Elimination of fetal scalp blood sampling on a large clinical service.

OBJECTIVES: To describe the use of fetal scalp blood sampling on a large teaching service over 7 years and to assess any association between changes in use and the rates of cesarean delivery for fetal distress and of various indirect indicators of perinatal asphyxia in term infants. METHODS: We reviewed computerized and tabular data bases for fetal scalp blood sampling, cesarean delivery for fetal distress, Apgar score, and the clinical diagnoses of asphyxia and meconium aspiration syndrome for the years 1986-1992. RESULTS: Live births averaged 16,330 annually. The rate of fetal scalp blood

[Neonatal birth trauma in relation to the means of delivery].

The frequency of birth trauma has fallen considerably in recent years. This decline reflects in particular a greater tendency to perform a caesarean section when difficulties of delivery would appear. Birth trauma often causes not only early, but late and continuous illness in neonates. For this reason it is an important problem in neonatal practice. 1791 newborn have been investigated: 1515 (84.5%) by vaginal delivery, 177 (9.8%) by caesarean section, 38 (2.1%) by forceps, 18 (1.01%) by vacuum extraction and 43 (2.4%) by pelvic presentation. The results show the frequency of birth trauma (5.5

[Complications and peculiarities of anesthesia in abdominal delivery].

Analysis of the reasons responsible for complications of anesthesia in abdominal delivery has shown that such complications are most frequent before fetus extraction. They are: poorly corrected hypo- or hypertension and tachycardia; damages of major vessels, pleura and lungs upon central vein puncture; severe hypoxia in difficult or abortive intubations with mouth, larynx, pharynx, trachea and esophagus injuries; regurgitation and development of aspiration pneumonia (Mendelson syndrome); toxic and allergic reactions to psychopharmacological, narcotic and local anesthetic agents; signs of laryn

Twin labour complicated by umbilical cord knotting.

We report a case of intrapartum distress of the first twin due to a knot in the umbilical cord. Cord blood flow is reduced according to Poiseuille's law as the knot tightens.

Mechanisms of perinatal brain damage.

Although the perinatal brain is more resistant towards hypoxia-ischemia than at any later stage of development, perinatal asphyxia still contributes to permanent brain damage in survivors. This short review focuses on the discussion of mechanisms of late neuronal death, i.e. cerebral damage related to lack of oxygen but caused by mechanisms during the stage of reoxygenation. Evidence is presented to indicate that the transient, dense glutamatergic innervation of basal structures of the perinatal brain may induce brain damage via overflow of excitatory amino acids during oxygen lack, causing a

The paradox of adult respiratory distress syndrome in neonates.

Six full-term newborn infants are described who suffered from severe adult respiratory distress syndrome (ARDS). The triggering event was intrauterine/perinatal asphyxia in five, and group B streptococcal (GBS) septicemia in three. All had severe respiratory distress/failure and were ventilated mechanically with high concentrations of inspired oxygen and positive end-expiratory pressure. Radiography of the chest showed dense bilateral consolidation with air bronchograms and reduced lung volume. Persistent pulmonary hypertension (PPH) was documented in all cases. The coincidence of ARDS and PPH

Neonatal hypoxic-ischemic encephalopathy: current management practices.

A national survey of training programs in neonatal-perinatal medicine was undertaken to determine neurologic management practices in dealing with the asphyxiated, term newborn. Seventy-two of 105 responses (68.6 per cent) were returned. Commonly used diagnostic tests include routine electroencephalography, cranial sonography, and determinations of arterial blood gases and serum calcium. Fluid restriction and treatment of seizures with phenobarbital are widely used therapies. Many available techniques for both diagnosis and treatment show wide variability in utilization. These results demonstra

Prediction of long-term outcome in high-risk infants: the use of objective measures of brain structure and function in the neonatal intensive care unit.

The ability to predict long-term outcome on the basis of objective measures made shortly after the birth of an infant has introduced a completely new approach to the investigation of the aetiology of childhood impairments and disabilities, and the evaluation of the effects of perinatal management regimes designed to avoid or ameliorate them. This approach also has important implications for the management of sick and vulnerable infants, both in the perinatal period and later. From the use of ultrasound brain scanning, a great deal has been learnt about all aspects of the aetiology, evolution a

Retinal dystrophy in Jeune's syndrome.

Jeune's syndrome is an autosomal-recessive condition characterized by multiple organ abnormalities, the most severe of which affect the skeleton, kidneys, and eyes. Severe respiratory insufficiency frequently results in death in infancy, but several patients have been reported to survive to adulthood. For this reason the prognosis for patients with ocular abnormalities is of interest in counseling families. We report a sibship of two patients who showed evidence for progression by visual field and electroretinogram testing when followed up over 3.7 years. Progressive electroretinographic abnor

The neuropathology of kernicterus in the premature neonate: diagnostic problems.

Yellow staining of central nervous system (CNS) nuclei occurs in the brains of some neonates, despite low levels of serum bilirubin. Two conditions appear to be important in the evolution of this form of kernicterus: prematurity and asphyxia. In a seven year retrospective study of a large neonatal autopsy population, 102 cases had kernicterus as indicated by selective macroscopic yellow staining and microscopic damage within specific CNS nuclei. Neuropathological study disclosed minor variations and numerous similarities in the manifestations of kernicterus in the asphyctic premature neonate w

Birth asphyxia. I. Measurement of visual evoked potential (VEP) in the healthy fetus and newborn lamb.

The visual evoked potential (VEP) of five healthy, near-term exteriorized fetal lambs was compared with the VEP obtained from the same animals during the first hour after ventilation. Characteristics of the individual wave components and their relationship to blood pressure, heart rate, and arterial blood gas measurements concomitantly obtained were examined. Results show that the VEP of the healthy fetus is a stable, reproducible signal consisting of four basic components, and that the latencies (msec) and the amplitude (mu V) of the individual components of the VEP are similar to those of th

[Vascular thrombosis after catheterization of the umbilical artery].

Two newborns with vascular thromboses due to catheterization of the umbilical artery are reported; one of them died and in the other one it was necessary to amputate the leg. Considerations are made on the predisposing factors for this complications, as well as the indications and precautions for this technic.

The effect of birth asphyxia on plasma free amino acids in preterm newborn infants.

The postnatal plasma amino acid pattern was compared in 16 asphyxiated and 13 non-asphyxiated preterm newborn infants. The lactic acidosis induced by asphyxia was associated with a marked rise in the total amino acid content of the plasma. Among the 17 individual amino acids determined the concentrations of alanine, proline, taurine, glutamate, valine, methionine and lysine were significantly elevated. The accumulation of alanine was particulary marked and its concentration showed a significant linear correlation with that of lactate (p less 0.001). A similar relationship was observed between
