A 22-year-old man has been recently admitted to our Department with a 10-day history of melena. Because of nodular acne, the patient had been treated with Isotretinoin, a 13-cis-retinoic acid (20-mg twice daily, for 15 d). Upper gastrointestinal endoscopy revealed edema and hyperemia of the gastric mucosa of the body and antrum. Flexible sigmoidoscopy revealed edema and hyperemia of the mucosa of the rectum and sigmoid colon with numerous erosions. To exclude the possibility of small bowel involvement the patient underwent video capsule endoscopy that showed a diffuse and extensive intestinal …

Wireless capsule endoscopy (CE) was introduced for human clinical diagnostic utilization in 2001. CE has become a first line method of evaluating the small intestine for suspected abnormalities and disease. Contraindications to CE include the presence of intestinal obstruction, fistulas, or structures. Capsule retention has been reported in patients with a strictured or stenotic area of intestine caused by occult neoplasm, nonsteroidal anti-inflammatory drugs, Crohn's disease, radiation enteritis, or previous abdominal surgery. Safe and effective use of CE has been reported in the evaluation o…

The Bravo pH probe is a wireless capsule allowing remote quantification of gastroesophageal reflux. A 14-year-old male presented for Bravo probe placement. Following esophagogastroduodenoscopy, attachment of the Bravo probe was attempted. Immediately, the patient began coughing without oxygen desaturation. Repeat endoscopy did not reveal the probe within the esophagus or stomach. He was then intubated, and fluoroscopy demonstrated a foreign body within the bronchus intermedius. Rigid bronchoscopy was performed to retrieve the probe using optical forceps. This is the first case of pediatric ina

CT and MR enterography and capsule endoscopy are increasingly used as routine diagnostic tests for patients with potential small bowel disorders and obscure gastrointestinal bleeding. Non-steroidal anti-inflammatory drugs (NSAIDs) are commonly used drugs that disrupt prostaglandin synthesis and result in a variety of localized complications within the small bowel ranging from ulcer formation to characteristic circumferential strictures, or diaphragms. NSAID enteropathy encompasses this spectrum of acute and chronic inflammatory sequelae, and is associated with typical findings at capsule endos…

Celiac disease (CD) is a type of intestinal malabsorption syndrome, in which the patients are intolerant to the gliadin in dietary gluten, resulting in chronic diarrhea and secondary malnutrition. The disease is common in Europe and the United States, but only sporadic reports are found in East Asia including China. Is CD really rare in China? We examined 62 patients by capsule endoscopy for chronic diarrhea from June 2003 to March 2008. Four patients with chronic diarrhea and weight loss were diagnosed to have CD. Under the capsule endoscopy, we observed that the villi of the proximal small b…

Small-bowel bleeding, accounting for 5% to 10% of gastrointestinal bleeding episodes, presents a distinct diagnostic challenge due to the organ's length and anatomical complexity. Over recent years, the management of small-bowel bleeding has significantly evolved, driven by advancements in both diagnostic and therapeutic technologies. This Taiwan Association for the Study of Intestinal Diseases (TASID) practical consensus integrates local epidemiology, up-to-date diagnostic advances, including early small-bowel capsule endoscopy, and emerging treatments for vascular lesions such as angiodyspla…

A 25-year-old man presented with 3-day history of abdominal pain, vomiting, diarrhea, and bloody stools. The contrast-enhanced CT examination of the abdomen detected thickening and edema of intestinal canal wall. The complete colonoscopy showed hyperemia, dropsy and erosion in the sigmoid colon and rectum. The biopsies revealed obvious bleeding points in mucosa. Then, wireless capsule endoscopy (OMOM ® JS-ME-I) was carried out and showed multiple lesions in the entire small intestine with diffused hyperemia, dropsy and erosion, even multiple and large ulcers. Subsequently, symmetrical scattere…

The introduction of device-assisted enteroscopy (DAE) in the beginning of the 21st century has revolutionized the diagnosis and treatment of diseases of the small intestine. In contrast to capsule endoscopy, the other main diagnostic modality of small bowel diseases, DAE has the unique advantages of allowing the observation of the region of interest in detail and enabling tissue acquisition and therapeutic intervention. As DAE becomes an essential procedure in daily clinical practice, there is an increasing need for correct guidelines on when and how it is to be performed and what technical fa…

BACKGROUND: The STRIDE-II position statement has established endoscopic healing as the long-term target of treatment for inflammatory bowel disease, and ileocolonoscopy is considered the gold standard for assessment of endoscopic healing. However, precise assessment of endoscopic healing cannot be achieved by ileocolonoscopy alone in patients with Crohn's disease (CD).
SUMMARY: Approximately 70-80% of patients with CD have small bowel disease, and intestinal complications develop more frequently in the small bowel than in the colorectum. The recent advent of small bowel capsule endoscopy and …

BACKGROUND AND OBJECTIVES: Celiac disease is an autoimmune disease occurring in about 1 in 100 people worldwide. Early diagnosis and efficient treatment are crucial in mitigating the complications that are associated with untreated celiac disease, such as intestinal lymphoma and malignancy, and the subsequent high morbidity. The current diagnostic methods using small intestinal biopsy histopathology, endoscopy, and video capsule endoscopy (VCE) involve manual interpretation of photomicrographs or images, which can be time-consuming and difficult, with inter-observer variability. In this paper,…

Juvenile polyps are the most common gastrointestinal polyps in childhood. Typically, they are located in the colon and present with intermittent and painless hematochezia. A few case reports have described juvenile polyps in the small intestine, all presenting as intussusception requiring surgery. We report an isolated juvenile polyp in the small intestine presenting with painless anemia, identified using video capsule endoscopy, and removed via enteroscopy.

Existing techniques for diagnosing gastrointestinal disorders in stomach, small and large intestines, and colon depend on biopsy, endoscopy or colonoscopy methods which are invasive, expensive and time-consuming. In fact, such methods also lack the ability to access large parts of the small intestine. In this article, we demonstrate a smart ingestible biosensing capsule that is capable of monitoring pH activity in small and large intestines. pH is a known biomarker for several gastrointestinal disorders such as inflammatory bowel disease. Functionalized threads utilized as pH sensing mechanism

Altered gastrointestinal anatomy is common in patients with inflammatory bowel disease, particularly in those who underwent bowel surgery. Commonly performed surgeries are bowel resection and anastomosis and strictureplasty for Crohn's disease; and restorative proctocolectomy with ileal pouch-anal anastomosis for ulcerative colitis. The area of anastomosis and suture line is at the greatest risk for the development of postoperative bleeding. Altered bowel anatomy, especially the presence of strictures, strictureplasty, or structural or functional pouch outlet obstruction, puts these patients a

The diagnosis and management of small bowel bleeding (SBB) can be a clinical challenge. Advances in video capsule endoscopy, balloon-assisted enteroscopy, and multiphasic computed tomography allow for localization and therapeutic intervention. Etiologies of SBB including vascular, neoplastic, and inflammatory conditions are associated with age and comorbidities. The present review highlights terminologies that describe SBB, provides a differential diagnosis for bleeding etiologies, and summarizes a clinical approach to managing this condition.

Smart capsules are developing at a tremendous pace with a promise to become effective clinical tools for the diagnosis and monitoring of gut health. This field emerged in the early 2000s with a successful translation of an endoscopic capsule from laboratory prototype to a commercially viable clinical device. Recently, this field has accelerated and expanded into various domains beyond imaging, including the measurement of gut physiological parameters such as temperature, pH, pressure and gas sensing, and the development of sampling devices for better insight into gut health. In this review, th

Inflammation of the gastrointestinal (GI) tract accompanies several diseases, including Crohn's disease. Currently, video capsule endoscopy and deep bowel enteroscopy are the main means for direct visualisation of the bowel surface. However, the use of optical imaging limits visualisation to the luminal surface only, which makes early-stage diagnosis difficult. In this study, we propose a learning enabled microultrasound ( μ US) system that aims to classify inflamed and non-inflamed bowel tissues. μ US images of the caecum, small bowel and colon were obtained from mice treated with agents to i…
