Bronchial Leiomyoma: Importance of Preoperative Biopsy for Lung Preserving Surgery.

Tracheobronchial leiomyoma is a rare tumor of the airway. They arise from the lower respiratory tract tissue of the bronchi, trachea, and lung. Symptomatology is based on the degree of endoluminal bronchial obstruction, and surgical resection is generally the mainstay of treatment. We present the case of a 33-year-old male who suffered from chronic cough and breathlessness for two years caused by large endobronchial leiomyoma diagnosed by preoperative biopsy. The tumor was surgically resected through bronchotomy and complete preservation of the lung parenchyma. We stress the importance of a de…

A Large Pericardial Effusion Culminating in Left Lung Collapse.

Pericardial effusion is characterized by excess fluid accumulation in the pericardium. It can be asymptomatic or silent when the effusion is trivial in size or develops slowly. On the other hand, large rapidly developing effusions may present with hemodynamic instability or tamponade. In rare circumstances when a large effusion develops over a period of time, it may cause compression atelectasis of the surrounding bronchi and lung. We describe the case of a 70-year-old female who presented with acute respiratory insufficiency due to left lung collapse secondary to large pericardial effusion. T…

Tracheobronchopathia Osteochondroplastica as a Single Mass in Lingular Bronchus.

We describe a rare case of tracheobronchopathia osteochondroplastica (TBO) in a 35-year-old man who presented with recurrent chest infection and hemoptysis. Bronchoscopy and computed tomography scan revealed a single 1.5-cm hard nodular mass lesion in the lingular bronchus associated with collapse and bronchiectasis of the lingula. The patient underwent open left lingulectomy with removal of the mass. Histopathology of the lesion showed characteristic features of TBO. TBO is generally characterized by multiple nodules in the trachea or main bronchi. The presentation of TBO as a single dominant…

Eosinophilic COPD - a distinct phenotype of the disease.

Chronic Obstructive Pulmonary Disease (COPD) has been traditionally associated with neutrophilic inflammation of the bronchi. Studies from the early 1990s demonstrated that eosinophils may also get into the lower airways of patients with COPD and their increased numbers can be noticed during exacerbations as well as stable disease. Eosinophilic phenotype of COPD is characterized by several unique features, i.e. a specific pattern of airway inflammation and distinct clinical course or susceptibility to corticosteroid treatment. In this paper, we present an up-to-date review of the literature on…

Tracheobronchial tumors.

Tumors of trachea and bronchi are uncommon and can occur in the form of benign or low- and high-grade malignant tumors. Although tracheobronchial tumors (TBTs) represent only 0.6% of all pulmonary tumors, they are clinically significant. Delays in diagnosis of these tumors commonly occur because the signs and symptoms caused by these tumors are nonspecific and chest radiographs are often considered unremarkable. Therefore, novel radiological techniques and better access to flexible bronchoscopy enable detection of larger number of TBT. The purpose of this article is to provide a review of trac…

Unresectable Middle Mediastinal Biphasic Pulmonary Blastoma.

We report a case of a young male who presented with an unresectable, centrally-located classic biphasic pulmonary blastoma (CBPB) involving his bilateral mainstem bronchi and esophagus and a synchronous right testicular seminoma. CBPB is a rare and aggressive tumor that most commonly presents as a solitary mass in the periphery of the lung. Surgical resection is the preferred treatment for CBPB, as chemotherapy and radiation have demonstrated limited effectiveness. In the current case, four cycles of cisplatin, ifosfamide, and etoposide with concurrent radiotherapy resulted in a favorable resp…

Mitral valve replacement in an adult with left pulmonary agenesis.

Pulmonary agenesis is associated with the absence of pulmonary vessels, bronchi, or parenchyma. This condition usually occurs between the 4th and 5th week of gestation during the embryonic phase. Etiopathogenic factors associated with pulmonary agenesis are not fully understood. In the literature, genetic and teratogenic factors, viral infections, and vitamin-A deficiency are shown to be associated with pulmonary agenesis [Malcon 2012]. This condition may be seen unilaterally or bilaterally. Although the precise rate of incidence is unknown, it is estimated to occur in one of every 10,000 to 1…

Bronchial mucous gland adenoma presenting as massive hemoptysis: A diagnostic dilemma.

Mucous gland adenoma of the lung is an uncommon benign tumor that histologically resembles the mucus-secreting component of tracheobronchial gland. The majority arises within the main, lobar, or segmental bronchi but parenchymal involvement had also been reported. We herein present a case of mucous gland adenoma arising from the left lower lobe bronchus. The 32-year-old female presented with massive hemoptysis, productive cough, and dyspnoea and was clinically misdiagnosed as tuberculosis. Radiology proved to be inconclusive. This case highlights the importance of a complete lung work up in pa…

A case of tracheobronchial amyloidosis treated with endoscopic debulking and external beam radiation therapy.

We report on a 55-year-old patient who was admitted to hospital because of recurrent pneumonia. CT imaging provided airway narrowing and mural thickening of the distal trachea and mainstem bronchi, compatible with endobronchial polypoid, toric-shaped changes of the distal tracheal wall spreading into both the left and right bronchial system. Bronchoscopy was performed and biopsies revealed the diagnosis of tracheobronchial amyloidosis. We performed a combination of bronchoscopic debulking and consecutive external beam radiation therapy with the result of no further progression of the disease, …

Pulmonary artery sarcoma diagnosed by endobronchial ultrasound-guided transbronchial needle aspiration.

Pulmonary artery sarcoma (PAS) is a rare tumor that is often detected at an advanced stage, when disease is so widespread that a radical surgical procedure is no longer indicated. Therefore, less invasive biopsy techniques are required to establish a definitive preoperative diagnosis. Endobronchial ultrasound (EBUS) is useful for producing real-time images of both lymph nodes and the interior of pulmonary arteries adjacent to the bronchi. We report a case with masslike lesions in the pulmonary artery that were observed by EBUS and from which tissue was obtained by endobronchial ultrasound-guid…

[Bronchiectasis: diagnostic workup].

Bronchiectasis is a condition defined by permanent dilation of the bronchi, either idiopathic or associated with other disease states. Diagnostic workup for bronchiectasis is essential in determining management and treatment. For diffuse bronchiectasis, we propose a workup plan that includes testing for humoral immunodeficiency by measuring serum IgG and anti-pneumococcal antibodies, looking for primary ciliary dyskinesia by exhaled nasal NO and cytologic brush biopsy of the nasal mucosa, looking for cystic fibrosis by chloride sweat testing and genotyping, evaluation for allergic bronchopulmo…

[COPD: a disease with systemic inflammation].

COPD is considered today as an inflammatory disease of the lungs and bronchi, often associated with circulating inflammatory markers and non-respiratory co-morbidities. C-reactive protein is a good example of a circulating biomarker in this disease. Recent studies have shown that it is particularly associated with some phenotypes of COPD, that it is linked with cardiovascular and other co-morbidities, and that it has genetic determinants. In view of reports in the recent literature treatments aimed at decreasing systemic inflammation in COPD, in particular the use of statins, are discussed. Tr…

Crenosoma brasiliense sp. n. (Nematoda: Metastrongyloidea) parasitic in lesser grison, Galictis cuja (Molina, 1782) (Carnivora, Mustelidae) from Brazil, with a key to species of Crenosoma Molin, 1861.

This study describes Crenosoma brasiliense (Nematoda, Metastrongyloidea), a new species parasitic in bronchi and bronchioles of Galictis cuja (Molina) (Carnivora, Mustelidae) from Brazil. This species differs from other 11 species of Crenosoma by having a cuticular projection at the distal end of the spicules, forming a prominent blade at the tip of the spicule, a vulval cuticular appendage with a triangular shape and prominent vulval lips. There are no previous records of species of Metastrongyloidea in G. cuja or species of Crenosoma in South America. Therefore, the new species represents th…

Peripheral bronchopathia osteochondroplastica.

Tracheobronchopathia osteochondroplastica refers to the abnormal presence of osseous and/or cartilaginous nodules in the submucosa of the trachea and/or bronchi. This disease presents with exophytic lesions that are covered by normal mucosa. In this study, we report the case of a 51-year-old man who complained of exertional dyspnea. Chest x-ray and computed tomography showed diffuse bilateral micronodular pulmonary infiltrate. Mucosal appearance during bronchoscopy was completely normal. Histologic examination of the biopsy taken by transbronchial lung biopsy showed discrete cartilaginous nodu…

Congenital abnormalities of intrathoracic airways.

Multidetector CT has broadened the potential of imaging to demonstrate anomalies of the lung and the tracheobronchial tree with increasing frequency. Two-and three-dimensional reformatting improve the understanding of complex tracheobronchial anomalies. Most congenital tracheobronchial anomalies are rare and almost always nonsymptomatic; however, some may be confused with or even responsible for respiratory disease. Tracheal and accessory cardiac bronchi are among the most frequent anomalies, but other ectopic or supernumerary lung buds, developmental tracheobronchial interruption, obstruction…

Plastic bronchitis caused by neoplastic infiltrates in a child.

We report on a case of a 7-year-old girl admitted for pneumonia not responding to oral antibiotics. During hospitalization, her pulmonary status deteriorated as a result of significant atelectasis. An extensive workup revealed an anaplastic large-cell lymphoma with neoplastic cells, found in both a biopsied lymph node and pleural fluid aspirate. Bronchoscopic examination showed nearly complete obstruction of the left side by bronchial casts composed of tumor cells, fibrin, and necrotic material, consistent with plastic bronchitis. Neoplastic infiltration of the bronchi should be considered in …

[Therapy of bronchial asthma in childhood].

The aim of the study is to summarize the up-to-date therapy of bronchial asthma in childhood. Many children in the world (1-30%) suffer from this disease. The essence of it is the special allergic inflammation of the bronchi, so the chief aim of the therapy is the diminution of this inflammation and the ceasing of the actual symptoms. The therapy is based on three possibilities: diminution of the causative allergens and trigger factors in the environment, drug therapy, and allergen vaccination (immunotherapy). The drug therapy is considered on the four step severity criteria of the WHO. The st…

Management of bronchiectasis.

Around 1 in 1,000 adults in the UK has bronchiectasis, a potential outcome of various underlying lung diseases. It results from a 'vicious circle' of recurrent infection, inflammation, excessive mucus production, reduced mucociliary clearance, and dilatation and destruction of bronchi. Associated damage to blood vessels can lead to haemoptysis, which is usually slight, but may be severe. Bronchiectasis may be limited to one area (localised) or be more widespread (generalised). It can reduce lung function and quality of life, and may lead to respiratory failure and premature death. Here we revi…

[Postmortem detection of hemp intoxication].

The diagnostic tools applicable to the forensic medical expertise of cadavers in cannabinoid intoxication (CI) were evaluated. Histochemistry with incubation of stable blue "B" (SBB) were used for the detection of cannabinoids in the bronchi and lungs. Hyperemia and capillarostasis in the mucous tunics of the mouth and stomach as well as hyperemia and edema in the lungs and brain were histochemically detected in CI. The histochemical activity of aldehyde dehydrogenase (AlDG) was most essentially decreasing in the adrenal glands and brain of younger persons. The SBB reaction in the bronchial an…

[Endobronchial and laryngeal metastasis. An unusual form of dissemination of the hypernephroma].

OBJECTIVE: To describe uncommon forms of dissemination of hypernephroma. METHODS: A case of hypernephroma that metastasized to the laryngeal vallecula and bronchi is presented. Our findings were compared with those reported in the literature. The diagnostic, radiological, clinical aspects and route of dissemination of some atypical sites of metastasis are discussed. RESULTS/CONCLUSIONS: It is important to be familiar with these atypical sites of metastasis since these lesions may appear at the same time or before the primary tumor is detected. A high index of suspicion will make a major impa…
