Making the Cut: Parathyroidectomy Before or After Kidney Transplantation?

BACKGROUND: Hyperparathyroidism is common in patients with end-stage kidney disease and may persist after kidney transplantation (KT). Parathyroidectomy (PTx) is curative, but whether PTx should be performed before or after KT remains controversial. There is concern that PTx can adversely affect renal allograft function if performed post-KT and result in persistent hypocalcemia. This study evaluated outcomes and postoperative complications of PTx before and after KT at our institution. METHODS: We performed a retrospective review of patients at our center (1/2012-2/2019) who had PTx either pr

Retrospective observational study of the management of multiple sclerosis patients with resistant spasticity in Spain: the '5E' study.

BACKGROUND: Multiple sclerosis spasticity (MSS) is a common and disabling symptom for which a number of antispastic agents are available; however, evidence-based guidelines for optimal management are lacking. OBJECTIVE: This retrospective observational assessment investigated the current management approach for resistant MSS in Spain. Secondary objectives were to evaluate the evolution of MSS and to estimate the social and health-related costs of managing MSS in the Spanish healthcare system. METHODS: A retrospective analysis was performed using case records from 212 MS patients with spastic

Living alone and positive mental health: a systematic review.

BACKGROUND: Living alone has become more common in today's societies. Despite the high number of the population living alone, research directed towards the mental wellbeing issues related to living alone has been limited. This systematic literature review aimed to assess the association between living alone and positive mental health. METHODS: We conducted searches in Medline, Web of Science, Cochrane Library, CINAHL, PsycINFO, and other complementary databases from January 1998 to May 2019. Randomised trials and observational studies investigating adults over 18 years of age and living alone

Anifrolumab treatment improves patient-reported quality of life and decreases disease activity and corticosteroid use in patients with systemic lupus erythematosus: A qualitative study in Denmark.

Anifrolumab is a new therapeutic approach for individuals with systemic lupus erythematosus (SLE) directed at blocking the type 1 interferon pathway. Despite the expanding body of literature on Anifrolumab, an essential aspect remains absent: the subjective patient experience of treatment effects and implications on patients' health-related quality of life (HRQoL). The present study aimed to fill this void by elucidating the nuanced perspectives of SLE patients receiving Anifrolumab treatment by conducting qualitative in-depth interviews (IDIs). SLE patients at Aarhus University Hospital who h

Scoping review on the prioritisation of high-consequence infectious pathogens for research preparedness and response to health emergencies.

BACKGROUND: Funding for health research is intrinsically limited; therefore, priority-setting is necessary to optimise investments. Prioritisation is particularly important for epidemic and pandemic pathogens, as preparedness needs for hypothetical threats can be difficult to assess, and response during outbreaks requires accelerated decision-making. Exercises which prioritise epidemic and pandemic pathogens have been published; however, to our knowledge, there has been no systematic analysis of how, why and by whom these exercises are conducted. Guidelines and good practices for prioritisatio

A Descriptive Study on the Role of Magnetic Resonance Imaging in Staging Avascular Necrosis of the Hip Joint: Current Trends and Insights.

BACKGROUND: Avascular necrosis (AVN) of the hip, also known as osteonecrosis or ischemic necrosis, is cellular necrosis of bone and marrow elements. Although clinical evaluation is the first step in assessing the hip joint, imaging is fundamental for accurate diagnosis and staging of AVN. OBJECTIVE: We aim to assess the role of magnetic resonance imaging (MRI) in evaluating AVN of the hip, describe imaging features, and classify AVN using various classification systems. METHODS: A cross-sectional study was conducted over two years at the Department of Radiodiagnosis, Aarupadai Veedu Medical

Vibration, current, torque, RPM dataset for multiple fault conditions in industrial-scale electric motors under randomized speed and load variations.

This article presents a comprehensive dataset acquired from two fault diagnosis environments: (1) industrial AC motors operating under various real-world conditions, and (2) belt-loosening scenarios in HVAC air handling units. The dataset was collected to support the development and validation of data-driven fault detection methods across diverse mechanical and electrical systems. For the AC motor dataset, faults were deliberately introduced to simulate common degradation modes, including coil winding faults, inter-phase short circuits, misalignment, and bearing-related issues such as rolling-

Influence of DNA methylation and chromatin accessibility on regulation of gene expression during Trichomonas vaginalis-host cell interaction.

UNLABELLED: Trichomonas vaginalis is a common sexually transmitted parasite that colonizes the human urogenital tract, causing infections that range from asymptomatic to highly inflammatory. As an extracellular pathogen, adherence to host epithelial cells is an important step to colonize the human host. Hence, understanding how the process of attachment to host cells is regulated remains an important goal in parasitology research and human health. T. vaginalis-host interaction is regulated by changes in gene expression, but it is still largely unknown how these changes in transcriptional profi

Comprehensive in silico genomics analysis of global trends and host-specific emergence of aminoglycoside resistance in Staphylococcus aureus: a One-Health perspective.

BACKGROUND: Aminoglycosides remain clinically valuable against Staphylococcus aureus. Aminoglycoside resistance in S. aureus represents a critical One Health concern and is primarily driven by aminoglycoside-modifying enzymes (AMEs), which are frequently plasmid-encoded. Although regional studies have provided valuable insights, the global epidemiology of aminoglycoside resistance determinants remains poorly characterized because comprehensive data integrating human, animal, and environmental reservoirs are still lacking. This study addresses this gap by analyzing over 110,000 S. aureus genome

Subgrouping of ESBL-producing Escherichia coli from animal and human sources: an approach to quantify the distribution of ESBL types between different reservoirs.

Escherichia (E.) coli producing extended-spectrum beta-lactamases (ESBLs) are an increasing problem for public health. The success of ESBLs may be due to spread of ESBL-producing bacterial clones, transfer of ESBL gene-carrying plasmids or exchange of ESBL encoding genes on mobile elements. This makes it difficult to identify transmission routes and sources for ESBL-producing bacteria. The objectives of this study were to compare the distribution of genotypic and phenotypic properties of E. coli isolates from different animal and human sources collected in studies in the scope of the national

High interferon response signatures in SLE patient leukocytes are associated with increased transposable element expression in gene introns and intergenic regions.

BACKGROUND: Systemic lupus erythematosus (SLE) is a spontaneous systemic auto-immune condition for which the inciting factors and genetic basis are generally unknown. Although heterogeneous in its manifestations and severity, SLE involves chronic inflammation along with sustained autoantibody production. The root causes and pathophysiology of the inflammation and breaches of tolerance are incompletely understood, but neutrophils are thought to be important elements of the pathophysiology. Type I interferons (IFN) in the bloodstream and an IFN-stimulated gene (ISG) signature in circulating leuk

Medication burden of Saudi Arabian women receiving antiresorptive therapy.

BACKGROUND AND PURPOSE: Osteoporosis is common in the Saudi Arabian population, and its successful treatment requires full compliance. Patients who require antiresorptive therapy, such as oral bisphosphonates, may suffer from other diseases requiring medications, which increases the medication burden and ends up in drug noncompliance on the part of patients, making them vulnerable to osteoporosis-related fractures. We decided to undertake this study to analyze the concomitant medications that osteoporotic patients are receiving at King Fahd Hospital of the University, Al Khobar. METHODS: Oste

Systematic Analysis of Mobile Genetic Elements Mediating β-Lactamase Gene Amplification in Noncarbapenemase-Producing Carbapenem-Resistant Enterobacterales Bloodstream Infections.

Noncarbapenemase-producing carbapenem-resistant Enterobacterales (non-CP-CRE) are increasingly recognized as important contributors to prevalent carbapenem-resistant Enterobacterales (CRE) infections. However, there is limited understanding of mechanisms underlying non-CP-CRE causing invasive disease. Long- and short-read whole-genome sequencing was used to elucidate carbapenem nonsusceptibility determinants in Enterobacterales bloodstream isolates at MD Anderson Cancer Center in Houston, Texas. We investigated carbapenem nonsusceptible Enterobacterales (CNSE) mechanisms (i.e., isolates with c

A Predictive Analysis of Wall Stress in Abdominal Aortic Aneurysms Using a Neural Network Model.

Rupture risk assessment of abdominal aortic aneurysms (AAAs) by means of quantifying wall stress is a common biomechanical strategy. However, the clinical translation of this approach has been greatly limited due to the complexity associated with the computational tools required for its implementation. Thus, being able to estimate wall stress using nonbiomechanical markers that can be quantified as a direct outcome of clinical image segmentation would be advantageous in improving the potential implementation of said strategy. In the present work, we investigated the use of geometric indices to

Motivations for enrollment in a COVID-19 ring-based post-exposure prophylaxis trial: qualitative examination of participant experiences.

BACKGROUND: Ring-based studies are a novel research design commonly used for research involving infectious diseases: contacts of newly infected individuals form a ring that is targeted for interventions (e.g., vaccine, post-exposure prophylaxis). Given the novelty of the research design, it is critical to obtain feedback from participants on their experiences with ring-based studies to help with the development of future trials. METHODS: In 2021, we conducted 26 semi-structured interviews with adult participants of a COVID-19 ring-based post-exposure prophylaxis trial based in Canada. We appl

AtrR Is an Essential Determinant of Azole Resistance in Aspergillus fumigatus.

Aspergillosis associated with azole-resistant Aspergillus fumigatus has a mortality rate that can approach 90% in certain patient populations. The best-understood avenue for azole resistance involves changes in the cyp51A gene that encodes the target of azole drugs, lanosterol α-14 demethylase. The most common azole resistance allele currently described is a linked change corresponding to a change in the coding sequence of cyp51A and a duplication of a 34-bp region in the promoter leading to a tandem repeat (TR). Our previous studies identified a positively acting transcription factor called A
