Various types of total laparoscopic nerve-sparing radical hysterectomies and their effects on bladder function.

OBJECTIVE: This study was conducted to ascertain the correlation between preserved pelvic nerve networks and bladder function after laparoscopic nerve-sparing radical hysterectomy. METHODS: Between 2009 and 2011, 53 patients underwent total laparoscopic radical hysterectomies. They were categorized into groups A, B, and C based on the status of preserved pelvic nerve networks: complete preservation of the pelvic nerve plexus (group A, 27 cases); partial preservation (group B, 13 cases); and complete sacrifice (group C, 13 cases). To evaluate bladder function, urodynamic studies were conducted

[Prevention and management of severe hemorrhage during gynecological operations].

OBJECTIVE: To investigate the prevention and management of severe bleeding during gynecological operations. METHODS: A retrospective study of 85,505 gynecological operations from 21 hospitals in China during the period of 1990-1999 was analyzed. RESULTS: There were 683 cases with bleeding more than 1,000 ml during surgery, an incidence of 0.80% (range 0.07%-6.98%). Operation for removal of malignant ovarian tumor was the commonest cause of severe bleeding (42.31%); followed by cervical carcinoma (28.71%); endometrial carcinoma (16.11%). Only 6 transvaginal surgeries (0.88%) had severe bleedi

The numb arm and hand.

Trauma and compression along the course of the median, ulnar or radial nerve from the brachial plexus to the fingers may cause pain, weakness, numbness or tingling of the upper extremity. Diabetes, smoking, alcohol consumption, rheumatoid arthritis and hypothyroidism are risk factors for nerve entrapment, although these disorders typically produce bilateral symptoms. Carpal tunnel syndrome, the most common nerve entrapment condition, results from median nerve compression at the wrist. The diagnosis is suggested by decreased pain sensation and numbness in the thumb and index and middle fingers;

Carotidynia: Overview of an uncommon identification for unilateral neck pain.

BACKGROUND: Carotidynia, also known as Fay Syndrome, manifests as an atypical facial neuralgia characterized by an unusual neck pain extending towards the head and associated with carotid artery tenderness. Diagnostic criteria include neck tenderness, elevated carotid pulse without anatomical abnormalities, and neck distension. It was initially classified as a vascular headache but later re-evaluated and reclassified as a nonentity-a general condition caused by nonvascular factors. The etiology has not been extensively elucidated. CASE PRESENTATION: We present two cases characterized by dysph

Utility of nerve ultrasound in the management of primary neurolymphomatosis: Case report and review of the literature.

INTRODUCTION: Primary neurolymphomatosis (NL) is a critical differential diagnosis of asymmetric multiple mononeuropathy and radiculoplexopathy. Its diagnosis is often challenging due to the lack of typical clinical signs of systemic lymphoma. We report a case of primary NL where nerve ultrasound (NUS) played an important role in the diagnosis and follow-up of the disease. CASE PRESENTATION: A 52-year-old man developed asymmetric painful multiple mononeuropathy in the right upper limb with cranial nerve involvement. After being referred to our department, the patient underwent NUS, which reve

Case report: Autoimmune nodopathy with concurrent serum and CSF IgG4 anti-neurofascin 155 antibodies.

OBJECTIVE: To report a case of autoimmune nodopathy (AN) with concurrent serum and CSF immunoglobulin (Ig)G4 anti-neurofascin 155 (NF155) and anti-GD1b antibodies. METHODS: A 20-year-old male presented distal weakness of the 4 limbs, hypoesthesia, absent tendon reflexes and sensory ataxia. Nerve conduction studies (NCS), MRI, and autoantibody tests were performed. RESULTS: NCS revealed a diffuse demyelinating neuropathy in the peripheral nerve with motor and sensory involvement. MRI of the cervical and lumbar plexus showed diffuse enlargement. IgG4 anti-NF155 antibodies in both serum and CSF

Endoscopic Posterior Approach for Cervicothoracic and Upper Thoracic Foraminotomies: 2-Dimensional Operative Video.

Lower cervical and cervicothoracic radiculopathies can be challenging to treat through an anterior approach in patients with short-statured necks. With unilateral pathology, a posterior foraminotomy affords preservation of motion and avoids risks to anterior structures; yet, traditional open or even tubular retractor-based open systems are associated with postoperative muscle pain. Endoscopic approaches reduce muscle retraction and resection and are associated with shorter recovery time. This video demonstrates the endoscopic technique for performing cervicothoracic and thoracic foraminotomies

Comparative clinic study on vascularized and nonvascularized full-length phrenic nerve transfer.

In order to understand whether the vascularizing procedure has any clinical value in nerve transfer and grafting, we compared nonvascularized and vascularized full-length phrenic never transfers in patients with a brachial plexus injury. Full-length phrenic nerve transfer to the musculocutaneous nerve had been conducted by the technique of video-assisted thoracic surgery in 15 patients. Three kinds of procedures were carried out. The first involved retaining the initial point of the phrenic nerve and dissecting the full-length distal nerve. The second involved keeping the cervical segment and

Neuronal activation of brain vagal-regulatory pathways and upper gut enteric plexuses by insulin hypoglycemia.

Neuronal activation of brain vagal-regulatory nuclei and gastric/duodenal enteric plexuses in response to insulin (2 U/kg, 2 h) hypoglycemia was studied in rats. Insulin hypoglycemia significantly induced Fos expression in the paraventricular nucleus of the hypothalamus, locus coeruleus, dorsal motor nucleus of the vagus (DMN), and nucleus tractus solitarii (NTS), as well as in the gastric/duodenal myenteric/submucosal plexuses. A substantial number of insulin hypoglycemia-activated DMN and NTS neurons were choline acetyltransferase and tyrosine hydroxylase positive, respectively, whereas the

Persistent primitive sciatic artery associated with other various anomalies of vessels.

A left persistent primitive sciatic artery was observed in a Japanese male cadaver. The sciatic artery arose from the internal iliac artery and perforated the ventral division of the sacral plexus. The sciatic artery did not anastomose with the perforating arteries nor the popliteal artery. The left femoral artery was incompletely developed, attenuating and terminating as the saphenous artery. Instead of the femoral artery, direct continuation of the profunda femoris artery, which probably corresponded to the fourth perforating artery, became the popliteal artery. Other vessel anomalies were o

[Thoracic outlet syndrome].

The term thoracic outlet syndrome (TOS) defines neuro-vascular disorders produced in the upper extremities due to the external compression of the brachial plexus, subclavian artery and vein (neurovascular bundle) within the thoracic outlet. Thoracic outlet syndrome may be produced by bone malformations, fibromuscular anomalies, neck, and shoulder injuries, and postural defects. Clinical symptoms of this syndrome are diversified. However, neurological disorders are diversified. However, neurological disorders are prevailing. Thoracic outlet syndrome is two-fold more frequent in women than in me

Report of a non-looped variant of ansa cervicalis with omohyoid innervation from accessory nerve branch and omohyoid attachment to mastoid process.

INTRODUCTION: A variant of the innervation of the infrahyoid neck musculature is reported in which the typical looped ansa cervicalis structure is absent. In this variant, the infrahyoid muscles (sternohyoid, sternothyroid omohyoid and thyrohyoid) were innervated by a presumptive superior root of "ansa cervicalis" traveling with vagus nerve (CN X) and not branching from hypoglossal nerve (CN XII). The omohyoid muscle, typically innervated by the inferior root of ansa cervicalis, is instead innervated by nerve fibers branching from the accessory nerve (CN XI). This formation created a non-loopi

Cranial and cervical nerve injuries after repeat carotid endarterectomy.

BACKGROUND AND PURPOSE: The incidence of cranial and/or cervical nerve injuries after primary carotid endarterectomy (CEA) ranges from 3% to 48%; however, the clinical outcome of these injuries after repeat CEA has not been thoroughly analyzed in the English-language medical literature. This prospective study analyzes the incidence and outcome of cranial nerve injuries after repeat CEA. PATIENTS AND METHODS: This study includes 89 consecutive patients who had repeat CEAs. Preoperative and postoperative cranial nerve evaluations were performed, including clinical examinations (neurologic) and

Sturge-Weber syndrome: a patient with a cervical port-wine nevus.

An unusual case with Sturge-Weber syndrome is reported. A computed tomography study revealed the presence of gyriform calcifications in the occipital lobe, and discovered a hidden occipital subcutaneous port-wine nevus, instead of the usually expected nevus in the distribution of the first division of the trigeminal nerve. Existence of an occipital port-wine nevus, which was in the distribution of the greater occipital nerve, suggested a variation with respect to the embryogenesis of the Sturge-Weber syndrome.

[The surgical treatment of cervical disk pathology. A review of cases. The long-term clinical and radiographic results].

The experience of the Clinica Ortopedica dell'Università di Firenze in the surgical treatment of cervical disk disease has changed over time due to both the ongoing improvement of surgical techniques and the availability of ever more effective and less invasive diagnostic tests. Although our first experience was indirect and we were not able to review all patients due to the long follow-up period, we believe that our current setup provides valid data, as is demonstrated by the thorough clinical and radiographic evaluation we performed on patients reviewed after a minimum of 5 years. We believe
