The Unique Case of Lumbar Dural Metastasis From Cervical Carcinoma on Pembrolizumab.

Lumbar metastasis is a rare manifestation of cervical carcinoma which may present as lower extremities symptoms. Immune-checkpoint inhibitors like pembrolizumab, used for the treatment of various metastatic carcinomas, have been linked to Guillain-Barré Syndrome (GBS), which is characterized by ascending weakness and areflexia. A 52-year-old female with metastatic cervical cancer on pembrolizumab for two months presents with lower back pain, progressive bilateral lower extremities ascending weakness, and areflexia of affected limbs. Cerebrospinal fluid analysis showed albuminocytologic disasso

Dysphagia as a symptom of anterior cervical hyperostosis - Case report.

Degenerative lesions with hyperostosis on the anterior surface of cervical spine are common in clinical practice. In addition to pain being … swallow difficulties was ineffective. CT revealed a bone excrescence on the anterior surface of two cervical vertebrae which caused an oesophageal obstruction and compression of the vocal folds. Structural abnormalities of cervical spine should be considered in differential diagnosis of symptoms from the oesophagus and upper respiratory tract, especially when a firs …

Low-level mesodermal somatic mutation mosaicism: late-onset craniofacial and cervical spinal hyperostoses.

Craniofacial and cervical spinal hyperostoses are rarely seen in the absence of other abnormalities. Only seven patients with isolated cranial hyperostoses have been reported, and only a single patient with both calvarial and cervical vertebral hyperostoses. We report on an adult with late-onset right-sided asymmetrical hyperostoses of the cranium, mandible, and cervical vertebrae in the absence of an AKT1 mutation. At presentation, the patient displayed neither generalized overgrowth nor dysregulated adipose tissue. Standard polymerase chain reaction and Sanger sequencing of DNA extracted fro…

Elective cesarean delivery in a parturient with Klippel-Feil syndrome.

Klippel-Feil syndrome is defined by congenital fusion of two or more cervical vertebrae and can be associated with abnormalities in multiple systems. Management poses challenges to the anesthesiologist, particularly in pregnancy. Cervical spine immobility and instability can make the management of the airway fraught with danger and vertebral column distortion may make neuraxial anesthesia unreliable. We present the management of a nulliparous patient with features consistent with Type I Klippel-Feil syndrome undergoing elective cesarean delivery. The patient had a potentially difficult airway …

Hematogenous cervical spondylodiscitis after severe burn injury.

A 47-year-old man sustained a 31% TBSA burn injury. In spite of early escharectomy and mesh-graft-transplantation the patient suffered a septicaemic phase in the first week, which was treated by a specific antibiotic. Five weeks after the burn injury a cervical spondylodiscitis was diagnosed. Immediate wound debridement, ventral and dorsal spondylodesis with a tricortical bone-graft from the left iliac crest and titanium plates and specific antibiotic therapy led to the stabilization and healing of the cervical spinal column. The spondylodiscitis was microbiologically proved to be hematogenous

Preoperative embolization of cervical spine tumors.

PURPOSE: To assess the technical success rate, complications, and effect on intraoperative blood loss of preoperative transarterial embolization of cervical spine tumors. METHODS: A retrospective analysis was performed on 38 patients with tumors of the cervical spine; 69 vertebrae were affected. Polyvinyl alcohol particles, coils, gelfoam particles, either alone or in combination, were used for preoperative tumor embolization. After embolization a total of 57 corporectomies with titanium basket implantation were performed. RESULTS: In 36 of 38 patients, complete (n = 27) or partial (n = 9) e

Successful surgical management of cervical tuberculous spondylitis with tetraparesis in a 17-year-old patient: A case report.

Cervical tuberculous spondylitis is a rare and potentially life-threatening manifestation of spinal tuberculosis, accounting for only 3%-5% of all cases of spinal tuberculosis This report describes a case of a 17-year-old male patient who developed tetraparesis 2 weeks prior to hospital admission, after a 6-month record of progressive neck pain. Magnetic Resonance Imaging revealed tuberculous spondylodiscitis involving multiple vertebrae, with a severe spinal cord compression at C2-C3 due to an intraosseous abscess and paravertebral soft tissue mass. The patient underwent anterior cervical cor

M6-C prosthesis failure in hybrid construction : a double case report.

Cervical disc arthroplasty (CDA) is a well known treatment for single level cervical disc degeneration. In multi level disc degeneration anterior cervical disc fusion (ACDF) is often combined with a CDA. In this hybrid construction, motion is preserved at the level of the CDA. This will give less stiffness in the neck and decreases stress at the adjacent level. However, there is a lack of literature about the long term efficacy, safety and complication rate of CDA in hybrid constructions. In this article we present two cases of spontaneous destruction of a M6-C prosthesis in a hybrid construct

Pathological burst fracture in the cervical spine with negative red flags: a case report.

OBJECTIVE: To report on a case of a pathological burst fracture in the cervical spine where typical core red flag tests failed to identify a significant lesion, and to remind chiropractors to be vigilant in the recognition of subtle signs and symptoms of disease processes. CLINICAL FEATURES: A 61-year-old man presented to a chiropractic clinic with neck pain that began earlier that morning. After a physical exam that was relatively unremarkable, imaging identified a burst fracture in the cervical spine. INTERVENTION & OUTCOMES: The patient was sent by ambulance to the hospital where he was d

Laryngeal Osteoblastoma: Unusual Location in Arytenoid Cartilage.

Osteoblastoma is a benign bone tumor with a slow growing and local aggressive nature. This tumor type is usually localized in the cervical vertebra and rarely involves laryngeal cartilage structures. Very few osteoblastoma cases have been reported in the literature. Here we present laryngeal osteoblastoma originating from arytenoid cartilage in a patient with a history of cordectomy due to glottic squamous cell carcinoma.

Recent onset neck pain with associated neurological deficit--Pott's disease remains an important differential diagnosis.

The incidence of spinal tuberculosis is increasing in developed nations. In Ireland, half of all cases seen in the most recent decade for which figures are available were diagnosed in 2005-2007, the three most recent years for which there is complete data. We discuss a patient who presented with neurological complications due to destructive spinal tuberculous disease affecting the sixth cervical vertebra.

Aneurysmal bone cyst: a report of three cases.

Three children presenting with aneurysmal bone cysts are described. The first patient was 10 months old with a cyst of the scapula. The second was more typical but his cyst was treated initially as a malignant tumour. In the third child the second cervical vertebra was affected which posed considerable problems of management; it was treated by radiotherapy. Despite the problems all 3 children have made a good recovery.

TENS attenuates response to colon distension in paraplegic and quadriplegic rats.

Individuals with spinal cord injuries above thoracic level 6 experience episodic bouts of life-threatening hypertension as part of a condition termed autonomic dysreflexia (AD). The hypertension can be caused by stimulation of the skin, distension of the urinary bladder or colon, and/or muscle spasms. Transcutaneous electrical nerve stimulation (TENS) may reduce the severity of AD because TENS has been used to inhibit second-order neurons in the dorsal horn. Therefore, we tested the hypothesis that TENS attenuates the hemodynamic responses to colon distension. Eleven Wistar rats underwent spin

Paravertebral foramen screw fixation for posterior cervical spine fusion: biomechanical study and description of a novel technique.

OBJECTIVE Fusion surgery with instrumentation is a widely accepted treatment for cervical spine pathologies. The authors propose a novel technique for subaxial cervical fusion surgery using paravertebral foramen screws (PVFS). The authors consider that PVFS have equal or greater biomechanical strength than lateral mass screws (LMS). The authors' goals of this study were to conduct a biomechanical study of PVFS, to investigate the suitability of PVFS as salvage fixation for failed LMS, and to describe this novel technique. METHODS The authors harvested 24 human cervical spine vertebrae (C3-6) f

Incidence of cervical kyphosis and cervical posterior translation in neck pain associated with radiological evidence of degenerative disc disease.

BACKGROUND: Cervical kyphosis mechanically influences the intervertebral discs, which may lead to neck, shoulder, and scapular pain. OBJECTIVES: To investigate the incidence of cervical kyphosis and posterior displacement of cervical vertebrae (cervical posterior translation index) in patients with radiologically evident degenerative disc disease. DESIGN: Retrospective-cohort study. METHODS: Ninety-five patients with radiologically evident degenerative disc disease were recruited at our clinic from 2015 to 2019. Radiography images were used to analyze the straight-line index in each patient…

Cervical curvature variations in patients with infraocclusion.

The purpose of this study was to observe the variations of cervical curvature in patients with infraocclusion, and to compare this with the controls. In this study, the infraocclusion criteria were defined with the Pr-id as <17 mm on the cephalometric image. The subjects were 32 patients with infraocclusion, and 28 controls which matched the distribution for gender and age. The six points of inquiry were as follows: (i) cervical vertebra height, (ii) neck alignment, (iii) ratio of lower facial height, (iv) vertical dimension of occlusion, (v) cervical angle and (vi) occlusal angle. In over 90%

[Sports injuries of the cervical spine (author's transl)].

The article reports on injuries of the cervical spine occurring during sports activities. An attempt is made to reconstruct the movements which led to the cervical spine injuries in question. In two cases of accidents occurring during bathing, one football accident and a toboggan accident, the injuries concerned point to hyperextension of the cervical spine as cause of the injury. In another football accident and a riding accident, the changes observed allow us to conclude that the movement leading to the injury must have been a hyperflexion. One accident occurring while jumping on the trampol
