Treatment of postburn deformities of the hand is a real challenge to reconstructive surgeons. A functional reconstruction was achieved with two sensate island flaps on two discrete pedicles in a case with severe postburn deformity involving both thumb and index finger. Surgical treatment was based on amputation of the second ray and reconstruction of the thumb with the flaps derived from the discarded index finger. Two sensate island flaps on two discrete pedicles, "1st dorsal metacarpal artery based flap" from the dorsal aspect of the proximal phalanx and "palmar digital artery based fillet f

We report the case of a 58-year-old man who sustained an open palmar dislocation of the fifth metacarpophalangeal joint of the little finger secondary to a bicycle accident with an uncommon presentation. These open dislocations have only been reported once in the literature. A palmar and dorsal approach had to be used to completely free the joint from incarcerations and allow complete reduction and repair of the damaged anatomical structures. We then discuss this particular type of dislocation, its pathophysiology and management.

We report a 39-year-old man with pachydermodactyly resulting in progressive digital deformation. Pachydermodactyly is a rare superficial benign digital fibromatosis affecting the proximal interphalangeal joints (PIPs). Our patient had flexion deformities and mal-alignment primarily at the PIPs with associated soft tissue swelling. Radiographs revealed nonerosive subluxation. Surgical examination and biopsy with subsequent immunohistopathological study, revealed increased collagen production and proliferation from both fibroblasts and myofibroblasts consistent with pachydermodactyly. This patie

Juvenile rheumatoid arthritis affects approximately 60,000 to 250,000 children in the United States and is the most common connective tissue disease occurring during childhood. This article discusses the signs, symptoms, and general management of the disease, including surgical techniques.

Silicone arthroplasty of the proximal interphalangeal joint is traditionally performed through a dorsal approach, which requires violation of the extensor mechanism and may contribute to postoperative stiffness and extension lag. Volar approaches preserve the extensor apparatus and permit earlier active mobilization; however, previously described techniques differ in flexor sheath entry, volar plate management, and collateral ligament handling, limiting reproducibility. We describe a standardized volar technique that emphasizes controlled neurovascular mobilization; elevation of the A3 pulley

Acute calcific periarthritis (ACP) in the interphalangeal joints of the hand is rare, with less than 100 cases reported. A rare case of ACP in a proximal interphalangeal (PIP) joint of the hand, in a young black woman, after acute trauma, is presented. She experienced severe pain and limited range of motion, and was medicated with an oral corticoid, which was followed by a rapid resolution of the symptoms. At six months, there were no signs of clinical or radiographic recurrence. Recognition of ACP allows for avoiding unnecessary treatments. In this case, treatment with corticoids might have p

The relative motion concept is simply recognition of the normal functional anatomic relationships that allow powerful extrinsic muscles, the extensor digitorum communis (EDC) and flexor digitorum profundus (FDP), to vary forces on individual finger joints and function in response to the relative position of adjacent metacarpophalangeal joints (MCPJs) in the hand, one to another. First identified as a cause of complications after surgery, a better understanding now allows us to harness these forces by way of differential metacarpophalangeal joint (MCPJ) positioning using an orthosis. This can r…

A dorsally displaced epiphyseal fracture of the middle phalanx (Salter-Harris Type I) is described. The epiphyseal fragments were attached to the central slip of the extensor tendon and collateral ligaments. The articular surface of the PIP joint was intact and smooth. The epiphysis was reduced and fixed without cutting the central slip or the collateral ligaments 8 months after injury. This kind of fracture can occur in the PIP and DIP joints, and presents special diagnostic difficulties. Open reduction is evidently necessary to correct the displacement.

Symphalangism was introduced into Hawaii by a Cherokee Indian in the late 1800s. The resultant pedigree, along with two other racially distinct pedigrees, confirms that the condition is an autosomal dominant genetic trait. Associated conditions include carpal and tarsal coalitions, cervical fusions, brachydactylism, symphalangism of the toes, and hearing loss. Surgical intervention in the hands is seldom, if ever, indicated but correction of the associated hearing loss is rewarding.

Closed injuries to the proximal interphalangeal joint are very common. Many anatomic structures are located in this small area; any one or several may be injured. Each causes a painful, swollen joint. A precise diagnosis of which structures are damaged is important for rational treatment. It is essential to distinguish between injuries to the flexor or extensor tendon mechanism, fractures about the proximal phalangeal joint, and ligamentous and volar plate injuries. The examining physician must understand the functional anatomy of this joint, perform a detailed evaluation, obtain the proper ro

The term »the diabetic hand« traditionally denotes complications affecting the hand in individuals with diabetes mellitus, such as restricted finger movement, numbness, and pain. Trigger finger, Dupuytren's disease, carpal tunnel syndrome, ulnar nerve entrapment, and osteoarthritis of the first carpometacarpal joint are all conditions that are more prevalent among individuals with both type 1 and type 2 diabetes. This overview aims to shed light on a somewhat neglected area in diabetes complications, i.e. the diabetic hand, to increase the knowledge among physicians and surgeons as well as nur

The mallet finger is a frequently encountered fingertip injury that leads to extensor lag of the distal phalanx. Classification systems stratify these injuries as ranging from soft-tissue disruption of the extensor mechanism alone to those that have articular involvement and volar subluxation. The management of mallet finger injuries varies based on injury pattern and surgeon preference. These treatment options include splinting regimens, closed reduction and percutaneous pinning, and open reduction and internal fixation. Although the final goal of treatment is to establish a congruent joint,

A 75-year-old woman presented with a four month history of a slowly growing tumour on the distal portion of the third finger of her left hand. The lesion suggested clinically an inflammatory process or an epidermoid carcinoma. The finger was amputated through the proximal interphalangeal joint. Microscopic examination showed an acral-lentiginous melanoma, subungual type (Clark level V). No elective lymph node dissection was done, and no prophylactic chemotherapy was given. The patient remains free of disease eight months after operation. Clinicians should be aware of this rare lesion, which ma

The proximal interphalangeal joint (PIPJ) is critical for proper finger and hand function. Arthritis of this joint can lead to significant pain and functional impairment. The APEX IP® Extremity Medical fusion device (Extremity Medical, Parsippany, New Jersey, USA) is an interlocking intramedullary screw device that provides a reliable method of hand PIPJ arthrodesis with good patient outcomes. We describe an easily reproducible surgical technique guide for using this device. Level of Evidence: Level V (Therapeutic).

From 1966 to 1989, we performed free toe transfer for thumb and finger reconstruction in 300 patients, with 286 survivals and 14 failures, a total survival rate of 95.55 percent. Follow-up for 2 to 23 years showed that movement and sensation of the reconstructed thumbs and fingers were satisfactory in all of the 286 cases. The optimal indications for the operation were defect of the thumb at the plane of the metacarpophalangeal joint and defects of the second to fifth fingers. Besides atraumatic isolation of the second toe and sophisticated microsurgical suturing technique, special attention w
