and redness, swelling on the left anterior chest wall. DIAGNOSIS: Magnetic resonance imaging showed infectious arthritis in the left SCJ, with multip

Acute phospholipid microspherule associated arthritis: Is it rare?

Joint fluid analysis must be performed as part of the diagnostic workup for acute arthritis, most notably to rule out septic arthritis and to allow the identification of crystal-induced arthritis (gout or calcium pyrophosphate deposition disease), which is one of the most common causes. However, the detection of monosodium urate or calcium pyrophosphate microcrystals is not the only goal of the polarized light microscopy examination of joint fluid. Other, less common microcrystals may be found. Among them are phospholipid microspherules, which are easily recognized microscopically based on the

The child with joint pain in primary care.

Joint pains are a common reason for children to present to primary care. The differential diagnosis is large including some diseases that do not primarily affect the musculoskeletal system. Although the cause for many patients will be benign and self-resolving, in rare cases the diagnosis is associated with long-term morbidity and mortality if not detected early and appropriately treated. These include primary and secondary malignancies, septic arthritis, osteomyelitis, inflammatory arthritis, slipped upper femoral epiphysis (SUFE) and non-accidental injury. We highlight the importance of a th

Musculoskeletal manifestations of human immunodeficiency virus infection.

Musculoskeletal manifestations of the human immunodeficiency virus (HIV) are common and are sometimes the initial presentation of the disease. Knowledge of the conditions affecting muscle, bone, and joints in HIV-infected patients is essential for successful management. Myopathies may be caused by pyogenic infection (eg, pyomyositis), idiopathic inflammation (eg, polymyositis), or drug effect (eg, AZT myopathy). Characteristic skeletal infections, such as tuberculosis and bacillary angiomatosis, require a high index of suspicion for accurate diagnosis. Neoplastic processes, such as non-Hodgkin

Benign polyarthritis and undifferentiated arthritis an epidemiological terra incognita.

Introduction of new diagnostic criteria and the development of HLA-typing and microbiological--serological techniques have aided in classifying and differentiating inflammatory rheumatic diseases. Reviewing epidemiological prevalence studies shows that inflammatory rheumatic diseases are often so called benign polyarthritides and that arthritides, classified as definite and probable rheumatoid arthritis (RA), may belong to a group of benign or undifferentiated arthritides. Own data show that only 25% of patients of an early synovitis out-patient clinic have definite or probable RA, 25% have an

Anti-type II collagen antibody in naturally occurring canine joint diseases.

Autoimmunity to collagen was investigated in several naturally occurring arthropathies of the dog. Increased levels of serum anti-native collagen type II antibody, as assessed by ELISA, were shown in 72.4% of dogs with rheumatoid arthritis (RA), 88% of dogs with infective arthritis (IA) and 52% of dogs with osteoarthritis (OA) (p less than 0.001). The mean levels of antibody in cruciate disease patients (CR) were also significantly increased compared to control dogs (p less than 0.01). Serum anti-collagen antibody in OA dogs correlated with that in precipitated serum immune complexes. There wa

Shoulder arthritis. Distinguishing among the many causes of inflammation.

Shoulder inflammation occurs in many rheumatic diseases. It is present in about half of the patients with rheumatoid arthritis but in a higher percentage of patients with less common diseases (eg, amyloid arthropathy, polymyalgia rheumatica). Shoulder arthritis, probably more than arthritis involving any other joint, requires prompt diagnosis and treatment to prevent loss of motion and function. For most patients, antiinflammatory medications, range of motion exercises, and instruction in joint protection constitute adequate treatment. A baseline shoulder roentgenogram is usually recommended.
