"TRASH" Transolecranon Fracture-Dislocation with Occult Osteochondral Coronoid Fracture: A Case Report.

CASE: A 7-year-old girl sustained a radiographic appearance seemed harmless (TRASH) elbow lesion after falling from play equipment. Initial radiographs demonstrated a transolecranon fracture-dislocation. Additional injury was suspected, given valgus instability after reduction. Subsequent magnetic resonance imaging (MRI) revealed a large osteochondral coronoid fragment only partially visible on initial radiographs and computed tomography (CT). Successful outcomes were achieved by ulnar nerve decompression, open reduction, and fixation. CONCLUSION: TRASH lesions, including this osteochondral c

Reconstruction of a severe open distal humerus fracture with complete loss of medial column by using a free fibular osteocutaneous graft.

OBJECTIVE: Open intra-articular fractures of the distal humerus can be associated with considerable bone loss and extensive soft tissue damage. The management of these injuries is quite challenging, and the restoration of elbow anatomy may require multiple bone and soft tissue surgical procedures. The purpose of this case report is to present the option of addressing at the same time a complex skin, muscular, and bone distal humerus defect by using a composite vascularized graft. METHODS: We present a case of a high-energy open fracture of the distal humerus in a polytrauma young patient. Apa

Safe Ultrasonography-Assisted Elbow Arthroscopy Technique.

Elbow synovectomy has traditionally been performed using direct visual observation. However, recent advancements in elbow arthroscopy have increased the popularity of arthroscopic synovectomies. This minimally invasive procedure has garnered attention because of its efficacy in halting joint deterioration while minimizing patient discomfort and invasiveness. Conversely, the technical demands of arthroscopic elbow synovectomy pose challenges, often resulting in notable complications, such as nerve damage. Considering the utility of real-time nerve visualization for injury prevention, we suggest

A Rare Atypical Case of a Pulled Elbow in an Infant.

A pulled elbow is a common type of injury in children aged one to four years, where the forearm is pulled in an extended pronated position. There are a few cases of pulled elbow reported in children under one year old. We experienced an atypical pulled elbow case in a six-month-old girl after her mother rolled her from a right lateral position to a supine position, leaving her arm trapped behind her back. The pulled elbow was suspected based on the classical presentation of pain over the elbow, a less mobilized limb, and a pronated arm position as well as the exclusion of fracture from a plain

Additionally, 55% of pitchers with a UCL injury had a history of elbow injuries as an adolescent/child, compared with 18% in the uninjured group (P < .0001). Also, 72% of all surveyed pitchers agreed that fatigue over the course of a season increases the risk of UCL injuries, and the majority of pitchers agreed that inadequate …

Biologics in shoulder and elbow pathology.

In recent years, orthobiologics have been of increasing clinical interest in the treatment of shoulder and elbow pathology. In some conditions, such as rotator cuff injury and lateral epicondylitis, there have been high-quality trials that support the use of platelet-rich plasma in reducing pain, restoring functionality, and improving clinical outcomes. However, as the numbers of both cellular-based biologics and the conditions being augmented by biologics continue to expand, there is a substantial need for high-quality investigations to support their routine use in most shoulder and elbow con

Persistent posterolateral rotatory subluxation of the elbow in cubitus varus: a case report.

A 23-year-old woman who had an episode of elbow injury which was treated cast immobilisation in childhood with residual cubitus varus sustained a posterior fracture dislocation of the same elbow. She was treated with closed reduction. Six months later, however, her elbow became stiff, and a persistent posterolateral rotatory subluxation was noted. Operations were performed in two steps; first, the radial head, which was subluxed posteriorly, was reduced and the lateral collateral ligament complex was reconstructed, and second, the stiff elbow was mobilised. Two years after the first surgery, t

Understanding the recurrent pulled elbow.

Nursemaid elbow is subluxation of the radius head seen in early childhood. The aim of this study was to examine the epidemiology of recurrent dislocations and the effect of hyperlaxity and bone anatomy on recurrent dislocations in these injuries, for which the pathogenesis has not been fully clarified. The study included a total of 329 paediatric patients who presented at the Paediatric Emergency Department (ED) between January 2016 and December 2022, and were diagnosed with Nursemaid Elbow. On presentation at ED, two-directional elbow radiographs were taken of all the patients and the radius

Elbow range of motion is stable or improves following ulnar collateral ligament repairs and reconstructions.

BACKGROUND: The increasing incidence of ulnar collateral ligament (UCL) injuries in throwing athletes has resulted in a concomitant increase in annual UCL repair and reconstruction procedures. While elbow stiffness remains a small but significant postoperative complication, little data have been published regarding range of motion (ROM) trajectories postoperatively and factors that impact final recovery. This study aimed to determine the trajectory of elbow ROM recovery following UCL repairs and reconstructions and to identify the factors that impact a patient's final motion. METHODS: For our

Midterm outcomes of a short-cemented bipolar radial head arthroplasty, in a cohort of 56 cases with minimum 2-years follow-up.

BACKGROUND: Radial head prostheses are used in comminuted radial head fractures for elbow stabilisation when reduction and internal fixation is not possible. Several implant designs exist, but no large series exist about a short-cemented and bipolar implant. HYPOTHESIS: The hypothesis was that this prosthesis design shows good clinical, functional, and radiological results, with acceptable rate of complications. STUDY DESIGN: This retrospective study included the prostheses with bipolar design and short-cemented stem (Evolutive™), with a minimum 2-years follow-up. MATERIALS AND METHODS: All

Injuries to the Coronoid Process of the Ulna with Involvement of the Lesser Sigmoid Notch.

INTRODUCTION: This study aimed to describe the involvement of the lesser sigmoid notch in fractures to the coronoid process. We hypothesized that injuries to the lateral aspect of the coronoid process regularly involve the annular ligament insertion at the anterior lesser sigmoid notch. MATERIAL AND METHODS: Patients treated for a coronoid process fracture at our institution between 06/2011 and 07/2018 were included. We excluded patients < 18 years, patients with arthritic changes or previous operative treatment to the elbow, and patients with concomitant injuries to the proximal ulna. In pat

Radial head prosthesis after radial head and neck fractures - current literature and quality of evidence.

Due to the elbow joint's complex functional anatomy, the multifragmentary nature of many fractures and concomitant destabilizing associated injuries, dislocated fractures of the radial head and neck still present a serious challenge for the orthopedic surgeon. Thorough knowledge of the elbow's anatomy and biomechanics is essential to analyze and understand the injury and plan its treatment. The aim of a differentiated therapy approach is to restore the joint's anatomy and kinetics, stable and painless joint function, and to avoid or at least delay posttraumatic joint changes. The degree of dis

Effective treatment of fracture-dislocations of the olecranon requires a stable trochlear notch.

Our goal with this study was to better define and characterize fracture-dislocations of the olecranon and to provide additional data regarding complications and elbow function after operative treatment. Twenty-six patients with fracture-dislocations of the elbow were reviewed retrospectively. Ten had anterior and 16 had posterior fracture-dislocations. Five of 10 patients with anterior injuries and all of the patients with posterior injuries had an associated fracture of the coronoid process of the ulna. One of 10 patients with anterior and 13 of 16 patients with posterior injuries had fractur

Simple dislocations of the elbow: evaluation and treatment.

Simple dislocations of the elbow or dislocations occurring without an associated fracture are common injuries. Evaluation of these injuries must include an assessment of the entire involved upper extremity and a complete neurovascular examination. Principles of management include a prompt, controlled reduction, a determination of postreduction stability, and an immediate rehabilitation protocol that considers the stability of the joint following reduction. For those joints that are stable throughout the arc of motion, an unrestricted range of motion protocol can be started. When instability is

ARM: a modular hinged joint for the AO tubular external fixator.

Certain complex traumatic elbow lesions challenge the orthopaedic and trauma surgeon. If they are not treated correctly, they cause a high rate of disability, arising from elbow instablility and stiffness, either by fibrosis or joint incongruity. Injuries such as complex fractures of the proximal third of the ulna, coronoid fractures associated with radial head fractures (the "terrible triad"), are even worse if they are accompanied by soft tissue lesions. Hinged external fixators, complemented by other surgical procedures, are, for many, a recommended alternative when dealing with irreparable

Surgical approaches for nonneurogenic elbow heterotopic ossification with ulnar neuropathy.

BACKGROUND: Few reports are available concerning elbow heterotopic ossification (HO) and its optimal management in nonneurologic or nonburn patients after repetitive elbow manipulation. The unique anatomic relationship of the ulnar nerve at the elbow renders it rather vulnerable to injury when elbow HO occurs medially or posteromedially and extends into the vicinity of the cubital tunnel. METHODS: A total of 16 consecutive patients without neurologic injury were diagnosed with heterotopic ossification formation in the elbow and referred to the Upper Extremity Unit of Chang Gung Memorial Hospi
