Ultrasonographic evidence of joint thickening reversibility in acromegalic patients treated with lanreotide for 12 months.

BACKGROUND: A major cause of morbidity and functional disability in acromegaly is represented by axial and peripheral arthropathy. OBJECTIVE: The effect of a 12-month treatment with lanreotide (LAN) on arthropathy in 12 untreated acromegalic patients has been evaluated. Twelve healthy subjects served as controls. STUDY DESIGN: Open prospective. STUDY PROTOCOL: Articular cartilage thickness of shoulder, wrist and knee, as well as the size of the heel tendons, was measured by ultrasonic (USG) examination before, monthly for the first 3 months and quarterly thereafter, during treatment with 60…

DMPS can reverse the features of severe mercury vapor-induced neurological damage.

CASE REPORT: A 36-year-old jewelry producer presented with tremor, slurred speech, lethargy, headache, and incoordination. On examination he had a coarse tremor of the outstretched hands and protruded tongue, slurred speech, "Hatter's shakes" handwriting, impaired heel-toe walking and heel-shin coordination, mild dysdiadochokinesis, and constricted visual fields to confrontation. The patient received four 5-day courses of oral 2,3-dimercapto-1-propanesulfonate (30 mg/kg/day), which was associated with substantial objective clinical improvement and the excretion of 99,406 microg mercury. [Surgical treatment of persistent congenital clubfoot in the young child].

From 1979-1985 seventeen children with twenty-six club-feet were treated. Thirty-four operations have been carried out, divided in three surgical procedures. The heel cord was lengthened in eight feet, a posterior release was done in thirteen feet and a posteromedial release according to Turco was done thirteen times as well. Lengthening of the heel cord seemed to be an insufficient procedure, because six out of eight feet showed a recurrent deformity. The posterior release has given in half of the cases a good result. After posteromedial release we saw in 70% a good or excellent result, when …

A comparative analysis of analgesic efficacy of ultrasound and shock wave therapy in the treatment of patients with inflammation of the attachment of the plantar fascia in the course of calcaneal spurs.

INTRODUCTION: Troublesome heel spur is a nuisance condition that affects people of all ages. Treatment of patients with heel spur is a difficult and lengthy process requiring patience from both the patient and the therapist. Sometimes, the only and ultimate method of treatment is surgery, although spurs tend to recur. The aim of the study is a comparative analysis of the analgesic efficacy of ultrasound and shock wave therapy in patients with heel spur. The cause of pain in the course of calcaneal spur is inflammation of the attachment of the plantar fascia, which plays an important role in th…

Cutaneous hypersensitivity following peripheral tissue damage in newborn infants and its reversal with topical anaesthesia.

The flexion reflex threshold has been used as a measure of sensation in a group of premature infants born at 27-32 weeks postmenstrual age. The threshold in an area of local tissue damage created by routine heel lances was half the threshold on the intact heel on the other side. This indicated a hypersensitivity to tissue damage analogous to tenderness or hyperalgesia reported in adults. In a double-blind study, treatment of the damaged area with the topical anaesthetic cream, EMLA, was found to reverse this hypersensitivity or in other words increase the flexion reflex threshold. Treatment wi…

Gait analysis. Alterations in support phase forces using supportive devices.

This study documents changes in momentary distribution of forces under the foot, comparing barefoot gait to that with heel cups, medial arch supports, and low-dye taping. Cholesterol crystal force plate analysis and a computerized Kistler force platform were used in the study. Low-dye taping or a heel cup significantly diminished the duration of forces under the midfoot, medializing the instant center of forces. A medial arch support shifted the instant center of forces laterally, though it did not diminish the duration of forces under the arch. The relationship between these alterations of fo…

Neurilemmoma of the first branch of the lateral plantar nerve causing tarsal tunnel syndrome.

In this article, the authors report a case of tarsal tunnel syndrome caused by neurilemmoma of the first branch of the lateral plantar nerve, with symptom resolved after excision. A 42-year-old man presented with left medial heel pain radiating to the lateral sole for 6 months. On examination, there was positive Tinel sign over the medial heel with pain radiating to the lateral sole. Ultrasonography and magnetic resonance imaging confirmed the presence of a 1-cm neurogenic tumor inside the tarsal tunnel. Intraoperatively, a 1-cm neurilemmoma was found at the first branch of the lateral plantar…

Features resisting primary treatment of congenital club foot.

The aims of this study were: (1) to see whether the number of relapses of hindfoot deformity in congenital club foot in the first year of life could be reduced by operative treatment, and (2) to try by surgery to pinpoint features of congenital club feet that make them resistant to treatment. As compared to treatment of 95 feet with manipulation and plaster cast only or with heel cord tenotomy added, early tenotomy of both the heel cord and the tibialis posterior tendon in 23 feet markedly reduced the number of relapses of hindfoot deformity, and the need for additional treatment. The results …

Anatomical Features of the Tarsal Sinus in Patients with Pes Planus: Implications for Clinical Management.

BACKGROUND In the treatment of pes planus, if the implant does not match the anatomical structures of the sinus tarsi, synovitis can develop, causing pain symptoms. In the interest of making clinical recommendations for extra-osseous talotarsal stabilization, the goal of the present study was to characterize the anatomical characteristics of the sinus tarsi in patients with pes planus using magnetic resonance imaging (MRI) scans. MATERIAL AND METHODS This was a retrospective study involving 56 pes planus patients and 56 healthy volunteers from January 2014 to May 2022. The sinus tarsi was meas

An anatomical approach to the tarsal tunnel syndrome: what can ankle's medial side anatomy reveal to us?

BACKGROUND: The heel is a complex anatomical region and is very often the source of pain complaints. The medial heel contains a number of structures, capable of compressing the main nerves of the region and knowing its anatomical topography is mandatory. The purpose of this work is to evaluate if tibial nerve (TN) and its main branches relate to the main anatomical landmarks of the ankle's medial side and if so, do they have a regular path after emerging from TN. METHODS: The distal part of the legs, ankles and feet of 12 Thiel embalmed cadavers were dissected. The pattern of the branches of …

An in vivo model for overloading-induced soft tissue injury.

This proof-of-concept study demonstrates that repetitive loading to the pain threshold can safely recreate overloading-induced soft tissue d … ing injury in the foot to improve the management of conditions such as diabetic foot ulceration and heel pain syndrome. Moreover, the link between overloading and tissue stiffening, which was demonstrated here for the first time for the plantar soft tissue, opens the way for an assessment of overloading thresholds that is not based on the subjective me …

Pain on the Plantar Surface of the Foot.

BACKGROUND: Plantar fasciitis (PF) is characterized by pain on weight-bearing in the medial plantar area of the heel, metatarsalgia (MTG) by pain on the plantar surface of the forefoot radiating into the toes. Reliable figures on lifetime prevalence in Germany are lacking. METHODS: This review is based on pertinent publications retrieved from a selective search in PubMed, on guidelines from Germany and abroad, and on the authors' clinical experience. RESULTS: Plantar fasciitis is generally diagnosed from the history and physical examination, without any ancillary studies. In 90-95% of cases,…

The role of intrinsic musculature in the formation of inferior calcaneal exostoses.

Review of the anatomy contributing to calcaneal spur formation is provided. Freshly amputated limbs, used because they are not subject to the effects of the preservatives that effect cadaveric limbs, are the basis for the study the authors report. An alternative theory to the cause of heel spurs is presented based on that study.
