Patient Characteristics, Patterns, and Repair of Aneurysms in Human Immunodeficiency Virus-Positive Patients.

BACKGROUND: Human immunodeficiency virus (HIV) is a multisystem disease and is associated with vascular complications including aneurysm formation. HIV-associated aneurysms are well documented and may present in unusual locations with concerning features. However, the literature regarding aneurysms in HIV-positive patients is limited to case series with limited data regarding aneurysm patterns. Furthermore, several small series have documented poor outcomes with surgical repair. Thus, our aim was to investigate the characteristics, patterns, and repair of aneurysms in HIV-positive patients in

Proximal stent graft combined with distal bare stent vs. conventional thoracic endovascular aortic repair in the treatment of complicated Stanford type B aortic dissection: a systematic review and meta-analysis.

BACKGROUND: The combination of a proximal stent graft (PSG) and a distal bare stent (DBS) has a number of advantages over standard thoracic endovascular aortic repair (TEVAR) in the treatment of complicated Stanford type B aortic dissection (TBAD). However, direct comparisons between these two treatment approaches remain limited. This meta-analysis aimed to compare the efficacy of these techniques in the treatment of complicated Stanford TBAD. METHODS: The Cochrane Library, Embase, PubMed, and Web of Science databases were searched using the following key terms: "provisional extension to indu

Rescue baroreflex activation therapy after Stanford B aortic dissection due to therapy-refractory hypertension.

Clinical trials have demonstrated significant and durable reduction in arterial pressure from baroreflex activation therapy (BAT) in patients with resistant arterial hypertension. There is a lack of data, however, concerning the use of BAT in a rescue approach during therapy-refractory hypertensive crisis resulting in life-threatening end-organ damage. Here, we describe the first case in which BAT was applied as a rescue procedure in an intensive care setting after ineffective maximum medical treatment. A 34-year-old male patient presented with Stanford B aortic dissection and hypertensive cri

A Novel Technique for Total En bloc Spondylectomy of the Fifth Lumbar Tumor Through Posterior-Only Approach.

STUDY DESIGN: A retrospective study. OBJECTIVE: To describe a new surgical technique for total en bloc spondylectomy (TES) of the fifth lumbar (L5) tumor and evaluate the efficacy and safety of this new technique. SUMMARY OF BACKGROUND DATA: TES has been considered an optimal treatment for tumor, including certain spinal tumors, but it requires a combined posterior-anterior approach, which is often complicated by a long operation time, considerable blood loss and severe trauma. METHODS: Seven patients with primary or solitary metastatic tumors of L5 were treated with this new technique in o

Open Surgery of the Descending Thoracic Aorta in the Endovascular Era: A Single Centre Experience.

OBJECTIVE: Although thoracic endovascular aortic repair is the predominant treatment for most descending thoracic aorta (DTA) pathologies, a subset of patients remain unsuitable, leaving open surgical repair as their only viable option. This study reports the outcomes of open DTA repair performed in a high volume tertiary aortic referral centre. METHODS: Data on patients who underwent open repair for DTA pathology from 2013 to 2024 were collected retrospectively. Baseline characteristics, operative details, and post-operative outcomes were reviewed. The primary outcome was operative death; se

Recurrent aneurysm 11 years after thoracoabdominal aortic replacement due to recanalization of intercostal arteries of the wrapping native aorta in Marfan syndrome: a case report.

BACKGROUND: Persistent blood flow is common in aortic aneurysms following endovascular repair. However, the recanalization of thrombosed intercostal arteries after open aortic repair is rare. We report a case of aortic aneurysm recurrence 11 years after thoracoabdominal aortic replacement resulting from recanalization of the intercostal arteries like a type II endoleak in a patient with Marfan syndrome. CASE PRESENTATION: A 50-year-old woman with Marfan syndrome presented with left-sided back pain after meals. She underwent abdominal aortic replacement at 35 years of age, distal aortic arch a

Unique Coexistence of Left Inferior Suprarenal, Accessory Renal, and Arched Testicular Arteries Arising From a Common Trunk: A Rare Vascular Variation With Dual Left Testicular Veins.

Visceral blood vessels often display significant anatomical variability, with testicular artery (TA) anomalies more frequently observed on the left side. During a routine cadaveric dissection of an 88-year-old Caucasian male with no known kidney or testicular disease, a unique vascular pattern was identified. A left common trunk arose from the abdominal aorta at approximately the level of L2 vertebra, positioned partially behind the left renal vein. This trunk ascended and bifurcated into superior and inferior branches. The superior branch gave rise to a left inferior suprarenal artery and a l

Intraluminal morphometric analysis of abdominal aortic branches: spatial landmarks for interventional radiology and colorectal surgery.

PURPOSE: Precise knowledge of the spatial relationships of abdominal aortic branches is essential for safe colorectal surgery and interventional radiologic (IR) procedures. While most studies describe these vessels from an external or imaging perspective, intraluminal morphometric data relevant to catheter-based navigation remain limited. This study aimed to evaluate the positional relationships and morphometry of major abdominal aortic branches from an intravascular perspective. METHODS: Intravascular dissections were performed in 19 formalin-embalmed cadavers (12 males, 7 females; age range

Pregnancy with aortic dissection in Ehler-Danlos syndrome. Staged replacement of the total aorta (10-year follow-up).

Pregnancy complicated by aortic dissection in patients with hereditary disorder of connective tissue presents interesting considerations including management of caesarean section with the unexpected need for cardiac surgery in emergency. Generalizations can be made on management principles with long-term follow-up requiring an aggressive individualized approach by a multidisciplinary team. A 33-year-old parturient presenting an aortic dissection at 37 weeks gestation required prompt diagnosis of Ehlers-Danlos syndrome in combination with correct surgical therapy resulted in the survival of bot

Complications of endovascular aneurysm repair of the thoracic and abdominal aorta: evaluation and management.

In recent decades, endovascular aneurysm repair or endovascular aortic repair (EVAR) has become an acceptable alternative to open surgery for the treatment of thoracic and abdominal aortic aneurysms and other aortic pathologies such as the acute aortic syndromes (e.g., penetrating aortic ulcer, intramural hematoma, dissection). Available data suggest that endovascular repair is associated with lower perioperative 30-day all-cause mortality as well as a significant reduction in perioperative morbidity when compared to open surgery. Additionally, EVAR leads to decreased blood loss, eliminates th

A Case of Acute Non-ST Elevation Myocardial Infarction Later Revealed by Contrast-Enhanced Computed Tomography.

BACKGROUND Acute coronary syndrome is life-threatening. The diagnosis can be confirmed by electrocardiography (ECG) and serum cardiac biomarkers. Early diagnosis and treatment of non-ST segment elevation myocardial infarction (NSTEMI) is important because delayed treatment is associated with poor prognosis, especially in older adults. CASE REPORT An 82-year-old woman presented to the Emergency Department (ED) with epigastric and back pain. Despite the symptoms, the electrocardiogram revealed no abnormality, and the high-sensitivity cardiac troponin (Hs-cTn) value was below the detection limit.

Combined mortality and morbidity of direct surgical treatment of abdominal aortic aneurysm.

Two hundred aneurysms of the abdominal aorta were treated surgically from 1980 to 1987 by the same surgeon. There were 187 men and 13 women whose mean age was 66.1 years. Nine patients were 80-years-old or more. Eighty-seven percent of patients had preoperative risk factors, 30% of which were coronary artery disease. The operative approach was through a transverse laparotomy in 188 patients compared to 11 midline incisions and one lumbotomy. An aortoaortic tube was inserted in 87 patients, a bifurcated prosthesis in 99, and a tube bypass in 14. Five patients (2.5%) died within the 30 day perio
