Plasmacytoma is a malignant tumour composed of plasma cells. Most commonly this takes the form of a plasma cell infiltration of bone marrow-multiple myeloma. This may be seen radiologically as either discrete lytic lesions or diffuse osteoporosis. Plasma cells are seen on bone marrow biopsy, and monoclonal immunoglobulins may occur in plasma and/or urine. Less frequently, plasma cell tumours may present as a solitary myeloma of bone, which often progresses to multiple myeloma, or as a plasma cell leukemia. We conducted a case-control study, using 429 cases with histologically confirmed sigmoid adenoma, 75 cases with rectal adenoma, and 3101 controls showing normal colonoscopy at least up to 60 cm from the anus. Forty-one consecutive patients with acute sigmoid volvulus were prospectively randomized into 2 groups to compare percutaneous deflation prior to emergency tube decompression followed electively by colopexy with banding (n = 20) versus emergency tube decompression followed electively by sigmoid colectomy (n = 21). The case notes of 15 patients undergoing colonic resection for sigmoid volvulus at St Mark's Hospital over 25 years have been reviewed. Eight patients underwent sigmoid colectomy, four left hemicolectomy and three total colectomy. Although 12 of the 15 patients complained of a constipated bowel habit for 'all their lives' prior to operation, all but two had a much improved bowel habit thereafter, regardless of the extent of the resection. If there is evidence of acute or recent sigmoid volvulus at operation, sigmoid colectomy alone is recommended in the first instance.

We reviewed a 30-year experience in management of 129 patients with 163 acute obstructions due to sigmoid volvulus. Recurrent obstruction of the colon was observed in 47 (or 45%) of 104 patients who survived their initial obstructive episode: 61% after rectal tube insertion, 45% after detorsion, 33% after sigmoid plication, and 21% despite sigmoid colectomy. A review of the barium enema studies in 73 patients with diverticular disease of the colon and with and without associated neoplasm leads to the conclusion that the correct radiologic recognition of an associated neoplasm in a colon with diverticulitis or diverticulosis can be made only about half the time. The limitations of the radiographic method and radiologic findings in distinguishing diverticulitis from carcinoma should be more clearly recognized by the clinician.

A patient with stricture of the descending colon secondary to use of a pneumatic hammer is presented. Barium enema and angiographic studies suggested that the etiology was a traumatic vascular injury. Barium granulomata, found in the resected specimen, were presumably due to barium having entered the devitalized tissues during an air contrast study performed during an acute ischemic colitis. The mechanisms of traumatic strictures of the colon are discussed.

A ureterocolic fistula in a renal transplant patient is a rare complication. Even rarer is a ureterocolic fistula involving a native kidney in a patient with a functional, ipsilateral transplanted kidney, with no prior cases published in the English literature. In the current case report, we describe a patient with a previously successful right renal transplant, who presented three years later with a ureterocolic fistula involving the right native kidney and the sigmoid colon.
