Endorectal mucosal advancement flap: the preferred method for complex cryptoglandular fistula-in-ano.

Cryptoglandular fistula-in-ano is a common affliction that usually responds well to conventional surgical procedures such as fistulectomy, fistulotomy, and seton placement. These procedures, however, can be associated with varying degrees of fecal incontinence. Endorectal mucosal advancement flap has been advocated as an alternative procedure that avoids this problem. This study was undertaken to determine the risks and benefits associated with endorectal mucosal advancement flap in the treatment of complex fistula-in-ano. One hundred sixty-four patients underwent 167 endorectal mucosal advanc…

A case of undiagnosed tethered cord syndrome aggravated by transurethral prostate resection.

BACKGROUND: A 68-year-old man presented with a history of significant urinary urge incontinence, pollakiuria, and weak bladder sensation. He also reported mild fecal incontinence and a hypotrophic and slightly weaker left leg. At 63 years of age he had presented to a urologist for treatment of irritative lower urinary tract symptoms and incontinence. A transurethral resection of the prostate had been performed. After the operation, the symptoms had persisted and the incontinence seriously worsened. INVESTIGATIONS: Clinical neurologic examination, videourodynamic examination, neurophysiologic …

Treatment of incontinence in children with bladder exstrophy after rectal urinary diversion: the anal sling procedure.

PURPOSE: An original technique for reinforcement of the anal sphincter using a sling prosthesis with a flexible fixing point that prevents incontinence and rectal prolapse is presented. MATERIALS AND METHODS: Of 43 patients with exstrophy who have undergone urinary rectal diversion between 1988 and 1997 incontinence was observed in 9 boys and 5 girls. From January 1993 to December 1998 these children were treated with an original method, the anal sling procedure. RESULTS: There were no postoperative complications. Investigations revealed no urinary or fecal incontinence after a mean followup…

Robot-assisted Mitrofanoff and Malone antegrade continence enema reconstruction using divided appendix.

Fecal soiling or intractable constipation frequently occurs in association with urinary incontinence in children with varying pathologic conditions. To manage double incontinence or the combination of wetting and severe constipation, a Mitrofanoff conduit and a channel for antegrade continence enema is constructed using the appendix by open surgery. We present a video of the first case of robot-assisted laparoscopy for simultaneous reconstruction of a continent urinary diversion using the Mitrofanoff principle and a Malone antegrade continent enema using the divided appendix.

Preventing faecal incontinence in patients with functional gastrointestinal disorders.

Nurses assist patients with activities of daily living, including the provision of excretion care. Although excretion may be accomplished by some patients independently, others exhibit disorders in this process. Despite the high prevalence of faecal incontinence, the proper nursing care protocol in this context remains unclear. Therefore, this narrative review was conducted to investigate what types of nursing care are needed to prevent or improve faecal incontinence through a review of existing knowledge, challenges and future prospects. [Faecal incontinence in the elderly: causes and management approach].

Faecal incontinence (FI) in the elderly is a common but underdiagnosed condition. It leads to social stigma and impacts patients' quality of life. The aetiology of FI is multifactorial. Assessment, diagnosis and management involve a comprehensive evaluation. Treatment encompasses a multimodal approach, ranging from lifestyle modifications, the use of certain drugs, a physiotherapeutic approach to surgical interventions in selected cases. The important role of health professionals in education, support, and personalised interventions is emphasised.

Hirschsprung's disease. Management.

BACKGROUND: Hirschsprung's disease (HD) is a rare congenital disease that is characterised by the absence of ganglion cells in the myenteric plexus starting in the distal bowel. This results in distal functional obstruction and may lead to complications like enterocolitis. The treatment is surgical and requires the resection of the aganglionic segment, and the pull-through of normal intestine into the anal opening. However, even after successful surgery, patients may continue to have symptoms. AIM: Discuss current surgical techniques and management strategies for patients with postoperative s

Anal sphincter fatigability in assessing anal incontinence: A systematic review.

BACKGROUND: Diagnosing anal incontinence (AI) based on manometry results is challenging due to the variation of the normal values and overlap between patients with and without AI. This study aimed to perform a systematic review on the difference in sphincter fatigability between patients with and without AI. METHODS: MEDLINE, EMBASE, SCOPUS, and Google Scholar were searched. Studies were included if they included adult patients and assessed anal sphincter fatigability between using manometry. The effect size was estimated as the standardized mean difference (SMD) with 95% confidence intervals

Insert devices for faecal incontinence.

BACKGROUND: Faecal incontinence (FI) affects 1-19% of the general population and carries significant physical and psychological morbidity. Treatment strategies vary greatly with respect to morbidity and efficacy and relatively little is known regarding the role of mechanical devices such as anal and vaginal inserts. This is an up-to-date systematic review of the use of these devices in the management of patients with FI. METHODS: A systematic electronic search was performed of the Medline, Pubmed and Embase databases using the key words and/or MeSH 'anal plug', 'anal insert', 'vaginal insert'
