The influence of 90 h of acute nutritional deprivation (ND; water ad libitum only) on in vitro contractile and fatigue properties, muscle fiber type proportions, and cross-sectional areas (CSA) of the adolescent rat diaphragm was determined. An infant who presented with signs of duodenal atresia had proximal duodenal obstruction by upper gastrointestinal barium study, but had a small amount of air in the distal bowel. Exploration showed an annular pancreas and microperforation of a duodenal diaphragm. Prior to concluding that pancreaticobiliary duct anomalies are the path of air into the distal bowel in patients with duodenal atresia, microperforation of a duodenal diaphragm must be excluded.

The effect of perinatal undernutrition on the postnatal elimination of immature myosin isoforms in rat diaphragm muscle was examined using electrophoretic and immunocytochemical techniques. From 1970 to 1987, 54 patients were treated for rupture of the diaphragm as a result of blunt trauma, mostly due to road accidents. In more than 75% of the cases, other severe post-traumatic lesions were noted in association with the rupture. There were 46 lesions in the left hemidiaphragm. Both Hg2+ and CH3Hg+ exert a depressive action on the twitch amplitude elicited by nerve stimulations more rapidly than that by muscle stimulation. The authors report two cases of congenital abnormal rightsided high insertion of the diaphragm. Though there was no acute respiratory distress in the neonatal period, both presented other congenital deformities that required further investigation. At one month of age, both children developed severe respiratory problems, leading to a semi-urgent surgical intervention, that revealed the final diagnosis of high insertion. The discussion, following the embryological development, underlines the rarity of this anomaly.

The vaginal diaphragm is currently less popular than other contraceptive methods, such as the pill, IUDs or surgical sterilization. It is however, regarded as one of the safest methods of contraception available and complications of diaphragm use are rare. This report describes a case in which a vesicovaginal fistula and genitourinary tract infections complicated the use of this contraceptive method.

For 25 years in the clinic there were 21 children surgically treated for the central (sliding) hernia of the esophageal opening of the diaphragm. No recurrences were noted in the postoperative period. Two patients were additionally subjected to surgical procedures in order to liquidate esophageal stenosis. Remote results of the treatment were good. After operation the opening and closing of the cardia were normal. No gastroesophageal reflux was noted. The patients were practically healthy.

We have developed a percutaneous technique for placement of electromyogram electrodes in cat diaphragmatic crura. The technique is convenient and relatively atraumatic. Guided by external landmarks, small gauge hypodermic needles are used to advance electrodes through implant sites within or in contact with the crural portions of the diaphragm. Satisfactory EMG is reliably obtained from both acute and chronic preparations.

Among the 17,032 women included in the Oxford-Family Planning Association contraceptive study, 65 developed biopsy proven cervical neoplasia (including dysplasia) prior to 1 September 1977. The incidence rate in diaphragm users (0.17 per 1000 woman-years of observation) was much lower than the rates in oral contraceptive users or intrauterine device users (0.95 and 0.87 respectively).
