Exercise-Induced Irisin Antagonizes TGFBR2 to Attenuate Islet Fibrosis and Improve Glucose Homeostasis in Type 2 Diabetes.

This study examined the effects of exercise on islet fibrosis in mice with type 2 diabetes and investigated the role of irisin in the regulation of islet stellate cell (ISC) activation. Following the 16-week moderate-intensity exercise intervention, db/db mice showed reduced body weight and improved glucose tolerance and insulin sensitivity. This exercise intervention also decreased collagen-I (Col-I), fibronectin (FN), and α-smooth muscle actin (α-SMA); reduced islet fibrosis area (assessed by Masson staining); and lowered the numbers of α-SMA-positive ISCs in the pancreas. Serum irisin was i

Assessment of exercise testing after repair of tetralogy of fallot.

Tetralogy of Fallot (TOF) is the most common form of cyanotic congenital heart disease. The aim of this study was to examine the exercise pe … tly lower exercise performance when compared to those without aortopulmonary shunt. In conclusions, exercise tolerance after total correction of TOF is slightly impaired on short-term followup with more affection among patients with previous aortopulmonary shunts. The present study did not reveal any serious ventricular arrhythmia.

[The influence of a beta-adrenergic receptor blocking drug and a calcium antagonist on heart function in normal subjects (author's transl)].

In 13 healthy volunteers, ergometric examinations in the supine position after intake of 2 x 40 mg/d (-)-1-(tert.-butyl-amino)-3-(o-cyclopentylphenoxy)-2-propanol (penbutolol, Betapressin), a new beta-adrenergic receptor blocking substance, or 3 x 60 mg/d N-(3,3-diphenylpropyl)-alpha-methyl-phenethylamine (prenylamine, Segontin) showed no alteration of the exercise tolerance compared to controls. Also there was a significant decrease of heart rate and systolic pressure and an increase of the maximal heart volume at rest and during exercise after intake of the beta-adrenergic receptor blocking …

Statin therapy depresses total body fat oxidation in the absence of genetic limitations to fat oxidation.

Cholesterol lowering drugs are associated with myopathic side effects in 7% of those on therapy, which is reversible in most, but not all patients. This study tested the hypothesis that total body fat oxidation (TBFO) is reduced by statins in patients with genetic deficiencies in FO, determined by white blood cells (FOwbc) and by molecular analysis of common deficiencies, and would cause intolerance in some patients. Six patients on statin therapy without myopathic side effects (tolerant) and 7 patients who had previously developed statin-induced myopathic symptoms (intolerant) (age = 58 +/- 8

Body composition, maximal fitness, and submaximal exercise function in people with interstitial lung disease.

BACKGROUND: Cardiopulmonary exercise testing (CPET) is feasible, valid, reliable, and clinically useful in interstitial lung disease (ILD). However, maximal CPET values are often presented relative to body mass, whereas fat-free mass (FFM) may better reflect metabolically active muscle during exercise. Moreover, despite the value of maximal parameters, people with ILD do not always exercise maximally and therefore clinically relevant submaximal parameters must be identified. Therefore, this study assessed peak oxygen uptake (VO2peak) relative to FFM, identifying the validity of common scaling

Applying Exercise Capacity and Physical Activity as Single vs Composite Endpoints for Trials of Cardiac Rehabilitation Interventions: Rationale, Use-case, and a Blueprint Method for Sample Size Calculation.

OBJECTIVE: To conceptualize a composite primary endpoint for parallel-group RCTs of exercise-based cardiac rehabilitation (CR) interventions and to explore its application and statistical efficiency. DESIGN: We conducted a statistical exploration of sample size requirements. We combined exercise capacity and physical activity for the composite endpoint (CE), both being directly related to reduced premature mortality in patients with cardiac diseases. Based on smallest detectable and minimal clinically important changes (change in exercise capacity of 15 W and change in physical activity of 10

Examining the Role of Exercise Training in Enhancing Life for Adult Congenital Heart Disease: Systematic Review.

BACKGROUND: Current guidelines advise exercise for most congenital heart disease patients (CHD). However, physical activity remains low in CHD individuals, with limited research on exercise's effects in adults. OBJECTIVES: The aim of this study is to evaluate the safety and efficacy of exercise training on exercise capacity and quality of life in adult congenital heart disease (ACHD) patients. METHODS: We searched PubMed/Medline, Cochrane Library, Web of Science, and Scopus through December 2022 for randomized controlled trials assessing aerobic and resistance training effects on exercise ca

Reliability and Accuracy of a Standardized Shallow Water Running Test to Determine Cardiorespiratory Fitness.

A standardized fitness assessment is critical for the development of an individualized exercise prescription. Although the benefits of aquatic exercise have been well established, there remains the need for a standardized nonswimming protocol to accurately assess cardiorespiratory fitness (CRF) in shallow water. The present investigation was designed to assess (a) the reliability of a standardized shallow water run (SWR) test of CRF and (b) the accuracy of a standardized SWR compared with a land-based treadmill (LTM) test. Twenty-three healthy women (20 ± 3 years), with body mass index (23.5 ±

The "second wind" in McArdle's disease patients during a second bout of constant work rate submaximal exercise.

Patients with McArdle's disease (McA) typically show the "second-wind" phenomenon, a sudden decrease in heart rate (HR) and an improved exercise tolerance occurring after a few minutes of exercise. In the present study, we investigated whether in McA a first bout of exercise determines a second wind during a second bout, separated by the first by a few minutes of recovery. Eight McA (44 ± 4 yr) and a control group of six mitochondrial myopathy patients (51 ± 6 yr) performed two repetitions (CWR1 and CWR2) of 6-min constant work rate exercise (∼50% of peak work rate) separated by 6-min (SHORT) …

Eccentric and concentric cardiac hypertrophy induced by exercise training: microRNAs and molecular determinants.

Among the molecular, biochemical and cellular processes that orchestrate the development of the different phenotypes of cardiac hypertrophy in response to physiological stimuli or pathological insults, the specific contribution of exercise training has recently become appreciated. Physiological cardiac hypertrophy involves complex cardiac remodeling that occurs as an adaptive response to static or dynamic chronic exercise, but the stimuli and molecular mechanisms underlying transduction of the hemodynamic overload into myocardial growth are poorly understood. This review summarizes the physiol

[Functional possibilities and physical capacity of patients with mitral lesions of the heart].

A total of 231 women with mitral valvular disease at the stage of compensation were examined. An analysis of the complex of clinico-instrumental values of the functional state showed that the level of lowered physical working capacity was associated not only with disease type but also with functional reserves of the adaptation to exercise. Insufficiency of compensatory-adaptation reactions and lowered resources were revealed in the patients with a decreased tolerance to physical exercise. Among persons with low adaptation resources were mainly patients with mitral stenosis and its prevalence.

Exercise Intolerance and Response to Training in Patients With Postacute Sequelae of SARS-CoV2 (Long COVID): A Scientific Statement From the American Heart Association.

The postacute sequelae of SARS-CoV-2, also known as Long COVID, may affect 10% to 25% of individuals diagnosed with SARS-CoV-2. More than 100 symptoms have been reported among patients with Long COVID, but almost all patients report severe fatigue, orthostatic intolerance, shortness of breath, and reductions in exercise tolerance. Emerging data suggest that cardiovascular deconditioning plays a major role in the development of this syndrome and that reductions in functional capacity among patients with Long COVID are comparable to reductions seen among individuals with cardiovascular deconditi…

Cardiac rehabilitation is safe and effective also in the elderly, but don't forget about drugs!

In the setting of heart failure (HF) pharmacotherapy demonstrates a quantifiable improvement in exercise tolerance also in HF with preserved ejection fraction (HFpEF). For patients with HFpEF, often older, with higher prevalence of hypertension, diabetes mellitus, atrial fibrillation and other comorbidities, endpoints such as quality of life and functional capacity may be more clinically relevant. However several study show as the use of ACE-I and B-blocker were lesser than expected. Beta-blocker therapy is the keystone of pharmacotherapy of HF patients and exercise training is the essential c…
