Atrial fibrillation in transthyretin amyloid cardiomyopathy: Mechanisms, prognostic implications, and management.

Atrial fibrillation (AF) is common in transthyretin amyloid cardiomyopathy (ATTR-CM), affecting over 70% of patients and often preceding cardiomyopathy diagnosis. Amyloid infiltration promotes structural, mechanical, and electrical atrial remodeling, creating an arrhythmogenic substrate and increasing thromboembolic risk that may be independent of Congestive heart failure, Hypertension, Age, Diabetes, Stroke, Vascular disease, Age, and Sex category score, supporting a low threshold for anticoagulation. AF management in ATTR-CM is challenging because restrictive physiology, autonomic dysfunctio…

Antithrombotic management in left sided- ablation and appendage device-closure procedures.

Atrial fibrillation is the most prevalent sustained cardiac arrhythmia, and the therapeutic landscape for stroke prevention has expanded to include catheter ablation for rhythm control and left atrial appendage closure in patients unsuitable for long-term oral anticoagulation. Although minimally invasive, these interventions present complex thrombotic challenges. The left atrial and appendage environment is inherently prothrombotic due to structural remodeling, endothelial injury, blood stasis, and abnormal hemostasis-processes further amplified by procedural instrumentation such as transsepta

Chromosome 4q25 Variant rs6817105 Bring Sinus Node Dysfunction and Left Atrial Enlargement.

Genome-wide association studies have reported a strong association of the single nucleotide polymorphism (SNP) rs6817105 (T > C) on chromosome 4q25 with atrial fibrillation (AF), but phenotype alterations conferred by this SNP have not been described. We genotyped SNP rs6817105 and examined the relationships among rs6817105 genotype, clinical characteristics, echocardiographic parameters, and electrophysiological parameters in 574 AF patients and 1,554 non-AF controls. Further, multiple microRNAs (miRNAs) are reported to be involved in atrial remodeling and AF pathogenesis, so we investigated …

[Atrial fibrillation screening-reading the P-wave].

BACKGROUND: Atrial fibrillation (AF) is frequently intermittent and may therefore remain undetected on a short resting electrocardiogram (ECG). At the same time, a sinus-rhythm ECG contains information on the underlying atrial substrate. OBJECTIVE: To review conventional and artificial intelligence (AI)-based ECG markers of an atrial risk phenotype and their potential role in targeted AF screening. MATERIALS AND METHODS: Narrative appraisal of current consensus and guideline documents and of clinical, electrophysiological, and population-based studies on P‑wave parameters, AI-ECG, and risk-a

Optimising risk factors for atrial fibrillation post-cardiac surgery.

Postoperative atrial fibrillation (POAF) is an ongoing complication following cardiac surgery, with an incidence of 15%-60%. It is associated with substantial mortality and morbidity, as well increased hospital stays and healthcare costs. The pathogenesis is not fully understood, but the literature suggests that POAF occurs when transient, postoperative triggers act on vulnerable atrial tissue produced by preoperative, procedure-induced and postoperative processes such as inflammation, oxidative stress, autonomic dysfunction and electrophysiological remodelling of the atrial tissues. This sets

Galectin-3: A biochemical marker to detect paroxysmal atrial fibrillation?

PURPOSE: Atrial fibrillation (AF) is the most common form of arrhythmia. AF leads to electrical remodelling and fibrosis of the atria; however, the mechanism(s) remain poorly understood. Galectin-3 is a potential mediator of cardiac fibrosis. The present study aimed to examine the relationship between serum galectin-3 levels and paroxysmal AF. METHODS: Forty-six patients with paroxysmal AF and preserved left ventricular systolic function, and 38 age- and gender-matched control subjects, were involved in the study. Serum galectin-3 levels were analyzed with an enzyme-linked immunosorbent assay

Relationship between left atrial fibrosis and function in a selected cohort undergoing surgery for severe primary mitral regurgitation.

BACKGROUND: In primary mitral regurgitation (MR) atrial dysfunction is believed to be the result of atrial interstitial fibrotic replacement, and left atrial (LA) strain has been suggested as a non-invasive tool to identify a maladaptive atrial remodelling beyond LA enlargement. We aim to investigate the association between LA function and the extent of interstitial fibrosis within the atrial wall in patients undergoing surgery, and their relation with MR characteristics and clinical profiles. METHODS: 49 consecutive patients (61 ± 10 years, men 75%) with severe primary MR due to prolapse or

Dronedarone produces early regression of myocardial remodelling in structural heart disease.

BACKGROUND AND AIMS: Left ventricular hypertrophy (LVH) in hypertension is associated with a greater risk of sustained supraventricular/atrial arrhythmias. Dronedarone is an antiarrhythmic agent that was recently approved for the treatment of atrial fibrillation. However, its effect on early regression of LVH has not been reported. We tested the hypothesis that short-term administration of dronedarone induces early regression of LVH in spontaneously hypertensive rats (SHRs). METHODS: Ten-month-old male SHRs were randomly assigned to an intervention group (SHR-D), where animals received droned

Atrial fibrillation and its arrhythmogenesis associated with insulin resistance.

BACKGROUND: Insulin resistance (IR) is considered as a risk factor for atrial fibrillation (AF) even before diabetes develops. The pathophysiology and underlying mechanism are largely unclear. METHODS: We investigated the corresponding mechanism in two IR models of rats fed 15-week high-fat (HFa) and high-fructose/cholesterol (HFr) diets. AF was evaluated and induced by burst atrial pacing. Isolated atrial myocytes were used for whole-cell patch clamp and calcium assessment. Ex vivo whole heart was used for optical mapping. Western blot and immunofluorescence were used for quantitative protei

Blockade of the renin-angiotensin-aldosterone system (RAAS) for primary prevention of non-valvular atrial fibrillation: a systematic review and meta analysis of randomized controlled trials.

BACKGROUND: The renin-angiotensin-aldosterone system is suggested to play a key role in the development of atrial fibrillation through structural and electrical remodeling. This review aims to assess the effectiveness of blockade of the renin-angiotensin-aldosterone system in the prevention of new onset atrial fibrillation. METHODS: Electronic databases from 1984 onwards and previous systematic reviews were searched in addition to contacting experts in the field. Two reviewers independently selected eligible randomized controlled trials that reported on new onset atrial fibrillation, and that

The aging process of the heart: obesity is the main risk factor for left atrial enlargement during aging the MONICA/KORA (monitoring of trends and determinations in cardiovascular disease/cooperative research in the region of Augsburg) study.

OBJECTIVES: This prospective study evaluated the association of obesity and hypertension with left atrial (LA) volume over 10 years. BACKGROUND: Although left atrial enlargement (LAE) is an independent risk factor for atrial fibrillation, stroke, and death, little information is available about determinants of LA size in the general population. METHODS: Participants (1,212 men and women, age 25 to 74 years) originated from a sex- and age-stratified random sample of German residents of the Augsburg area (MONICA S3). Left atrial volume was determined by standardized echocardiography at baselin

Effects of Intrinsic and Extrinsic Cardiac Nerves on Atrial Arrhythmia in Experimental Pulmonary Artery Hypertension.

Atrial arrhythmia, which includes atrial fibrillation (AF) and atrial flutter (AFL), is common in patients with pulmonary arterial hypertension (PAH), who often have increased sympathetic nerve activity. Here, we tested the hypothesis that autonomic nerves play important roles in vulnerability to AF/AFL in PAH. The atrial effective refractory period and AF/AFL inducibility at baseline and after anterior right ganglionated plexi ablation were determined during left stellate ganglion stimulation or left renal sympathetic nerve stimulation in beagle dogs with or without PAH. Then, sympathetic ner

Post-transplant atrial tachyarrhythmias: Epidemiology, mechanisms, outcomes, and management across solid organs.

Post-transplant atrial tachyarrhythmias - including atrial fibrillation (AF), atrial flutter, and organized atrial tachycardias - occur afte … chycardias at anastomotic or incisional sites. These late arrhythmias can signal graft pathology or atrial remodeling. In heart transplantation, late atrial tachyarrhythmias depend on surgical technique and often coincide with rejection or cardiac allograft vasculopathy. In abdominal transplantation, AF is less common but remains relevant. In liver transplantation, …

X-ray Radiotherapy Impacts Cardiac Dysfunction by Modulating the Sympathetic Nervous System and Calcium Transients.

Recent epidemiological studies have shown that patients with right-sided breast cancer (RBC) treated with X-ray irradiation (IR) are more susceptible to developing cardiovascular diseases, such as arrhythmias, atrial fibrillation, and conduction disturbances after radiotherapy (RT). Our aim was to investigate the mechanisms induced by low to moderate doses of IR and to evaluate changes in the cardiac sympathetic nervous system (CSNS), atrial remodeling, and calcium homeostasis involved in cardiac rhythm. To mimic the RT of the RBC, female C57Bl/6J mice were exposed to X-ray doses ranging from …

MicroRNA as a Therapeutic Target in Cardiac Remodeling.

MicroRNAs (miRNAs) are small RNA molecules that contain 18-25 nucleotides. The alterations in their expression level play crucial role in the development of many disorders including heart diseases. Myocardial remodeling is the final pathological consequence of a variety of myocardial diseases. miRNAs have central role in regulating pathogenesis of myocardial remodeling by modulating cardiac hypertrophy, cardiomyocytes injury, cardiac fibrosis, angiogenesis, and inflammatory response through multiple mechanisms. The balancing and tight regulation of different miRNAs is a key to drive the cellul

Electrical Remodeling in Right Ventricular Failure Due to Pulmonary Hypertension: Unraveling Novel Therapeutic Targets.

Arrhythmias in the setting of right-ventricular (RV) remodeling contribute to majority of deaths in patients with pulmonary hypertension. However, the underlying mechanism of electrical remodeling remains elusive, especially ventricular arrhythmias. Here, we analyzed the RV transcriptome of pulmonary arterial hypertension (PAH) patients with compensated RV or decompensated RV and identified 8 and 45 differentially expressed genes known to be involved in regulating the electrophysiological properties of excitation and contraction of cardiac myocytes, respectively. Transcripts encoding voltage-g
