The left ear of a 57-year-old female who suffered sudden deafness during the course of relapsing polychondritis was examined under a scanning electron microscope. Marked degeneration of the organ of Corti in all turns and dislocated and encapsulated tectorial membrane were found in the cochlea. Marked decrease in number of the sensory cells in the utricular and saccular maculae and total loss of sensory hair bundles in the ampullary cristae of the semicircular canals were seen in the vestibule. These findings strongly suggest that the cause of sudden deafness in this case might be viral. The u…

A 27-year-old man with a prosthetic mitral valve had bilateral cerebral infarcts that caused a nonfluent aphasia, oral apraxia, and deafness. A computer-assisted tomographic scan showed symmetrical bilateral temporoparietal lesions. A review of the literature on other cases of cortical auditory deficits suggests that the clinical syndrome of pure word deafness in many cases is probably a less severe form of cortical deafness and is due to less extensive bilateral temporal gray matter lesions. However, strictly white matter lesions may produce some cases of either syndrome.

Two families are presented in which affected members had deafness, preauricular sinuses, branchial fistulae and malformation of the external ear. In Family I, a renal abnormality was an associated feature. In Family 2, two individuals had an abnormality of the incudostapedial joint. In both families inheritance was autosomal dominant with variable expressivity. The occurrence of external-ear malformations, branchial fistulae, and preauricular sinuses should indicate the need to search for a hearing loss not only in the patient but amongst relatives.

Many disorders may result in delay of language development. There is on the one hand a large group of children with bilateral peripheral deafness, most of whom will be amenable to appropriate treatment, and on the other a group, possibly more numerous, who have a variety of disorders of speech and language, varying from lack of speech associated with severe mental retardation to more specific defects, such as congenital dyslexia and developmental articulatory dyspraxia. These disorders are probably due to a defect of cerebral function at the highest level and in these children no-one will have…
