Melatonin Increases Bone Mass around the Prostheses of OVX Rats by Ameliorating Mitochondrial Oxidative Stress via the SIRT3/SOD2 Signaling Pathway.

Bone mass loss around prostheses is a major cause of implant failure, especially in postmenopausal osteoporosis patients. In osteoporosis, excess oxidative stress largely contributed abnormal bone remodeling. Melatonin, which is synthesized from the pineal gland, promotes osteoblast differentiation and bone formation and has effectively been used to combat oxidative stress. Thus, we determined if melatonin can inhibit oxidative stress to promote osteogenesis and improve bone mass around prostheses in osteoporosis. In this study, we observed that received melatonin at 50 mg/kg body weight signi

Reconstruction of compound tibial and soft tissue loss using a traction histogenesis technique.

We used simultaneous bone and soft tissue transport for reconstruction of large compound tissue loss in the lower leg. We report the results and complications of a 12-patient series. The average age of the patients was 31.2 years (range 20 to 48 years). Seven patients had grade IIIB open tibial fractures, three had complications after grade II or IIIA, and two had an en-bloc resection of bone, muscle, and skin for a malignant bone tumor. Arteriography was routinely performed, and demonstrated at least one patent tibial artery. Every patient had normal sensibility of the sole. The first stage o

Autograft versus sterilized allograft for lateral calcaneal lengthening osteotomies: Comparison of 50 patients.

Sterilized allografts may be less resistant to collapse and prone to nonunion leading to loss of correction in open wedge osteotomies. These adverse events usually occur at early time points (i.e., < 9 months postoperatively). The goal of this study was to compare sterilized allografts to autologous grafts in respect to secondary loss of hindfoot alignment and graft incorporation after lateral calcaneal lengthening osteotomies.Fifty patients (22 F/ 28 M, age: 16-69 years) who had undergone 50 lateral calcaneal lengthening osteotomies for adult flatfoot deformity were included in this retrospec

Femoral and tibial lengthening.

Results and complications are reviewed in 18 femoral and seven tibial lengthenings performed using Wagner's technique. Lengthening achieved averaged 5.7 cm for the femoral and 5.6 cm for the tibial patients. Shortening was congenital in origin in 12 patients, and a soft tissue excision is recommended for these individuals. Two nonunions developed and two stress fractures occurred after plate removal. Pin tract infections occurred in 15 patients and deep infection in three. No serious long-term sequelae of infection have been seen at follow-up averaging 3 years 3 months. Additional complication

[Tissue response in circum-maxillary sutures after transverse palated suture expansion].

OBJECTIVE: This study was carried out to investigate the histological reactions of midfacial sutures induced by transverse palatal suture expansion. METHODS: Nine mongrel dogs of 8-week old were used in the study. Six of them were used as experimental group as described in a previous paper. The additional three served as controls. In the experimental dogs, a NiTi-shape memory alloy (NiTi-SMA) arch wire was used to expand the transverse palatal suture. Tetracycline was injected intramuscularly (50 mg/kg) three days before operation and at the time of sacrifice. Tissue reactions of premaxillo-m

Polymethyl methacrylate-based bone cement using a prototype for gingival smile: A case report.

BACKGROUND: The treatment of gummy smile (GS) with polymethyl methacrylate (PMMA)-based implants is suggested in specific cases such as those in which there is a lack of lip support due to a marked depression of the anterior maxillary process; however, it can be associated with certain complications related to the material and the surgical approach. METHODS: A patient with a GS of combined etiology was treated with clinical crown lengthening and lip repositioning surgery with placement of a PMMA-based bone cement implant to fill the subnasal depression. Surgical planning was performed using a

A 32-year follow-up of lunate excision for Kienbock's disease: a case report and a review of results from excision and other treatment methods.

Lunate excision alone is seldom utilized in the management of Kienbock's disease due to concerns about progressive carpal collapse following removal of this central carpal bone. We report a 32-year follow-up of a patient who underwent lunate excision only for treatment of Kienbock's disease with a successful outcome. Although lunate excision is thought to be associated with a high failure rate, a review of the literature suggests that success rates following lunate excision are comparable to those reported for other more conventional techniques such as radial shortening, ulnar lengthening, lim

Silver calcaneal osteotomy for flexible flatfoot: a retrospective preliminary report.

There are many procedures in vogue today for the surgical correction of pathologic pes plano valgus deformity. The Silver opening wedge calcaneal osteotomy is utilized to correct excessive heel valgus and attempts to realign the heel under the weight-bearing surface of the tibia. It has no effect on the position or function of the subtalar joint. Midtarsal joint stress is decreased and the foot retains its ability to adapt to uneven terrain. The procedure is rarely used alone and is most frequently combined with heel cord lengthening or gastrocnemius tendon recession. In this presentation, the

Challenges of bone tissue engineering in orthopaedic patients.

Bone defects may impede normal biomechanics and the structural stability of bone as an organ. In many cases, the correction of bone defects requires extensive surgical intervention involving the use of bone-grafting techniques and other procedures in which healing is slow, there is a high risk of infection and considerable pain is provoked - with no guarantee of complete correction of the defect. Therefore, the search for surgical alternatives continues to present a major challenge in orthopaedic traumatology. The reamer-irrigator-aspirator (RIA) system, which was devised to avoid the problems

Treatment of Severe Maxillary Hypoplasia With Combined Orthodontics and Distraction Osteogenesis.

Distraction osteogenesis (DO) is a technique that allows the generation of new bone in a gap between 2 vascularized bone surfaces in response to the application of graduated tensile stress across the bone gap.Distraction osteogenesis has become a routine treatment of choice to correct skeletal deformities and severe bone defects in the craniofacial complex over the past decade. Distraction osteogenesis has been successfully chosen in lengthening the maxilla and the mandible; in the maxilla and recently in the mandible, the jawbones have been distracted and widened transversely to relieve sever

Physiological and Pharmacological Roles of PTH and PTHrP in Bone Using Their Shared Receptor, PTH1R.

Parathyroid hormone (PTH) and the paracrine factor, PTH-related protein (PTHrP), have preserved in evolution sufficient identities in their amino-terminal domains to share equivalent actions upon a common G protein-coupled receptor, PTH1R, that predominantly uses the cyclic adenosine monophosphate-protein kinase A signaling pathway. Such a relationship between a hormone and local factor poses questions about how their common receptor mediates pharmacological and physiological actions of the two. Mouse genetic studies show that PTHrP is essential for endochondral bone lengthening in the fetus a…

Jansen's Disease: Bone Abnormalities Beyond Chondrodysplasia.

CONTEXT: Jansen metaphyseal chondrodysplasia (JMC) is an ultra-rare autosomal dominant disease that is caused by heterozygous, activating PTH1R mutations resulting in PTH- and PTHrP-independent hypercalcemia and hypercalciuria, leading to nephrocalcinosis and impaired renal function later in life. The activated PTH1R plays critical roles in mineral ion homeostasis and bone lengthening, as well as bone formation and resorption. Currently, little is known about bone turnover markers and bone histomorphometric changes in JMC patients. OBJECTIVE: This study aimed to assess changes in bone microar…

Postoperative limb lengthening following Total Hip Arthroplasty (THA) through a posterior approach--a challenge for the orthopaedist or physiotherapist?

BACKGROUND: As coxarthrosis progresses, pain and mobility limitation exacerbate, usually presenting as a progressive flexion-abduction contracture. This, combined with the degradation of articular ends of bones, is responsible for anatomical and functional shortening of the limb. The sensation of postoperative leg length difference is a significant problem both for the patient and the operator. A sense of the operated limb being longer may be due to excessive length of the femoral segment following total hip arthroplasty. It may also result from a postoperative abduction or flexion-abduction c

[Treatment of radial agenesis with the Ilizarov method].

PURPOSE OF THE STUDY: Typical cases of lateral longitudinal hemimelia of the upper limb are characterized by agenesia of the radius and the thumb. The hand has four fingers and due to the lack of a radius, deviates laterally beyond 90 degrees. The ulna is short in 50 to 60% of the cases. Hand function is very limited causing a major handicap in everyday life activities. We report here the long-term outcome of 14 patients treated with the Ilizarov method. MATERIAL AND METHODS: Fourteen cases in thirteen patients, mean age 14.8 years, with radial hemimelia (one bilateral surgery) were treated w

Correlation between Risk Factors and Healing Times in Long Bone Nonunions Treated with Corticoperiosteal Flap from the Medial Femoral Condyle.

BACKGROUND: The rate of fracture nonunion varies depending on the anatomical site. Numerous procedures have been proposed to treat recalcitrant nonunions. The vascularized medial femoral condyle corticoperiosteal free flap (MFCCFF) is increasingly used in nonunions with small bone loss. The percentage of success of the MFCCFF is high but the factors involved in delayed bone healing or failure of this technique or the contraindications are not described in the literature. This multicentric study aims to identify and report the different factors involved in determining the time of bone healing in

Acute shortening and re-lengthening (ASRL) procedure combined with RhBMP-2 to treat femur non-union with leg length discrepancy: a case report.

BACKGROUND: Femoral shaft non-union complicated by leg length discrepancy (LLD) and segmental bone loss remains a significant reconstructive challenge. Conventional bone transport techniques may involve long external fixation times, multiple surgeries, and soft-tissue morbidity. The acute shortening and re-lengthening (ASRL) method offers an alternative by achieving immediate bone contact and tissue closure, followed by gradual restoration of length. The adjunctive use of recombinant human bone morphogenetic protein-2 (rhBMP-2) may enhance osteogenesis at the docking and regenerate sites. CAS

Inflammatory-Driven Angiogenesis in Bone Augmentation with Bovine Hydroxyapatite, B-Tricalcium Phosphate, and Bioglasses: A Comparative Study.

INTRODUCTION: The clinical use of bioactive materials for bone augmentation has remained a challenge because of predictability and effectiveness concerns, as well as increased costs. The purpose of this study was to analyse the ability to integrate bone substitutes by evaluating the immunohistochemical expression of the platelet endothelial cell adhesion molecules, vascular endothelial growth factor, collagen IV, laminin, and osteonectin, in the vicinity of bone grafts, enabling tissue revascularization and appearance of bone lamellae. There is a lack of in vivo studies of inflammatory-driven
