[Concept, etiology and epidemiology of primary hyperparathyroidism].

Primary hyperparathyroidism (PHPT) is one of the most frequent endocrinological disorders. In PHPT, there is abnormal regulation of parathyroid hormone (PTH) by calcium, which translates into inappropriately high PTH secretion for the level of calcemia. Most patients with PHPT have increased serum PTH levels, with increases in serum calcium, especially ionic calcium. The incidence of PHPT rises with age, the mean age at diagnosis being 55 years. This disorder affects mainly women with a female-to-male ratio of approximately 3:1. Most (80-85%) of cases are produced by chief cell parathyroid ade…

[Hyperparathyroidism in patients on long-term hemodialysis].

The author presents case reports of parathyroid adenomas in patients receiving long-term hemodialysis and emphasizes the necessity of dynamic monitoring phosphorus-calcium and bone metabolism starting from the pre-dialysis phase of chronic renal insufficiency. Preventive treatment and therapy of secondary hyperparathyroidism should be based on calcium-containing and phosphate-binding preparations and alfacalcidol at doses adjusted to individual requirements of the patients.

Radioguided parathyroidectomy for hyperparathyroidism in the reoperative neck.

BACKGROUND: The purpose of this study was to determine if radioguided parathyroidectomy (RGP) is effective for hyperparathyroidism (HPT) in the reoperative neck. METHODS: We retrospectively reviewed all patients with HPT and a history of neck surgery who underwent RGP over a 7-year period. Data are reported as mean +/- SEM. RESULTS: We identified 110 patients with primary (n = 94), secondary (n = 7), or tertiary (n = 9) HPT who underwent 138 previous neck operations. The average hospital stay was 0.6 +/- 0.1 days. The in and ex vivo counts obtained with the gamma probe were 310 +/- 26 and 13…

[Patient with relapsed hyperparathyroidism and neck swelling].

72-year-old woman with a history of primary hyperparathyroidism, for which she underwent surgery years previously, went to see her general practitioner because of a swelling in her neck that had been present for a few months and was growing in size. Other than this she had no symptoms. During the physical examination a solid elastic, non-fixed swelling with a diameter of about 3 cm was palpable on the right of the neck, medially to the sternocleidomastoid muscle. The swelling did not move when she swallowed. Laboratory tests and an MRI scan were suggestive of parathyroid carcinoma. An examinat…

Current status and treatment of primary hyperparathyroidism.

Primary hyperparathyroidism (HPT) is diagnosed in approximately 100,000 patients in the US each year, with a 2-3:1 female-to-male distribution. In most cases, occurrence is sporadic rather than familial, and 80% to 85% of cases of sporadic primary HPT are caused by a solitary parathyroid adenoma. The diagnosis is made by hypercalcemia with an inappropriately elevated parathyroid hormone (PTH) level and a 24-hour urine calcium excretion level that is normal or high. Truly asymptomatic primary HPT is rare, as most patients have symptoms or metabolic complications when carefully evaluated by stan…

Parathyroidectomy for renal hyperparathyroidism in children and adolescents.

BACKGROUND: Renal hyperparathyroidism (rHPT) almost inevitably develops in pediatric patients with end-stage chronic kidney disease (CKD) and may require parathyroidectomy (PTX) despite intensified conservative therapy. Long-term duration of uncontrolled rHPT may result in disabling osteodystrophy and vascular calcifications. Only a few reports on children undergoing PTX for rHPT are available and mainly consist of case reports with short follow-up periods. To study this entity, we analyzed the course of 23 pediatic patients who underwent PTX for rHPT. METHODS: Twenty-three patients with a me…

Biomarkers of hypercoagulability and inflammation in primary hyperparathyroidism.

BACKGROUND: Severe primary hyperparathyroidism (PHP) has been associated with increased cardiovascular morbidity. Hypertension, dyslipidemia and impaired glucose metabolism were demonstrated in severe PHP, with improvement after surgery in these variables. Such an association in mild PHP is not known. The study was conducted to determine biomarkers of hypercoagulability and inflammation for cardiovascular disease in patients with primary hyperparathyroidism. MATERIAL/METHODS: Patients (n=35) without CVD were evaluated. Patients were subdivided into two groups, severe (n=15) and mild (n=20) hy…

Hyperparathyroidism in a patient with autoimmune polyglandular syndrome.

Autoimmune hypercalcemia has been reported in only a few cases, and never in the context of autoimmune polyglandular syndrome. A patient with type 1, insulin-dependent diabetes mellitus, Graves' disease, and antiparietal cell antibodies presented with persistent hypercalcemia with inappropriate PTH secretion. Other causes of hypercalcemia were excluded. In this context of two associated organ-specific autoimmune diseases we searched for autoantibodies directed to parathyroid tissue and to calcium-sensing receptor. Anti-parathyroid autoantibodies were detected by indirect immunofluorescence on

Parathyroidectomy for secondary hyperparathyroidism in the era of calcimimetics.

Secondary hyperparathyroidism (SHPT) is one of the major complications experienced by patients with renal failure. Cinacalcet hydrochloride, a calcimimetic, is a new modality for the treatment of SHPT and is able to suppress a high parathyroid hormone level remarkably well. However, for patients with uncontrollable SHPT while on cinacalcet, those with severe SHPT symptoms and those with difficulty being treated with cinacalcet because of side-effects, parathyroidectomy (PTx) may be indicated as usual. PTx can induce a remarkable improvement in SHPT: postoperative serum phosphorus and calcium l…

The diagnosis and management of asymptomatic primary hyperparathyroidism.

Classical primary hyperparathyroidism--a disease of 'bones, stones, and psychic groans'--is a rarity in the US, although it is still seen in some parts of the world. Today, most patients with primary hyperparathyroidism are asymptomatic. This change in clinical profile is not merely because of improved detection techniques, and patients with primary hyperparathyroidism who lack symptoms, if left untreated, rarely develop the 'classical' features that were common previously. Indeed, currently available data suggest that there is, today, a disorder with different, yet characteristic, effects on …

Surgical approach to the patient with familial hyperparathyroidism.

Familial hyperparathyroidism encompasses the diagnoses of multiple endocrine neoplasia (MEN) type 1, MEN type 2A, and familial isolated primary hyperparathyroidism. All patients should undergo bilateral neck exploration and identification of all four or more parathyroid glands to evaluate for gross abnormalities. MEN-1 patients should have subtotal parathyroidectomy and cervical thymectomy because this operation achieves an appropriate balance between optimizing the potential for cure yet minimizing the risk of permanent hypocalcemia. However, MEN-2A patients may best be treated by selective r…

Coeliac disease and primary hyperparathyroidism: an association?

Primary hyperparathyroidism may present with non-specific symptoms, and this may be one reason why patients with coeliac disease fail to improve despite compliance with a gluten-free diet. Seven case reports of primary hyperparathyroidism due to sporadic adenoma occurring in a series of 310 patients with coeliac disease are presented, highlighting the importance of looking for this condition in this population group. A prevalence of primary hyperparathyroidism of 2.3% in this series suggests a significant association between hyperparathyroidism and coeliac disease; most studies have indicated …

Cellular and molecular mechanisms of secondary hyperparathyroidism.

The objective of this review is to share important basic research insights into the mechanisms of secondary hyperparathyroidism. An understanding of these mechanisms is essential to more effectively treating the disease. Central underlying abnormalities are increased parathyroid hormone (PTH) gene expression and secretion, and parathyroid cell proliferation. Significant progress has been made in understanding these abnormalities at the cellular and molecular level, particularly their regulation. Studies point to a prominent role of calcium, phosphate and vitamin D as regulators of PTH and para…

The role of cinacalcet in treating secondary hyperparathyroidism.

Compelling evidence exists to use cinacalcet as a primary therapy for the treatment of secondary HPT in patients requiring dialysis. The benefits of directly lowering PTH in combination with improving calcium-phosphorus homeostasis provide a means to better achieve the K/DOQI bone guidelines. Cinacalcet is usually well tolerated and safe to use, making it possible to treat a wide range of patients with varying levels of disease severity. The ideal course of action for this novel therapy is for nephrology nurses and renal dietitians to use a team approach in the management of secondary HPT.
