BACKGROUND AND AIM: Budd-Chiari syndrome (BCS) is a rare condition associated with hepatic venous outflow obstruction classically treated with portosystemic shunts or liver transplantation. Recent reports indicate promising results with the use of transjugular intrahepatic portosystemic shunts (TIPS) in the treatment of these patients.
PATIENTS AND METHODS: We reviewed a 10-year single-institution experience with TIPS in patients diagnosed with BCS.
RESULTS: Eleven patients with BCS underwent TIPS procedures, 3 of whom carried a diagnosis of paroxysmal nocturnal hemoglobinuria, a relative co…

The similarity of this defect to that previously described in a patient with paroxysmal nocturnal hemoglobinuria suggests that complement-mediated hemolysis could play a role in the anemia of SCD.

Serum erythropoietin (EP) concentration was measured by the recombinant EP-based radioimmunoassay and was examined to standardize the hemoglobin (Hb) related level of 144 normal control and 56 patients with iron deficiency anemia and hemolytic anemia excluding paroxysmal nocturnal hemoglobinuria. The standardization was achieved by logarithmic regression of the EP titier on Hb either by the two-phase linear form or by the third degree sigmoid form at a 95% confidence limit for each regression. The third degree regression was found to be preferable from the view point of both statistics and the…

OBJECTIVES: We examined the frequencies and sizes of glycosylphosphatidylinositol (GPI)-deficient cells as per the International Clinical Cytometry Society/European Society for Clinical Cell Analysis (ICCS/ESCCA) consensus guidelines for the high-sensitivity detection of GPI-deficient cells.
METHODS: In 2018, the ICCS/ESCCA guidelines for the high-sensitivity detection of GPI-deficient cells were published. We evaluated frequencies and sizes of GPI-deficient red blood cells (RBCs), neutrophils, and monocytes as determined using the ICCS/ESCCA guidelines and Clinical and Laboratory Standards I

OBJECTIVES: Paroxysmal nocturnal hemoglobinuria (PNH) is an acquired clonal stem cell disorder characterized by partial or absolute deficiency of glycophosphatidyl-inositol (GPI) anchor-linked surface proteins on blood cells. A lack of precise diagnostic standards for flow cytometry has hampered useful comparisons of data between laboratories. We report data from the first study evaluating the reproducibility of high-sensitivity flow cytometry for PNH in Russia.
METHODS: PNH clone sizes were determined at diagnosis in PNH patients at a central laboratory and compared with follow-up measuremen…

The treatment of persistent tympanic membrane perforation is tympanoplasty. Presence of perforation in the anterior annulus of the patients undergoing tympanoplasty surgery is a commonly encountered problem in the postoperative period. In this study, we investigated the effects of supporting anterior tympanomeatal angle with a secondary layer of temporal muscle fascia graft in the patients undergoing type 1 tympanoplasty on postoperative hearing outcomes and early graft success. Twenty-seven patients from our clinic who underwent surgery due to chronic otitis media (COM) and whose anterior tym

Deficiencies in glycosylphosphatidyl-inositol (GPI)-linked proteins on erythrocytes and leucocytes in patients with paroxysmal nocturnal haemoglobinuria (PNH) are well known; however, expression on platelets in these patients is less well documented. We have studied CD55 and CD59 on the platelets of PNH and aplastic anaemia (AA) patients using flow cytometry. In all cases of PNH, CD55 and CD59 negative populations of platelets were detected with single or bimodal distribution and these results showed close correlation with the CD55 and CD59 patterns of neutrophils. Previous published studies h

Paroxysmal nocturnal haemoglobinuria (PNH), aplastic anaemia (AA) and myelodysplastic syndrome (MDS) are haemopoietic stem cell disorders. These disorders have some features in common, and a percentage of cases progress to acute leukaemia. We speculated that changes in gene stability are involved in the pathogenesis of these haemopoietic stem cell disorders. Therefore we investigated in vivo mutation frequencies in these disorders by erythrocyte glycophorin A (GPA) mutation assay. The assay enumerates NO or NN variant cells in 106 erythrocytes of the MN type using a flowcytometric technique. P

Hepatic vein thrombus is a common and often fatal complication of paroxysmal nocturnal haemoglobinuria. Rapid diagnosis with ultrasound, computed tomography or venography is required to improve the prognosis. Although heparin is often the initial therapy of choice, thrombolytic agents may be more beneficial in the early management.

Paroxysmal nocturnal haemoglobinuria (PNH) was diagnosed in 11 patients attending the haematology clinic of a private community hospital in Hyderabad, India over a 3-year period. Compared to PNH occurring in Caucasians, the patients were younger, and showed a marked male preponderance; severe thrombocytopenia, haemorrhagic complications from thrombocytopenia and infectious complications were not seen. Thrombotic complications were common. Fever during periods of haemoglobinuria was a notable feature in 4 patients. None of the patients had the aplastic anaemia PNH syndrome.

Examples of patients are used as a basis for discussing the treatment of severe aplastic anaemia with clonal chromosomal aberrations in haematopoietic cells, the aplastic form of paroxysmal nocturnal haemoglobinuria and myelodysplasia, with allogeneic bone marrow transplantation. Transplantation with marrow from an HLA-identical sibling donor has curative potential in selected patients with these disorders. The procedure should preferably be carried out before the patients have received massive treatment for the complications of marrow failure. The use of matched unrelated bone marrow donors i

Male Wistar rats were used to study changes in ultrastructure of proximal renal tubular cells after an haemoglobin (Hb) load with or without temporary renal ischaemia. An i.v. injection of stroma-free Hb was given either 30 min before, immediately after, or 2 h after ischaemia. Appropriate control groups were used for comparison. Electron microscopic examination was performed on kidneys removed 48 h after treatment. Hb transport and degradation was markedly delayed by renal ischaemia. Hetero-ambi- and autolysosomes were found after Hb load, and the number of lysosomes was higher in animals wit

Five children with 'paroxysmal cold haemoglobinuria' are described. In all of them an upper respiratory tract infection preceded the episode of acute intravascular haemolysis and potentially pathogenic organisms were recovered from four. The haematological features are presented and the importance of technique in the detection of the pathognomonic antibody is stressed and discussed in detail. Although the clinical manifestations are usually self-limiting and of short duration, it may be necessary, because of the rapidity and severity of anaemia, to provide interim support with appropriate bloo

Several characteristics of normal renal function increase the risk to the kidney of damage by environmental toxins. Due to the magnitude of renal blood flow the total amount of noxious substance delivered may be disproportionately high. Furthermore, the capacity to concentrate substances within the kidney by processes of filtration, reabsorption and secretion has the potential to increase the toxicity of agents which would otherwise not lead to tissue injury. Unfortunately, there are few tests of renal function which are able to detect early functional abnormalities and which, at the same time
