Full-thickness flap/subepithelial connective tissue grafting with intramarrow penetrations: three case reports of lingual root coverage.

Three case reports are presented that demonstrate the use of full-thickness flap/subepithelial connective tissue grafting for root coverage on the lingual surfaces of the mandibular anterior teeth. This is accomplished using an envelope full-thickness flap technique with intramarrow penetrations at the recipient site. Miller Class I, II, and III gingival recession defects and gingival perforation defects were treated. Complete root coverage was achieved in two Miller Class I gingival recession defects, in one Miller Class II gingival recession defect, and in two gingival perforation defects in…

Analysis of gingival margin esthetic clinical conditions by dental students.

It has been suggested that one of the main factors contributing to the perception of esthetics is the position of the gingival tissues in th … the students. The cases included unchanged periodontium, gingival smile, uneven gingiva, localized gingival recession and generalized gingival recession. Esthetic perception was evaluated by a Visual Analogue Scale (VAS). The different situations were compared by One-way ANOVA, with Tukey test as post-hoc. The results showed statistically significant differences be …

Acellular dermal matrix grafts for root coverage procedures: review of products and introduction of a new technique.

Gingival recession remains a significant problem in dental esthetics as well as periodontal health. Adequate zones of keratinized, attached tissue are vital for long-term periodontal health and maintenance. The correction of gingival recession is an important principle in cosmetic dental procedures that requires a harmonious gingival complex. Correction of deficient gingival tissues by either autogenous or allogenic tissue grafts has been well documented in the literature. This article will delineate differences among the various acellular dermal grafts as well as introduce a new technique tha…

Mucogingival orthodontic and periodontal problems.

The absence of keratinized gingiva alone is not an indication for a periodontal surgical procedure. However, if recession increases during orthodontic treatment, then a gingival graft may be indicated. Orthodontic therapy and removal of a mandibular incisor where excessive gingival recession is present may be the indicated treatment.

A comparative clinical study of solid and granular porous hydroxylapatite implants in human periodontal osseous defects.

Solid and granular porous hydroxylapatite implants were compared in the surgical treatment of angular interproximal periodontal defects in 10 subjects. After completion of initial therapy, presurgical measurements of pocket depth, attachment level, gingival recession, gingival fluid and tooth mobility were recorded. Six months after the surgery the measurements were repeated. The use of both forms of porous hydroxylapatite resulted in reduction in pocket depth, and probeable attachment level gains as well as gingival recession and reduction of gingival fluid and tooth mobility. These changes w…

The relationship of the position of crown margins to gingival health.

A survey was conducted of 423 crown margins that were divided into 355 subgingival margins and 68 supragingival margins. The plaque index, gingival bleeding, and recession surrounding the artificial crown were recorded and compared to the unprepared contralateral tooth. Gingival tissues tended to bleed 2.42 times more frequently with subgingival margins and have a 2.65 times higher chance of gingival recession. Crowns with supragingival margins did not differ significantly compared with the contralateral tooth, but crowns with subgingival margins had greater bleeding and recession compared wit…
