A case of post-paraplegic, post-traumatic spasticity, alleviated by percutaneous epidural neurostimulation from implanted electrodes at the L1 and L3 intervertebral levels, is presented. Secondary beneficial physiological effects and controls of involuntary activity that were achieved were regulation of a bowel regime, production of sweating below the level of the lesion, increased piloerections, and morning penile erections. Electrical modulation of the involuntary motor and autonomic activities below a complete spinal cord injury is discussed. The main advantages of such treatment in modulat

For the first time the model of secondary contractures of facial nerves was used to study by long-term sterotaxic implanted electrodes the state of the cortex, amygdal complex, reticular formation of the middle brain, reticular nuclei of the pons and nuclei of the facial nerves. The experiments were accomplished on 31 rabbits. The experiments confirmed a close functional connnection of the VII pair of cranial nerves with the limbico-reticular complex. In secondary contracture there were factors facilitating the influence of pons reticular structures on the deficient system the facial nerve--fa

The lateral hypothalamus, and various sites within the limbic system and frontal lobe of the rhesus monkey brain were electrically stimulated using chronically implanted electrodes. A considerble increase in plasma aldosterone levels was observed after stimulation of the lateral hypothalamic area, certain localized sites in the cingulate area, and lower medial parts of the frontal lobe. Inactive sites included most of the amygdala, hippocampus, and basal ganglia, together with other areas within the frontal lobe and cingulate gyrus. Stimulation of all active areas was followed by an increase i

OBJECTIVE: Radiofrequency thermocoagulation (RF-TC) can be used to treat drug-resistant epilepsy during stereoelectroencephalography (SEEG) by implanting electrodes to create thermal lesions, although the optimal parameters and lesion strategies remain unclear. This study used computational modeling to analyze the influence of different electrode contact geometries on the shape of the coagulation zone.
METHOD: The computational model simulated parallel SEEG electrodes with hemispherical and cylindrical contacts at various distances from each other. RF-TC was applied at 5 W for up to 30 second

The treatment of essential tremor with thalamic deep brain stimulation (DBS) is considered to be more effective and to cause less morbidity than treatment with thalamotomy. Nonetheless, implantation of an indwelling electrode, connectors, and a generator is associated with specific types of morbidity. The authors describe three patients who required revision of their DBS systems due to lead breakage. The connector between the DBS electrode and the extension wire, which connects to the subclavicular pulse generator, was originally placed subcutaneously in the cervical region to decrease the ris

On September 10th 2013, the clinical medical research Lasker award winners were rewarded for their work on multichannel cochlear implant. It has been my pleasure to see that such a major topic had caught the attention of the Members of the Jury for this prestigious award. That is why I accepted an invitation to participate in a special issue of Hearing Research devoted to the three winners. Here I highlight four scientific contributions made by the French team in late 1970s and early 1980s to modern multichannel cochlear implant development. 1) Chouard and MacLeod plotted an approximate freque

Insulo-opercular epilepsies are rare, and pure opercular epilepsies are even rarer in the field of epilepsy surgery. They represent a complex form of focal epilepsy, often associated with noninformative MRI findings and, in some cases, extensive epileptogenic networks. In this context, stereoelectroencephalography (SEEG) is a critical tool for precise characterization of the epileptogenic zone (EZ) and for guiding surgical treatment; in selected cases, it may also serve as a therapeutic intervention through radiofrequency thermocoagulation (RFTC). The insulo-opercular region exhibits intricate

Due to the limited efficacy of antiarrhythmic drugs for atrial fibrillation, several nonpharmacologic therapeutic options have evolved. One of these is an implantable atrial defibrillator. Recent studies have shown that internal atrial defibrillation is feasible with relatively low energies. To date, the optimal electrode configuration involves large surface area catheters in the right atrium and coronary sinus. In humans, atrial defibrillation can generally be achieved with < 2 J using this electrode configuration and a biphasic shock waveform. For shocks < 5 J, there is no significant pathol

BACKGROUND: Transvenous lead extraction (TLE) is procedurally complex and carries significant risk. Evidence on optimal TLE techniques is limited and lacks comparative studies.
METHODS: PubMed, Embase, Cochrane Library, and Web of Science were searched through November 27, 2024. We included randomized clinical trials (RCTs) or non-randomized controlled trials (non-RCTs) comparing two or more TLE methods in adults undergoing lead extraction. A network meta-analysis was conducted to estimate pooled outcomes with 95% CIs. P-scores ranked treatments.
RESULTS: Eleven non-RCTs and one RCT were inc

Auditory brainstem implants (ABIs) can restore useful hearing to persons with deafness who cannot benefit from cochlear implants. However, the quality of hearing restored by ABIs rarely is comparable to that provided by cochlear implants in persons for whom those are appropriate. In an animal model, we evaluated elements of a prototype of an ABI in which the functions of macroelectrodes on the surface of the dorsal cochlear nucleus would be integrated with the function of multiple penetrating microelectrodes implanted into the ventral cochlear nucleus. The surface electrodes would convey most

Various objective electrophysiological techniques have been used to supplement the behavioral measurements for cochlear implant patients. This report summarizes various studies of these measurements, including stimulus artifact, averaged electrode voltages, electrical stapedius reflexes, electrical auditory brain stem responses, electrical middle latency responses, and cortical auditory responses. These techniques have been shown to provide valuable information in the evaluation of device and electrode integrity in addition to correlated values to behavioral thresholds and comfort levels.

Although cochlear implants have been reported to be easy to perform, some difficulties have been experienced in 4 of 21 operations. In order to make this surgery easier, the following points should be considered. Pre-operative CT and MRI must be performed in order to determine the condition of the cochlea. To determine the condition of the round window, exploratory tympanotomy is recommended in some cases. After insertion of the electrode in the cochlea, a radiograph should be taken during the operation to confirm that the electrode is correctly positioned.

Monitoring of electrocardiogram (ECG) and heart rate (HR) is essential in a wide range of experiments. For conscious animal studies, telemetry is the preferred approach; however, it requires 1-3 weeks of recovery after surgical device-implantation. The present paper describes a novel multi-dry-electrode plate (MDEP) sensor system to monitor ECG/HR in freely behaving mice without the need for surgery for device/electrode implantation. The MDEP sensor is a rectangular plate with 15 gold-plated stripe pattern electrodes, on which a mouse can walk around freely, and detects ECG whenever ≥2 paws (f

Single-lead atrial synchronized ventricular pacing using a floating atrial sensing electrode enables physiologic pacing in patients with complete atrioventricular block and normal sinus function. Open heart surgery performed on patient implanted with a single-lead atrial triggered ventricular VDD pacemaker is rarely reported. A 71-year-old male had been implanted with a single-lead VDD pacemaker because of Mobitz II atrioventricular block. He had a history of old myocardial infarction and diabetes mellitus. Coronary angiography revealed stenoses in the left main trunk and two main vessels. Byp

OBJECTIVES: Due to the challenges of direct in vivo measurements in humans, previous studies of cochlear tonotopy primarily utilized human cadavers and animal models. This study uses cochlear implant electrodes as a tool for intracochlear recordings of acoustically evoked responses to achieve two primary goals: (1) to map the in vivo tonotopy of the human cochlea, and (2) to assess the impact of sound intensity and the creation of an artificial "third window" on this tonotopic map.
DESIGN: Fifty patients with hearing loss received cochlear implant electrode arrays. Postimplantation, pure-tone

The weight of extraction of a pacemaker electrode from the endomyocardium was evaluated in 29 hearts (31 pacemakers). The maximum weight of extraction was 2375 g, the mean being 760 g. The weight of extraction depends on the duration of the implantation of the pacemaker, and on the thickness of the right ventricle, but there is no relationship to the shape of the pacemaker electrode, nor to the site of insertion in the right ventricular endomyocardium. In half the cases, pieces of endomyocardium adhered to the extracted electrode, the largest piece being 5 cm x 0.5 cm x 0.5 cm. The epicardium

This report summarizes data collected over the span of more than a decade from four laboratories on the sound quality of cochlear implants. Sixteen projects (ten published and six unpublished) are summarized; two video and eleven audio files accompany the summaries. Fourteen of the projects used single sided deaf patients (SSD) fit with an implant (CI) as listeners; one project used SSD-CI patients with preserved hearing (HP) in their implanted ear; one project used conventional CI patients. Approximately a third of the SSD-CI patients could create close matches to CI sound quality. The matche

PURPOSE: While the scala tympani (ST) is usually the preferred site for electrode insertion in cochlear implantation, anatomical variations and cochlear ossification may require scala vestibuli (SV) insertion. This systematic review evaluates the feasibility, techniques, and clinical outcomes of SV insertions compared to ST insertions, focusing on their impact on auditory performance.
METHODS: A systematic review was conducted using PRISMA guidelines. Studies on SV cochlear implantation were identified from databases like PubMed and EMBASE. The review included research articles reporting on a
