Artificial intelligence (AI) is reshaping cardiovascular medicine, evolving from task-specific supervised models towards multitask and foundation models capable of broader, more scalable interpretation. Early AI systems achieved high performance in electrocardiographic interpretation, echocardiographic measurement and disease diagnosis but their dependence on predefined outputs and labelled datasets limits flexibility and confines them to individual tasks. Foundation models address these limitations by learning reusable representations from large-scale data, often through self-supervised or co

BACKGROUND: Early diagnosis of transthyretin cardiac amyloidosis (ATTR-CM) is essential for timely intervention but remains challenging due to its subtle and nonspecific clinical presentation. The CRONOS-ATTR study aimed to improve early detection of ATTR-CM by integrating multimodal data (clinical, electrocardiographic, and echocardiographic) within a model-guided medicine framework.
METHODS: Using artificial intelligence (AI) algorithms from CardiolyseECGSoftware and Ligence Heart, along with human intelligence (multidimensional interpretable models), we standardized and harmonized heteroge

We report on five newborns with an arterio-venous malformation of the vein of Galen. All newborns were cyanotic and in congestive heart failure without any evidence of congenital heart disease. Congestive heart failure in these cases was mainly due to an almost two-fold increase in cardiac output of approximately 8 l/min/m2 (normal: 4.5 l/min/m2). According to previous reports, mortality is very high in patients with this malformation when becoming symptomatic during infancy, and therapy by surgery or embolization is only successful in 10-30%. While three of our patients died shortly after dia

The arteriovenous fistula (AVF) is a preferred access for haemodialysis (HD) in patients with end-stage renal disease. However, its haemodynamic effect on the heart is not negligible; it may cause or exacerbate heart failure and pulmonary hypertension, especially when the flow is excessively high. After the transplantation, a decrease in cardiac output leads to reduced kidney graft perfusion, which can result in the development of cardiorenal syndrome. The guidelines for managing AVF after transplantation are still not well defined, so care and surveillance are based on the experience of trans

BACKGROUND Transient abrupt loss of consciousness due to sudden but pronounced decrease in cardiac output caused by a change in heart rate and rhythm is termed Stokes-Adams disease. Causes of Stokes-Adams syndrome are 1) transition from normal rhythm to high grade block, 2) slowing of idioventricular rhythm in the course of complete heart block, and 3) abnormal ventricular rhythm such as ventricular tachycardia and ventricular fibrillation. Paroxysmal ventricular standstill is one of the rarest causes of Stokes-Adams attack. It is well documented that some patients with a diagnosis of epilepsy

The aim of the study--to estimate the effect of therapy with Magne 6 on the indices of cardiac hemodynamics, tolerance to physical load, activity of antioxidant protection enzymes and grade of hypoxia in youths with 1 grade mitral valve prolapse (MVP) without regurgitation. In 73 cases with impaired compensatory adaptive possibilities the extent of main syndromes of autonomic dystonia and state of cardiac hemodynamic indices were evaluated. The activity of catalase (C), glutathione reductase (GR), superoxide dismutase (SOD), lactate (L) and pyruvate (P) level were detected in red blood cells,

1. The effects of a first dose and of chronic treatment with spirapril, a novel angiotensin converting enzyme (ACE) inhibitor, on short-term blood pressure and heart rate fluctuations were assessed by fast Fourier spectral analysis. The effects on systemic haemodynamics in supine and standing position were also studied. We treated 11 patients with 3 mg and 13 patients with 12 mg spirapril for 8 weeks. 2. Overall blood pressure variability was not changed by spirapril. By spectral analysis the changes in blood pressure and heart rate variability in various frequency bands can be assessed, which

Certain physical and physiological changes occur in the atmospheric levels where flight and space activities take place. Air pressure decreases with increasing altitude and the partial pres¬sure of O2 decreases in parallel with the atmospheric pressure drop and creates hypoxia in the flight crew and in the passen¬gers. In case of acute hypobaric hypoxia, blood is redistributed to the brain and the heart, whereas blood supply to internal organs, such as kidney and skin is reduced. Peripheral cyanosis can be observed on the fingertips and the lips during hypoxia-induced blood redistribution. Tac

Chronic Constrictive pericarditis (CCP) is a rare clinical entity that can pose diagnostic problems. Constrictive pericarditis is the final stage of a chronic inflammatory process characterized by fibrous thickening and calcification of the pericardium that impairs diastolic filling, reduces cardiac output, and ultimately leads to right heart failure and to atrial dilatation which can caused supravetricular arrythmia. Transthoracic echocardiography, computed tomography, and cardiac magnetic resonance imaging each can reveal severe diastolic dysfunction, increased pericardial thickness and calc

Dengue hemorrhagic fever is one of the most commonly encountered mosquito-borne viral infections of humans worldwide with multiple reported outbreaks. Cardiac involvement is a known manifestation of the disease usually presenting as rhythm abnormalities, myocarditis, or pericardial effusion, which may be clinically asymptomatic. We describe a case of a 30-year-old woman who presented to us with high-grade fever, headache, retro-orbital pain, generalized maculopapular rash with bilateral pleural effusion, and hypotension. Dengue non-structural protein 1 (NS1) antigen and IgM antibodies were pos

BACKGROUND: Delayed postoperative pericardial effusion after coronary artery bypass grafting (CABG) may pose a significant risk in patients with limited cardiac reserve because of impaired ventricular filling and potential hemodynamic instability. Positive pressure ventilation (PPV) may reduce venous return and cardiac output in preload-dependent patients.
CASE PRESENTATION: A 73-year-old male who had undergone CABG 10 days earlier presented with symptomatic delayed postoperative large pericardial effusion. Given hemodynamic fragility, a physiology-guided anesthetic plan that avoided intubati

Temporary mechanical circulatory support (tMCS) has evolved as a first-line treatment of cardiogenic shock refractory to medical therapy. In recent years, tMCS has been applied to bridge and precondition end-stage heart failure patients with structural heart disease for surgery or intervention. This review describes three clinical scenarios: preoperative stabilization and bridge-to-surgery (Type 1), planned intraoperative support in patients at high risk of perioperative low cardiac output (Type 2), and rescue support for failure to wean from cardiopulmonary bypass or postoperative heart failu

A 49-year-old male patient with multiple comorbidities, including end-stage renal disease (ESRD), diabetes mellitus, obesity, and obstructive sleep apnea, presented to the emergency department with clinical signs of hemorrhagic shock in the setting of volume overload. After initial stabilization, he underwent paracentesis and was diagnosed with hemorrhagic ascites (HA). Post-discharge, the patient was re-admitted multiple times due to refractory HA in the setting of poor volume management. Cirrhosis was ruled out with biopsy, and subsequent workup following his initial presentation eventually

This case report describes a 72-year-old woman who developed an acute postmyocardial infarction ventricular septal defect (VSD) with consequent cardiogenic shock. Intra-aortic balloon pump (IABP) counter-pulsation was urgently initiated in the cardiac catheterisation laboratory, with neither clinical nor haemodynamic improvement, prompting immediate removal of the IABP and the insertion of an Impella 2.5 heart pump (AbioMed Inc; Danvers, Massachusetts, USA), a temporary ventricular assist device. Thereafter, the patient improved clinically and was admitted to the cardiovascular intensive care

PURPOSE OF REVIEW: Positive inotropic agents are widely used in the management of the critical ill patient presenting with low cardiac output state. Different inotropic agents are available, and different effects on hemodynamic endpoints may be recognized, but data on relevant clinical endpoints are scarce. A growing body of literature suggests that overuse of inotropes may have detrimental effects on cardiomyocytes, resulting in an increased risk of morbidity and mortality. The present review will summarize recent literature, focusing on outcome studies among adult patients related to use of

Plastic bronchitis is a rare life-threatening complication of Fontan operation. When medical treatment is ineffective in the setting of high systemic venous pressures, Fontan fenestration may be considered to decompress venous pressures and improve cardiac output by creation of the right-to-left shunting. However, transcatheter approach can be difficult in patients with complex venous anatomy. We report a 4-year-old girl born with hypoplastic left ventricle and heterotaxy syndrome, who developed plastic bronchitis following extracardiac Fontan procedure. Her venous anatomy was complex with dex

Terlipressin--a long-acting analogue of vasopressin--has been described to restore blood pressure in patients with catecholamine-resistant septic shock without obvious complications. We administered low-dose terlipressin (a single i.v.-bolus of 0.5 mg) to a patient with severe, hyperdynamic septic shock requiring high dosage of noradrenalin. After terlipressin the dose of noradrenalin could be reduced by 2/3 to obtain the same blood pressure. Two hours after terlipressin, the cardiac index had decreased from 6.2 to 3.3 l min(-1) m(-2) and the concentration of L-lactate in the rectal lumen, as

Endomyocardial biopsy remains the primary method for diagnosis of cardiac allograft rejection. Generally, endomyocardial biopsy is considered a relatively safe procedure in heart transplant recipients. Complications that have been reported are related to catheter insertion and include carotid arterial puncture, prolonged bleeding, vasovagal reaction, ventricular tachyarrhythmias, and transient conduction abnormalities. Serious complications such as right ventricular perforation with cardiac tamponade may also occur. Most complications are usually without significant long-term sequelae. This re

The first 16 patients to have mitral valve replacement with the Starr-Edwards prosthesis at St. Bartholomew's Hospital, London, were reviewed from one to three years after operation. These patients had presented with serious mitral valve disease and there was a high incidence of severe dyspnoea and persistent congestive failure before operation. There were four operative deaths, and two survivors had poor results due to regurgitation alongside the prosthesis. The remaining 10 patients showed considerable improvement. Although no diuretics were given after operation there was little dyspnoea an

High fat diet (HFD) promotes cardiovascular disease and blunted cardiac vagal regulation. Temporal onset of loss of cardiac vagal control and its underlying mechanism are presently unclear. We tested our hypothesis that reduced central vagal regulation occurs early after HFD and contributes to poor cardiac regulation using cardiovascular testing paired with pharmacology in mice, molecular biology, and a novel bi-transgenic mouse line. Results show HFD, compared to normal fat diet (NFD), significantly blunted cardio/pulmonary chemoreflex bradycardic responses after 15 days, extending as far as
