Emergency management of poisoning and overdose.

The management of the poisoned patient can be difficult and challenging. The task often demands the physician's entire resources in putting together pieces of history and significant physical findings to make a diagnosis. He must then carefully support and stabilize the patient under constant monitoring, while making efforts to prevent further absorption of the toxin, enhance excretion, and treat complications as they arise.

Intracranial hemorrhage and amphetamine usage. Review of the effects of amphetamines on the central nervous system.

A 25-year-old man died after an intravenous injection of 100 mg of methedrine. Postmortem studies showed visceral congestion, lung edema, pericardial petechiae, centrolobular necrosis of the liver, and diffuse subarachnoid blood, intracranial vasculitis and cerebritis in the absence of aneurysms, arteriovenous malformations or chronic hypertension. A review of the English-language literature produced 3 other cases of fatal amphetamine-induced intracranial hemorrhage and seven nonfatal cases. Some were the result of overdose, others of hypersensitivity. Angiographic evidence suggests that such

Identification of targetable epitope surfaces from the high resolution structure of the superantigen Staphylococcal enterotoxin L.

Emetic exotoxins secreted by Staphylococcus species (Staphylococcal enterotoxins, SEs) are a major cause of food poisoning cases worldwide and many are additionally classified as superantigens-able to potently activate T cells in an antigen independent manner. Fewer than half of the gene products of the known SE genes have been extensively characterized. The gene for Staphylococcal enterotoxin L (SEL) occurs in both foodborne and clinical isolates but no detailed structural characterization has yet been available. We report here the crystal structure of SEL and confirm its function as a supera

Veterinary Enhanced Recovery After Surgery (Vet-ERAS) Program in Dogs Undergoing Emergency Laparotomy.

This study aimed to assess the efficacy of a veterinary enhanced recovery after surgery (Vet-ERAS) protocol in dogs undergoing emergency laparotomy (EL). The protocol, adapted from human medicine, encompassed a multidisciplinary approach and interventions such as early goal-directed fluid resuscitation, antibiotic prophylaxis, anti-emetic therapy, multimodal analgesia, and early enteral nutrition. A prospective observational study compared outcomes between dogs managed with the Vet-ERAS protocol (n = 59) and historical controls (n = 82). Implementation of the Vet-ERAS protocol resulted in a si

The Food Poisoning Toxins of Bacillus cereus.

Bacillus cereus is a ubiquitous soil bacterium responsible for two types of food-associated gastrointestinal diseases. While the emetic type, a food intoxication, manifests in nausea and vomiting, food infections with enteropathogenic strains cause diarrhea and abdominal pain. Causative toxins are the cyclic dodecadepsipeptide cereulide, and the proteinaceous enterotoxins hemolysin BL (Hbl), nonhemolytic enterotoxin (Nhe) and cytotoxin K (CytK), respectively. This review covers the current knowledge on distribution and genetic organization of the toxin genes, as well as mechanisms of enterotox

Risk of Bacillus cereus in Relation to Rice and Derivatives.

Rice is a very popular food throughout the world and the basis of the diet of the citizens of many countries. It is used as a raw material for the preparation of many complex dishes in which different ingredients are involved. Rice, as a consequence of their cultivation, harvesting, and handling, is often contaminated with spores of Bacillus cereus, a ubiquitous microorganism found mainly in the soil. B. cereus can multiply under temperature conditions as low as 4 °C in foods that contain rice and have been cooked or subjected to treatments that do not produce commercial sterility. B. cereus p

The Bacillus cereus Food Infection as Multifactorial Process.

The ubiquitous soil bacterium Bacillus cereus presents major challenges to food safety. It is responsible for two types of food poisoning, the emetic form due to food intoxication and the diarrheal form emerging from food infections with enteropathogenic strains, also known as toxico-infections, which are the subject of this review. The diarrheal type of food poisoning emerges after production of enterotoxins by viable bacteria in the human intestine. Basically, the manifestation of the disease is, however, the result of a multifactorial process, including B. cereus prevalence and survival in

The broad-spectrum antiemetic effects ETI-385 result from stimulation of 5-HT1A and 5-HT1D receptors.

In the present study, we describe how a nonstoichiometric ratio of the isomers of 8-hydroxy-2-(di-n-propylamino)tetralin (DPAT) produce a broad-spectrum of antiemetic effects in cats and shrews. Determination of the receptor profile of the isomers and testing them separately in cats revealed superior antiemetic effects but severe defensive behavior with the R isomer compared with the S isomer. Differing ratios yielded the best results with the 1:8 (R-S) ratio producing a drug more potent than DPAT and with negligible defensive behavior side effects. Studies with selective 5-HT1D ligands led to

Acute phlegmonous esophagitis as a rare but threatening complication of chemoradiotherapy: report of a case.

Phlegmonous infection involving the digestive tract has been reported to have a poor prognosis. However, the pathogenesis and clinical features of acute phlegmonous esophagitis have remained unclear due to the rarity of the disease. We herein report a case of acute phlegmonous esophagitis that showed a fulminant course during chemoradiotherapy for uterine cancer. The patient developed septic shock 10 h after postprandial nausea and vomiting, and a computed tomographic scan showed diffuse thickening of the esophageal wall. Severe leukopenia that was refractory to the administration of granulocy

[Clinical usefulness of oral aprepitant for alleviation of delayed nausea and vomiting induced by mFOLFOX6--report of a case].

A 50-year-old-woman underwent high anterior resection for sigmoid colon adenocarcinoma. Modified oxaliplatin/l / -LV/ 5-FU(mFOLFOX6)was started as adjuvant treatment due to final-stage III b. Granisetron 3 mg and dexamethasone 8 mg for prophylaxis chemotherapy-induced nausea and vomiting (CINV) were administered intravenously 30 min before oxaliplatin administration. Grade 3 delayed CINV was observed at course 4. CINV could not be controlled by any rescue medications. We adopted a neurokinin-1 receptor antagonist (aprepitant) that alleviated the emetic effects of substance P. The oral aprepita

How should staphylococcal food poisoning outbreaks be characterized?

Staphylococcal food poisoning is one of the most common food-borne diseases and results from the ingestion of staphylococcal enterotoxins (SEs) preformed in food by enterotoxigenic strains of Staphylococcus aureus. To date, more than 20 SEs have been described: SEA to SElV. All SEs have superantigenic activity whereas only a few have been proved to be emetic, representing a potential hazard for consumers. Characterization of staphylococcal food poisoning outbreaks (SFPOs) has considerably progressed compared to 80 years ago, when staphylococci were simply enumerated and only five enterotoxins

[Acupuncture for nausea: how does it work?].

Nausea, the unpleasant feeling that one is going to vomit, results from the stimulation of the Chemoreceptor Trigger Zone (CTZ) and Vomiting Center in the brain. Conventional medical therapy is not always effective, and medications often have severe adverse effects. Acupuncture is a treatment in which thin needles (diameter of 0.20-0.30 mm) are inserted into various points along the skin, according to energy channels (meridians) established thousands of years ago. The anti-emetic effects of acupuncture apparently stem from the resultant increase in hypophyseal secretion of beta-endorphins and

[The role of ondansetron (Qilu) in the prevention of non-cisplatin-induced vomiting--a randomized clinical trial].

The antiemetic effect of ondansetron (supplied by Qilu Pharmaceutical Company) in patients receiving non-cisplatin chemotherapy (containing CTX and/or ADM) was studied in a multiple centre, randomized cross-over trial. The patients who had vomiting in the first turn of chemotherapy entered the trial. The patients received randomly ondansetron or control drugs-metoclopramide or Zofran (Glaxo) in the second turn of chemotherapy. In the third, the patients were cross-over to use the other antiemetic drug. A total of 155 patients were enrolled into the study. The results showed, the effective cont

Amelioration of cytotoxic-induced emesis with high-dose metoclopramide, dexamethasone and lorazepam.

A double-blind randomised controlled trial comparing the antiemetic effects of sublingual lorazepam combined with high-dose, short course metoclopramide (3 mg/kg) infused twice 3 h apart with or without i.v. dexamethasone is reported. Sixty patients receiving a total of 209 cycles of potentially severely emetic cytotoxic chemotherapy were randomised to receive one or other antiemetic regimen. In those receiving platinum-based chemotherapy the addition of dexamethasone was associated with an improvement in freedom from nausea (P less than 0.01) and freedom from vomiting (P less than 0.05). Possible role of brain stem respiratory neurons in mediating vomiting during space motion sickness.

Vomiting is produced primarily by the coordinated action of the respiratory muscles. The object of this study was to determine if brain stem expiratory neurons control abdominal muscle activity during vomiting. We recorded the activity of 27 ventral respiratory group (VRG) expiratory neurons, which are known to be of primary importance for control of abdominal muscle activity during respiration. During fictive vomiting produced by emetic stimulants in decerebrate, paralyzed cats, only one-third of these neurons had the appropriate firing pattern to contribute to abdominal muscle activity durin
