Langerhans cell histiocytosis (LCH), formerly known as histiocytosis X, is a rare disorder (approximately 1:1,500,000 inhabitants) characterized by clonal proliferation and excess accumulation of pathologic Langerhans cells causing local or systemic effects. The exact etiology of LCH is still unknown. LCH could affect patients of any age, although most present when they are children. The most frequent sites of the bony lesions are the skull, femur, mandible, pelvis and spine. A variety of treatment modalities has been reported, but there was no evidence suggesting that any one treatment was mo

STUDY DESIGN: Primary clinical trial of limited fixation for unstable Atlas fracture.
OBJECTIVES: To clinically validate feasibility, safety and value of the C1 later-mass screw C2 pedicle screw and crosslink compression fixation technique.
SUMMARY OF BACKGROUND DATA: [corrected] In previous clinical studies, several techniques have been introduced to fix Atlas fracture. But all these treatments have intrinsic disadvantages. Now seeking a means of limited internal fixation has become a worthwhile subject of clinical study.
MATERIALS AND METHODS: From January 2001 to December 2004, 17 cases

BACKGROUND: Chiari I malformation, accompanied by superposed bony anomaly of the craniovertebral junction, is comparatively rare. We report a case of Chiari I malformation accompanied by assimilation of the atlas, Klippel-Feil syndrome, and syringomyelia.
CASE DESCRIPTION: The patient was a 61-year-old woman demonstrating numbness of the extremities, sensory impairment, muscular weakness, and tendon hyper-reflexia. X-ray images and CT scans demonstrated assimilation of the atlas to the occipital bone, C2 and C3 fusion, abnormal passage of the vertebral arteries, and an anomalous bony mass on

This review covers significant developments in the pathological classification of gynecological tumors in recent years. Topics covered include The Cancer Genome Atlas (TCGA) Classification of endometrial carcinomas and how to incorporate this into routine reporting, the fact that most synchronous endometrial and ovarian endometrioid carcinomas represent metastasis from the endometrium to the ovary, and the important subject of lymphovascular space invasion in endometrial carcinomas. The categorization of cervical squamous cell carcinomas (SCCs) and adenocarcinomas and vulval and vaginal SCCs i

Massive subarachnoid hemorrhage may occur on a traumatic basis. The injury is most often sustained by a blow with a clenched fist against the posterolateral part of the cranial basis, but the injury may also occur in relation to an accident. The condition is rare, most often occurring in alcohol intoxicated men. The victim typically collapses immediately and usually dies within a few minutes. The origin of the bleeding may be the vertebral artery on the neck or the intercranial basal brain arteries. In some cases the origin of the bleeding cannot be located. The pathogenetic mechanisms have be

We report a case of self-decapitation by suicidal hanging from a river bridge and discuss the decapitation mechanism, reviewing the literature. The torso and the head of the victim, respectively, were found apart in a river approximately 100 m and 600 m, respectively, downstream from the bridge in two days. Morphological, serological and DNA analyses confirmed their identity. Torn ligaments between the atlas and axis accompanied by fractures in the axis at the partes interarticulares were indicative of a traction force combined with anteroflexion of the head by falling from a height, and the r

Twenty cases with fracture-dislocation of the cervical spine were treated by U-shaped pin fixation. The U-shaped pin made of Steiman pin was used to hold sublaminar fixation of two segments above and below the vertebra of fracture. When atlas or axis was injured, a pair of small holes were drilled on the occipitale. The closed end of the U-Shaped pin was wired through the holes. Early postoperative ambulation was allowed without external fixation. 18 cases were followed up for 8-35 months. 5 cases of six patients with neurologic deficits recovered completely. The remaining one improved, no fai

Arthritic affection of the upper cervical spine is relatively common in rheumatoid arthritis (RA). Atlanto-axial subluxations are also reported to occur frequently, but it seems that neurological symptoms due to the subluxations seldom occur. We have studied 104 consecutive in-patients. In a group of 50 non-arthritics we have measured the distance from dens to the anterior arch of the atlas, and from dens to the foramen magnum line. By means of percentile-estimate we have defined values at risk for having pathological values for the measured distances. In 104 rheumatoid arthritis patients we f

A 48-year-old Japanese woman with adult T-cell leukemia/lymphoma (ATLL), histologically presenting CD30-positive large cell lymphoma is reported. The patient, who was from an ATLL endemic area in Japan, had cutaneous nodules in the head, trunk, and extremities, and cervical lymph node swelling; these had been found three months before her admission to our hospital. A biopsy specimen of a skin lesion showed diffuse large cell lymphoma; the lymphoma cells were positively stained with CD30 (Ki-1/Ber H-2), CD4 (helper-T), and CD25 (interleukin-2 receptor) antibodies. Anti HTLV-1 antibody (ATLA) wa

Since the release of 64-section computed tomographic (CT) scanners in 2004, there have been several new advancements in cervical spine imaging. A growing trend in polytrauma patients with obtain-dating is the use of multidetector CT as a stand-alone screening test to rule out cervical injuries. Spine surgeons are beginning to adopt the SLIC (Subaxial Injury Classification and Scoring) method for the cervical spine based on CT and MR imaging data. Multidetector CT is currently being used to reassess radiographic parameters that were previously established for the evaluation of craniocervical di

AIM: The aim of this study was to analyze the physiopathology of the acute cervical injure in the event of mandibular condylar fractures.
METHODS: As in the Part 1, 25 non-consecutive cases of condylar mandibular fractures (16 males and 9 females, mean age: 22.96/range 14-36 years) observed and treated in the Maxillofacial Surgery Department of the University of L'Aquila, have been studied. Types of fractures examined included: unilateral: 19 cases (solitary: 12; associated with other mandibular fractures: 7, homolateral: 2); bilateral: 6 cases (equivalent: 2, not equivalent: 4). A control gr

INTRODUCTION: Various techniques have been described for posterior atlantoaxial fusion. Sublaminar passage of the wire/cable is cumbersome with a risk of spinal cord injury. Packing morselized bone grafts into the C1-2 facet joints may be difficult and it may cause massive bleeding and neuropathic pain or posterior scalp numbness postoperatively. We introduce a modified method by using C1-2 screw-rod fixation (SRF) to compress a structural iliac bone graft between the posterior elements of C1 and C2 without supplemental wiring construct.
MATERIALS AND METHODS: From December 2006 to May 2009,

20% of all spine injuries are cervical spine injuries. Surgical treatment of these injuries must take into account the great mobility of this part of the spinal column. Therefore, biomechanical aspects must be considered, especially in the upper cervical spine-C1 and C2. Describing our own therapeutical regimen in 35 patients with unstable upper cervical spine injuries we explain the biomechanical back-ground and review the literature. It becomes evident that ventral approaches are superior to dorsal techniques for decompression, reposition, and stabilization with minimal loss of mobility.

BACKGROUND CONTEXT: In cases of basilar invagination-atlantoaxial dislocation (BI-AAD) complicated by atlas occipitalization (AOZ), the approach to cranial end fixation has consistently sparked debate, generally falling into two categories: C1-C2 fixation and occipitocervical fixation. Several authors believe that C1-C2 fixation carries a lower risk of fixation failure than occipitocervical fixation.
PURPOSE: To study the biomechanical differences among 3 different cranial end fixation methods for BI-AAD with AOZ.
STUDY DESIGN: This was a finite element analysis.
PATIENT SAMPLE: A 35-year-o

OBJECTIVE: The necessity of partial occipital condyle (OC) resection for lesions in the ventral craniocervical junction is debatable. This study's purpose was to compare the surgical exposure of the classic far-lateral approach (FLA) and transcondylar FLA.
METHODS: The classic FLA and transcondylar FLA were performed in 12 human cadaveric heads (24 sides). The surgical corridor of 3 levels (a: vagus nerve, b: from the midpoint of proximal ends of the vagus and hypoglossal nerves to the midpoint of the distal ends of each nerve, c: hypoglossal nerve) and the maneuverability (the area between n

Minimally invasive surgery to the posterolateral craniovertebral junction (CVJ) has not been sufficiently described. The aims of this study were to evaluate the feasibility of an endoscopic far-lateral approach to the posterolateral craniocervical junction and to better understand the related anatomy under distorted endoscopic view. Ten fresh cadavers were studied with 4-mm 0° and 30° endoscopes to develop the surgical approach and to identify surgical landmarks. After making a 3-cm straight incision behind the mastoid process, the superior oblique and rectus capitis posterior major muscles we

Structural defects of the posterior arch of the atlas are rare, and range from clefts of variable location and size to more extensive defects such as complete agenesis. These abnormalities are usually incidental radiological findings. We present a case of a fracture of the anterior arch of the atlas associated with a congenital abnormality of the posterior arch.

Occipitoatlantal dislocation (OAD) can be devastating. This injury may be fatal in many cases, but more survivors are reported because of improvements in diagnosis and treatment. This article describes the diagnosis and treatment of OAD. To diagnose and treat OAD appropriately, neurosurgeons must have a detailed understanding of the anatomy of the craniocervical junction. Various radiographic criteria are used to establish the diagnosis of OAD. A destabilizing injury such as OAD requires surgical fixation. Many surgical techniques are available for fixation of the craniocervical junction. Futu
