Gastrointestinal foreign bodies and strictures: radiologic interventions.

Food impaction and foreign body ingestion are significant and sometimes life-threatening medical problems. As described in part 1 of this monograph, a variety of techniques and instruments are available to diagnose and treat these conditions. The radiologist interested in interventional procedures can find ample opportunities to participate in the management of esophageal obstructions by applying radiographic and fluoroscopic techniques to the diagnosis and removal of foreign bodies and food impactions. The recommended radiologic procedures are relatively safe, cost-effective, and efficient me

Migration of solid silicone encircling element through four rectus muscles.

Anterior migration of an encircling element through a rectus muscle is an unusual complication of scleral buckling surgery. We describe two patients with migration of a solid silicone encircling element through the insertions of all four rectus muscles after retinal detachment surgery. Buckle migration was associated with elevated intraocular pressure and glaucomatous optic atrophy. Rectus muscle function was preserved in both patients. Despite transsection of the anterior ciliary blood supply, anterior segment ischemia did not develop.

Foreign body-induced Colovesical fistula: A case report.

BACKGROUND: Colovesical fistula (CVF) is a rare condition characterized by an abnormal connection between the colon and the urinary bladder. While diverticular disease is the most common cause, this report describes a unique case of CVF resulting from an ingested foreign body. CASE PRESENTATION: A 78-year-old woman with a history of COPD and chronic lymphocytic leukemia presented with prolonged urinary symptoms, including dysuria, frequency, hematuria, and a history of pneumaturia and fecaluria. A CT scan with rectal contrast revealed thickening of the sigmoid colon, contrast material extendi

Impaction of swallowed dentures in the sigmoid colon requiring sigmoid colectomy.

INTRODUCTION: Foreign body (FB) ingestion results in perforation in 1% of cases and is associated with significant morbidity and rarely mortality. Clinical presentation is variable and can present a diagnostic challenge. We report our experience and management of a patient with a delayed presentation of a sigmoid colon foreign body as a result of ingestion of a dental plate. PRESENTATION OF CASE: A 67 year old female attended the colorectal outpatient clinic following an incidental finding of a sigmoid mass on computed tomography (CT) abdomen. Further investigation identified a dental plate i

Suicide attempt by self-stabbing of the bladder: a case report.

INTRODUCTION: The presence of foreign bodies in the bladder often falls within questionable practices in psychiatric settings or in iatrogenic instances such as during endoscopy or migration of foreign bodies around the bladder remaining after surgery on organs close to the bladder. Psychiatric disorders have been reported in patients admitted for self-introduction of foreign bodies in the bladder during an act of sexual satisfaction. However, to the best of our knowledge, no similar case in the context of suicide has been reported in the English-language literature. CASE PRESENTATION: A 56-y

From contraception to urolithiasis: Migration of a contraceptive device into the urinary bladder lumen.

Intrauterine devices (IUDs) are widely used and generally safe contraceptives, though rare complications such as migration into adjacent organs may occur. We present a 34-year-old woman with suprapubic pain, dysuria, intermittent hematuria, and fatigue. Ultrasound revealed a bladder foreign body, further confirmed by multislice CT as an IUD. Cystoscopy demonstrated partial bladder wall embedding with stone encrustation. The device was successfully removed transurethrally. This case highlights the importance of considering IUD migration in women presenting with unexplained urinary tract symptom

Early pregnancy likely caused by an intravesical intrauterine device.

A 42-year-old female with remote history of intrauterine device (IUD) placement presented with gross hematuria, urinary urgency, and dyspareunia. Cystoscopy showed an encrusted, free-floating intravesical foreign body consistent with a heavily calcified IUD. It was removed endoscopically using holmium laser cystolitholapaxy. The patient remained symptom free postoperatively. While most intravesical IUDs are thought to be the result of migration after several months, this patient became pregnant within 4 weeks after initial insertion. Therefore this may represent a case either of early intraves

The role of XLIF in spinal revision surgery involving failed interbody implants: a review of technique, outcomes, and indications.

BACKGROUND: This review examines the novel application of the Extreme Lateral Interbody Fusion (XLIF) approach for the removal of foreign bodies in the spine, including subsided cages, failed hardware, and other implants. It aims to evaluate the efficacy, safety, and technical considerations of XLIF in this context and compare it to traditional anterior and posterior surgical approaches. METHODS: A comprehensive literature search in accordance with PRISMA was conducted using Google Scholar, PubMed/MEDLINE, and the Cochrane Library. Search terms included "extreme lateral interbody fusion," "la

Case report - Embolic shotgun pellet to the right internal carotid artery causing hemiplegia.

INTRODUCTION AND IMPORTANCE: Intravascular ballistic embolism is a rare and complex medical condition, posing diagnostic and management challenges. It involves the migration of ballistic materials within the arterial system, often stemming from neck and chest injuries. CASE PRESENTATION: A 13-year-old boy sustained a chest injury from an air rifle, leading to a pellet embolism in the right internal carotid artery, resulting in severe cerebral infarction. Despite intervention attempts, the lodged pellet left the patient with permanent hemiplegia. CLINICAL DISCUSSION: Intravascular ballistic e

Unintended coil migration into the right ventricle during the right ovarian vein coil embolization.

PURPOSE: The aim of this article is to report a rare case of unintended coil migration into the right (Rt) ventricle. MATERIALS AND METHODS: A 36-year-old Brazilian female was referred to us for the evaluation of pelvic pain. Venous duplex ultrasound findings were consistent with incompetent venous systems of right lower extremity (RLE). The diagnosis of grade III pelvic varices was confirmed with ovarian venography, and bilateral ovarian vein coil embolization was performed. The procedure was complicated with unintended migration of a small coil into the Rt ventricle. DISCUSSION: In cases w

Magnetic-assisted detection and extraction of dispersed metallic foreign bodies in the abdominal cavity: A case report.

BACKGROUND: Gastrointestinal foreign bodies represent a significant clinical challenge in emergency and surgical settings. While accidental ingestion predominates in healthy adults, intentional ingestion is frequently observed in pediatric, psychiatric, and incarcerated populations. Metallic sewing needles, characterized by their sharp morphology and high mobility, pose particular risks of visceral injury and complications due to their penetrative potential. Although preoperative imaging facilitates initial localization, subsequent migration of multiple needles complicates therapeutic interven

Rare case of extrahepatic biliary obstruction caused by a duodenal foreign body in a cat.

CASE SUMMARY: A spayed female British Shorthair cat aged 1 year and weighing 2.6 kg presented with a 5-day history of vomiting and anorexia. Physical examination revealed icterus, and serum biochemistry showed markedly elevated bilirubin and liver enzyme levels. Abdominal ultrasonography revealed a dilated common bile duct (4 mm), mild gallbladder wall thickening and a round, hyperechoic foreign body located at the major duodenal papilla. A contrast-enhanced CT scan confirmed the presence of a 1.9 cm doughnut-shaped foreign body in the proximal descending duodenum, causing extramural compressi

Ingestion of an unusually high number of foreign bodies in a mentally ill man.

The vast majority of ingested foreign bodies (FB) can pass through the digestive tract without major incidences. In some cases, they accumulate in large amounts in the stomach. They can also perforate the gastrointestinal wall and migrate to extraluminal sites, remaining quiescent for many years. We report a case of a psychiatric patient with more than 100 ingested FB in the stomach.

[Iatrogenic foreign body].

Three cases of iatrogenic foreign bodies are described. In case 1 a large stone was removed from the urinary bladder of a 80-year-old man 8 years after suprapubic prostatectomy. The concrement contained a fragment of the gauze tampon left during that operation. In case 2 a surgeon trying to stop a profuse hemorrhage lost a surgical needle in paraurethral tissues above the urinary bladder of an elderly man undergoing prostatectomy. Because of the foreign body the patient developed complications: scarry obliteration of the bladder neck, concrement, a vesical fistula, chronic cystitis and chronic

Biliary obstruction secondary to shrapnel.

Foreign bodies of the biliary tree represent infrequent causes of obstructive jaundice. We report a patient who developed biliary obstruction from metal shrapnel, 44 yr after a war injury. From our review of the literature, the syndrome of shrapnel-induced obstructive jaundice may occur many years after the initial injury. In the majority of patients, the missile lodges in the liver parenchyma, and migrates to the common bile duct. Complications from this injury include cholangitis, pancreatitis, and liver abscesses. As demonstrated by this case, computed tomography scan and endoscopic retrogr

An unexpected cause of abdominal pain: a case report of a toothpick in the liver successfully treated with gastroendoscopy.

Foreign body ingestion is a frequent occurrence in emergency departments, with most cases resolving without complications. However, sharp objects like toothpicks carry a higher risk of serious outcomes, including perforation and abscess formation. In this case, we are presenting a 62-year-old male patient who arrived at the emergency department with severe right upper quadrant abdominal pain and fever. Despite the absence of signs of gastrointestinal perforation or peritonitis, imaging revealed a toothpick penetrating both the stomach and liver, with no evidence of abscess formation. The tooth

Intravesical migration of intrauterine device complicated by multifocal sepsis from ruptured tubo-ovarian abscess and acute appendicitis: a comprehensive clinical case study.

BACKGROUND: Intrauterine device (IUD) migration is a rare but serious complication of IUD use, most often resulting from unnoticed uterine perforation. However, intravesical migration complicated by multifocal sepsis from ruptured tubo-ovarian abscess (TOA) and acute appendicitis is an exceedingly rare and complex clinical scenario. CASE PRESENTATION: A 39-year-old multiparous Asian female with history of IUD placement presented with features of sepsis. Laboratory and radiological investigations, and quick sequential organ failure assessment (qSOFA) score were promptly done. The assessment wa
