[Femur head necrosis in childhood].

The natural history of avascular necrosis of the femoral head in children and adolescents shows significant differences compared to the course of this disease in adults. Depending on the remaining growth potential, even extensive damage of the femoral head can regenerate, as long as the epiphyseal growth plate is not involved in the process. Otherwise, the deformity will increase with time. Current knowledge on the etiology, natural history, diagnosis and treatment is described on Perthes' disease and other possible causes of avascular necrosis, such as CDH, epiphyseolysis of the femoral head,

Open reduction of the severely slipped upper femoral epiphysis.

The results of open reduction of the severely slipped upper femoral epiphysis are reported for 115 hips with an average follow-up of 12 years 11 months (range 2 to 33 years). In 70 hips with a chronic slip and an open growth plate the incidence of complications was low: two developed avascular necrosis, five chondrolysis, and one had both. There were more complications in the 38 hips with an acute-on-chronic slip: six developed avascular necrosis, one chondrolysis, and three had both. Of the seven hips operated upon with a partially fused plate, only one did well. All these complications were

Radiotherapy changes of the pediatric hip.

Significant radiation-induced abnormalities (aseptic necrosis of the femoral heads, slipped capital femoral epiphysis, radiation-induced sarcoma) were identified in eight of 44 patients aged 16 years or younger at the time of radiotherapy and followed for at least 3 years. The incidence is 18% in the entire group and 25% (8/32) if only patients with radiographs of the hips 3 or more years after therapy are considered. The first evidence of abnormality developed 13 years after irradiation in one patient. The need for long term follow-up of therapeutically irradiated children is stressed.

The particular usefulness of radioisotope methods in some benign bone diseases.

The authors have performed radioisotope examinations in 271 patients with various non-neoplastic bone diseases. According to their opinion, early diagnosis and follow-up of therapeutic results are the main characteristics which allow radioisotopes to play an important and irreplaceable role. They particularly emphasize the usefulness of radioisotope methods in femoral aseptic necrosis and Paget's disease.

Surgeon Recommendations for Physical Activity in Patients With Pediatric Hip Conditions.

BACKGROUND: While physical activity (PA) is important during youth, it is unclear if children and adolescents with developmental dysplasia of the hip (DDH), Legg-Calvé-Perthes disease (LCPD), or slipped capital femoral epiphysis (SCFE) are expected or encouraged to return to PA once they have healed. This study examines the orthopaedic surgeons' role in advising pediatric hip patients on the PA they should engage in, assessing their practice, opinions, and consensus when making recommendations. METHODS: Orthopaedic surgeons were invited from member lists of 4 hip study groups. The survey incl

Opinion survey of members of British Society of Children's Orthopaedic Surgery related to specific case scenarios in slipped capital femoral epiphysis.

The aim of this study was to assess contemporary management of slipped capital femoral epiphysis (SCFE) by surveying members of the British Society of Children's Orthopaedic Surgery (BSCOS). A questionnaire with five case vignettes was used. Two questions examined the timing of surgery for an acute unstable SCFE in a child presenting at 6 and 48 h after start of symptoms. Two further questions explored the preferred method of fixation in mild and severe stable SCFE. The final question examined the management of the contralateral normal hip. Responses were entered into an Excel spreadsheet and

Late correction of neck deformity in healed severe slipped capital femoral epiphysis: short-term clinical outcomes.

PURPOSE: Contemporary methods for the management of neck deformity following a healed severe slipped capital femoral epiphysis (SCFE) include subcapital neck osteotomy. METHODS: 18 patients with chronic severe SCFE in the oblique plane (mean slip angle = 70°) constituted the study group. 6 patients with an open physis underwent modified Dunn capital realignment and 12 patients with a closed physis underwent surgical dislocation with a corrective neck osteotomy. 10 patients already had an in-situ pinning. RESULTS: The mean follow-up was 4.5 years (3-6 years). The mean preoperative modified Ha

Increased acetabular depth may influence physeal stability in slipped capital femoral epiphysis.

BACKGROUND: Multiple mechanical factors affecting the hip have been associated with the development of slipped capital femoral epiphysis (SCFE). Whether acetabular depth plays a role in the development of a SCFE has not been elucidated. QUESTIONS/PURPOSES: (1) What is the prevalence of a deep acetabulum in SCFE? (2) Is the presence of a deep acetabulum associated with physeal instability? (3) Is the presence of a deep acetabulum associated with the occurrence of a contralateral SCFE? METHODS: We retrospectively reviewed 232 patients (156 males) who presented with a unilateral SCFE. Fifty (22

Childhood cancer survivors exposed to total body irradiation are at significant risk for slipped capital femoral epiphysis during recombinant growth hormone therapy.

BACKGROUND: Childhood cancer survivors treated with cranial or total body irradiation (TBI) are at risk for growth hormone deficiency (GHD). Recombinant growth hormone (rhGH) therapy is associated with slipped capital femoral epiphysis (SCFE). We compared the incidence of SCFE after TBI versus cranial irradiation (CI) in childhood cancer survivors treated with rhGH. PROCEDURE: Retrospective cohort study (1980-2010) of 119 survivors treated with rhGH for irradiation-induced GHD (56 TBI; 63 CI). SCFE incidence rates were compared in CI and TBI recipients, and compared with national registry SCF

Valgus slipped capital femoral epiphysis: report of two cases and a comprehensive review of literature.

Valgus slip of the capital femoral epiphysis is a known but extremely rare form of a slipped capital femoral epiphysis. After the first report in 1926 up to the present, approximately 44 cases have been reported. We report two cases of valgus slip of the capital femoral epiphysis with a comprehensive review of the existing literature and the precautions that are needed while planning surgical management of these patients. Awareness of this condition is essential to make a prompt diagnosis and plan the appropriate management.

Urgent reduction, fixation, and arthrotomy for unstable slipped capital femoral epiphysis.

BACKGROUND: The management of unstable slipped capital femoral epiphysis (SCFE) is controversial. A high incidence of avascular necrosis (AVN) has been reported after unstable SCFE. METHODS: Twenty-eight consecutive patients with thirty unstable SCFE underwent urgent reduction and fixation with two 6.5-mm cannulated screws. Positional reduction was performed in 25 cases. Arthrotomy was performed percutaneously in 16 cases and as part of an open capsulotomy in 5 cases. RESULTS: Slip severity was mild in 13 patients, moderate in 9, and severe in 8. At mean duration of follow-up of 5.5 years (r

[Ultrasound diagnosis in trauma: elbow and hand].

Ultrasonography with high-frequency transducers nowadays plays an important role in diagnosing post-traumatic complaints of the elbow and hand tissue in children as well as in adults. We see it as an addition to clinical examination, standard X-ray, and other radiological methods. As a noninvasive procedure we can investigate post-traumatic changes and postoperative pain, detect foreign bodies, and also use it for postoperative metal removal. The possibility of imaging cartilage structures with ultrasound also enables us to diagnose post-traumatic injuries in children's elbows such as supracon

[Results of treating slipped capital femoral epiphysis by pinning in situ].

The primary aim in treatment of chronic SCFE consists of immediate stabilization of the epiphysis to prevent further slipping. For mild degrees of slipping (<30 degrees ), pinning in situ is the treatment of choice. With slips between 30 and 50 degrees, the decision should be based on individual factors (age, functional limitation of the hip joint) whether pinning in situ is sufficient or whether an additional intertrochanteric osteotomy according to Imhäuser should be performed. The choice of implant should ensure a safe and stable connection between the epiphysis and the femoral neck without

Congenital coxa vara: computed tomographic analysis of femoral retroversion and the triangular metaphyseal fragment.

Three patients with congenital coxa vara studied with two- and three-dimensional computed tomographic (2DCT and 3DCT) methods are reported. In all cases, the femoral retroversion was documented and subsequently corrected by proximal femoral osteotomy. In two patients with isolated coxa vara, the physeal-femoral neck angle was decreased as seen in slipped capital femoral epiphysis in adolescents. Our studies suggest that the triangular metaphyseal fragment reflects a Salter-Harris type II separation pattern through the defective femoral neck. The epiphysis and attached triangular fragment slip

Results after preoperative traction and pinning in slipped capital femoral epiphysis: K wires versus cannulated screws.

We have retrospectively reviewed 53 cases (62 hips) with a diagnosis of slipped capital wedge epiphysis. After admission to our Hospital, a skin longitudinal traction was applied for 2 weeks, All patients were treated afterward with pinning in situ without manipulation in the operating room. Group A (31 hips) consisted of patients treated with smooth K wires and group B (31 hips) of patients treated with cannulated screws. We found a high incidence of pin penetration in group A (27 hips), whereas there was just one case in group B. Physical closure was considered when 75% of proximal growth pl

Treatment of the unstable (acute) slipped capital femoral epiphysis.

Slipped capital femoral epiphysis, the most common hip disorder in adolescence, traditionally has been classified according to symptom duration. An acute slip is 1 in which there are symptoms for < 3 weeks; for a chronic slip, there are symptoms for > 3 weeks. An acute-on-chronic slip is characterized by a combination of both with a recent exacerbation of symptoms. This classification system is misleading because it does not consider stability. A stable slipped capital femoral epiphysis has a good prognosis, but an unstable slip has a guarded prognosis. The priorities in treating an unstable (

Hormonal studies in patients with slipped capital femoral epiphysis without evidence of endocrinopathy.

In a prospective study, plasma levels of T3, T4, thyroid stimulating hormone (TSH), cortisol, testosterone, growth hormone, and somatomedin-C were tested in 20 consecutive adolescent patients with slipped capital femoral epiphysis (SCFE) in an effort to identify any occult endocrinopathy. All patients had the clinical diagnosis of SCFE confirmed on radiographs. The patients were examined by the pediatric endocrinology service and were without clinical evidence of an endocrinopathy. Of 140 hormonal determinations only four different patients had four abnormal values. Routine sampling of these h
