Basing on the earlier experience with neurovascular flap from the dorsal part of the index when reconstructing soft tissues of thumb and thenar, first of the authors elaborated a method of harvesting this flap together with a dorsal aponeurosis and bone fragment of the proximal phalanx. Blood flow comes from the periosteum of the proximal phalanx. Many small vessels penetrate into it from the dorsal soft tissues thus piercing central part of the dorsal aponeurosis in many points. The flap was used in 25 year old male who had secondary reconstruction of the mutilating thumb pulp when the nail a

Nonunion after diaphyseal fracture of the femur or the tibia is a common but difficult complication for treatment. Currently, the main treatment modalities include nail dynamization, exchange nailing, and bone transport, but revision with compression plating in these nonunions was rarely reported. To evaluate the outcomes of compression plating in the treatment of femur and tibia shaft nonunions, we retrospectively reviewed 54 patients with diaphyseal nonunion of the tibia or the femur treated with locking compression plate (LCP) by compression technique. There were 46 aseptic and 8 septic non

Acral lentiginous melanoma affecting the nail is uncommon but carries a poor prognosis due to difficulties in early diagnosis. The gold standard of treatment for subungual melanoma is biopsy followed by wide local excision in form of amputation of the distal phalanx of the digit, in order to achieve at least 10 mm margin of clearance or by fixed tissue micrographic (Mohs') surgery. Here, we demonstrate a non-amputative approach for the excision of subungual melanoma in situ of the right great toe, involving removal of the nail unit with a layer of underlying bone before reconstruction with ful

Nail Patella Syndrome (NPS) is a rare hereditary disease affecting multiple systems with predominant involvement of Kidney, Bones and Nails and Eyes. We report a case of NPS which presented as renal failure in a 22 year old male. The patient was admitted with decreased urine output and features of fluid overload and was being evaluated for renal failure. Physical examination revealed associated bony deformities which raised the suspicion of NPS as a possible etiology. This was confirmed by the radiological evaluation which showed the classical features of NPS. Though NPS is a rare clinical con

HOOD syndrome is a rare genetic disorder also known as nail patella syndrome. Biochemical and molecular biological data are rare and not conclusive. Preliminary data suggest the involvement of collagen type IV. This would, however, not be enough to explain nail or bone involvement. As kidney pathology would be consistent with a cross-linking disorder, we tested collagen IV and kidney collagen cross-linking on SDS-PAGE. This method showed a remarkable reduction in high-molecular-weight collagen polymers. The patient's and his mother's urinary pyridinoline cross-link excretion was manifold incre

The results of 31 fractures of the tibial shaft treated with closed Ender nailing and early weight-bearing are reported. Twenty-five of these fractures were closed and six open. All fractures united between 6 and 16 weeks after operation. No case of deep wound infection was encountered. Comminuted fractures of the proximal and distal third were prone to angular deformities and needed cast protection post-operatively. Nail reinsertion became necessary in two patients. Delay of operation renders closed reduction very difficult. The advantages of this method are: 1) normal knee and ankle movement

Intramedullary tibial fixation is a commonly used and highly successfully treatment in acute fractures, nonunion settings, and correctional procedures. In the setting of a nonunion, removal of a failed implant can add to operative time and surgeon frustration while further compromising bone in an area already at risk for failure. Here we present a technique, using readily accessible equipment, for removing a distal solid tibial nail fragment in a manner that preserves bone.

Background: Displaced proximal humerus fractures (PHFs) in adults represent a common orthopedic challenge, especially in elderly patients with compromised bone quality. Intramedullary nails (IM) and locking plates (LP) are the most widely used fixation techniques, though the literature remains inconclusive regarding their comparative efficacy. Methods: This retrospective cohort study included 187 patients (mean age: 65.4 years) treated surgically for Neer ≥ 2-part proximal humerus fractures at a single tertiary referral center between 2018 and 2023. Patients underwent either IM nailing or LP f

Atypical femoral fractures (AFFs) are rare complications in children receiving bisphosphonate therapy for bone disorders. Early-onset osteoporosis in this population is often linked to monogenic bone diseases caused by genetic mutations. This study aims to highlight the risk of AFFs associated with bisphosphonate therapy in pediatrics and review the treatment approaches for these fractures. We report two cases of AFFs in nine-year-old identical twins with early-onset osteoporosis secondary to a lipoprotein receptor-related protein 5 (LRP5) gene mutation. Both had been receiving long-term zoled

BACKGROUND: A complication associated with the distal locking screw used in the proximal femoral nail (PFN) system is the formation of accidental additional holes. We hypothesized that an increase in stress around additional holes is a relevant factor contributing to fractures. This study aimed to evaluate stress changes in the cortical bone around additional screw holes using finite element analysis.
METHODS: Proximal femoral nail antirotation (PFNA)-II (Synthes, Solothurn, Switzerland) was inserted into a femur model. An additional 4.9-mm transcortical hole was made either anteriorly (anter

We evaluated the long-term functional and cosmetic results of homodigital neurovascular island flap (NIF) used to reconstruct extensive pulp defects with bone exposure in children. Twenty-three children (mean age 4.8 years, range 1-10 years) with fingertip injuries were reconstructed with a pedicled homodigital NIF and evaluated in terms of sensation quality, cold intolerance, scar formation, nail deformity, range of motion and overall finger length at a mean follow up of 7.8 years (range 2-13). Eleven patients reported cold sensitivity in the operated fingertip, and 15 presented with hook nai

Femoral fractures in childhood are rare and are usually the result of severe trauma. Conservative treatment (overhead extension, hip spica cast) can be used in children up to 3 years of age and beyond that elastic stable intramedullary nailing (ESIN) is the method of choice. The prognosis is very good if the surgical technique is adequate. In adolescents > 50 kg in body weight solid nails implanted via the greater trochanter are an alternative (adolescent lateral femoral nail, ALFN). External fixators and plate osteosynthesis are indicated in special situations. Femoral neck fractures are very

The various Wnt pathways that are related to upper limb anomalies are reviewed. Abnormalities in the Wnt7a pathway (located in the dorsal ectoderm) produce several clinically relevant conditions such as the palmar duplication syndrome, nail patella syndrome, ulnar ray deficiency, limb hypoplasia, polysyndactyly and the palmar nail syndrome. Abnormalities of the Wnt3/3a pathway (located in the apical ectodermal ridge) include tetra-amelia and loss of the distal phalanges/nails. Abnormalities of the Wnt5/5a pathway (located in the apical ectodermal ridge as well as in the mesoderm) will affect c

Between 1992 and 1996, 73 acute humeral shaft fractures in 71 patients were treated with antegrade Russell-Taylor humeral nailing. Sixty (84%) patients sustained multiple trauma, and 26 (36%) fractures were open. Three patients had preoperative brachial artery injuries, and 12 had preoperative nerve palsies. Sixty-nine (94.5%) fractures united primarily, and two additional fractures united after bone grafting. There were no infections. Two (2.7%) iatrogenic radial nerve palsies occurred, and both were transient. Full shoulder function returned in 66 (90%) patients. Two (2.7%) patients had impi

UNLABELLED: Tibiotalocalcaneal arthrodesis with an intramedullary nail is a procedure reserved for patients who have conditions affecting the ankle joint and subtalar joint. The most common complications include nonunion, malunion, delayed union, infection, periprosthetic fracture, hardware failure, lateral plantar nerve injury, and wound healing. Significant bone void may result if the hardware requires removal secondary to one of these complications. The purpose of this article is to report on 2 cases of infected intramedullary nails, which were revised with fibular inlay strut grafts. Both
