[The hormonal regulation of thymus activity in threatened abortion].

It is stated that in women with the abortion threat at any term of gestation (I, II, III trimesters) the endocrine function of the thymus essentially increases. The role of hormones in the thymic activity disorder is studied. It is shown that insufficiency of the functional activity of the trophoblast ranks first in the mechanism of hormonal regulation of thymic endocrine function in nonbearing of pregnancy. The other significant reason of an essential increase in the titer of thymic serum factor (TFS) is a disorder in interconnections inside the hypophyseal-adrenal system. Determination of TF

[The effect of adaptation to the periodic action of hypoxia on the indices of the immunity system and on the course of allergic diseases].

The authors studied the effect of adaptation to periodical exposure to hypoxia on the organism's immune status and the values of neurohumoral regulation in children with bronchial asthma and adults suffering from allergic dermatoses and autoimmune thyroiditis. It was found that adaptation facilitated normalization of humoral values of immunity in allergic and autoimmune disorders (the content of serum immunoglobulins increased while the level of circulating immune complexes reduced) and was attended by a stable therapeutic effect. The revealed changes of the immune values occurred in increase

[The functional state of the adrenal cortex in patients with gonadal dysgenesis and a female phenotype].

Excretion of 17-KS and 17-OCS was studied in the urine of 86 patients with various cytogenetic variants of gonad dysgenesis of a female phenotype under basic conditions and after ACTH stimulation; in 10 patients endogenous hypophyseal ACTH reserve after block of the adrenal cortex by metopyrone was investigated as well. A conclusion was drawn on the presence in these patients of differnet types of deviations of the adrenal gland function. In some of the cases there is a primary "global" or dissociated injury of the adrenal cortex, apparently directly associated with gonosome anomaly; in other

Endoscopic Endonasal Approach for Resection of Suprasellar Hemangioblastoma: Selective Pituitary Sacrifice and Use of Indocyanine Dye: 2-Dimensional Operative Video.

Suprasellar hemangioblastomas are rare lesions occurring sporadically or more commonly with von Hippel Lindau disease (VHLD).1,2 A 27-yr-old female presented with amenorrhea, mildly raised prolactin levels and otherwise normal pituitary function, homonymous hemianopia, and mild cognitive decline. Imaging revealed a large suprasellar lesion arising from the infundibulum, causing compression of optic chiasm with edema in the visual pathways. Lesion appeared hypervascular with prominent feeding vessels from the internal carotid arteries bilaterally. To access these vessels derived from the superi

Some aspects of the hypothalamic and pituitary development, metamorphosis, and reproductive behavior as studied in amphibians.

In this review article, information about the development of the hypothalamo-hypophyseal axis, endocrine control of metamorphosis, and hormonal and pheromonal involvements in reproductive behavior in some amphibian species is assembled from the works conducted mainly by our research group. The hypothalamic and pituitary development was studied using Bufo embryos and larvae. The primordium of the epithelial hypophysis originates at the anterior neural ridge and migrates underneath the brain to form a Rathke's pouch-like structure. The hypothalamo-hypophyseal axis develops under the influence of

Archosauriform endocranial morphology and osteological evidence for semiaquatic sensory adaptations in phytosaurs.

The examination of endocranial data of archosauriforms has led to advances on the evolution of body size, nerve pathways, and sensory abilities. However, much of that research has focused on bird-line archosaurs, resulting in a skewed view of Archosauria. Phytosauria, a hypothesized sister taxon to or early-branching member of Archosauria, provides a potential outgroup condition. Most previous phytosaur endocranial studies were executed without the use of modern technology and focused on derived members of Phytosauria. We present a comparative CT examination of the internal cranial anatomy of

A case of sudden death from primary intracranial germinoma complicated by microvascular disease of the heart.

A coroner's autopsy conducted on an 11.5-year-old girl, with a history of hypopituitarism, manifesting as growth hormone deficiency associated with short stature, demonstrated the presence of an undiagnosed hypothalamic intracranial germinoma, showing hypophyseal and mesencephalic invasion, together with histological evidence of microvascular (small coronary artery) disease of the heart. The substantive, or primary, cause of her sudden death was clearly the intracranial germinoma, which was acutely haemorrhagic and necrotic. However, it is postulated that there might be an, as yet, undescribed

[Obesity and hypersomnia].

Seventy one patients with hypersomniac paroxysms--Pickwickian and Kleine-Levin syndromes, were studied, the incidence of combination of hypersomnia and obesity--followed up. They are supposed to have common pathogenetic mechanisms, giving weight to lesions of hypothalamo-reticulo-limbic structures in viral infections, craniocerebral traumas, intoxications and tumours. The pathological processes advanced in the reticulo-hypothalamo-hypophyseal region lead to hormonal regulatory disorders and disturbances in adaptation and compensatory mechanisms of metabolism, of lipid metabolism including, of

[Effect of acute parlodel load on the prolactin and STH content in patients with various neuroendocrine diseases].

The results are presented of the treatment of 31 females with different hypothalamic-hypophyseal abnormalities: persisting lactorrhea-amenorrhea syndrome, acromegaly, hypothalamic syndrome, lactorrhea syndrome without amenorrhea. The effect of parlodel acute load on STH and the blood serum prolactin content was studied. It was concluded, that parlodel acute load produces a marked decrease in the prolactin level 2 hours after the drug injection both in the patients with an originally elevated prolactin level, and in the subjects, having the prolactin level within normal. Significant changes in

The Chiropteran Brain Database: Volumetric Survey of the Hypophysis in 165 Species.

For nearly two decades, a database of brain structures from a large sample (272 species) of chiropterans has been widely accessible and used for socioecological analyses of mammals. However, this database remains incomplete since the hypophysis has not been measured. Since this glandular/neural structure has reproductive significance to chiropterans as for other mammals, this investigation was carried out using serial coronal sections of bat brains comprising the Heinz Stephan collection, Düsseldorf, Germany. Complete serially sectioned brains were examined in 313 individuals (165 species, 15

Endocrine-Related Adverse Conditions in Pediatric Patients Treated with Immune Checkpoint Inhibitors: A Position Statement from the Clinical Practice Committee of the European Society for Pediatric Endocrinology.

Immune checkpoint inhibitors (ICIs) are new cancer therapies that function either by blocking the interaction between programmed cell death 1 (PD-1) and programmed cell death ligand 1 (PD-L1) or by inhibiting cytotoxic T-lymphocyte antigen 4 (CTLA-4). The utilization of ICIs has improved outcomes for advanced, relapsed, or refractory malignancies in adults and children. Concerns have been raised due to immune-related adverse events (irAEs). The most frequently observed endocrine irAEs in children and adolescents include adrenal insufficiency, diabetes, thyroid dysfunction, and hypophysitis. Th…

Hypoparathyroidism: an uncommon adverse effect of treatment with durvalumab.

SUMMARY: Immune checkpoint inhibitors (ICIs) are monoclonal antibodies approved for the treatment of numerous cancer types. Toxicities induced by ICIs may affect any organ system and manifest as endocrinopathy. The main side effects related to treatment are immune-related adverse events (irAEs), especially thyroid dysfunction and hypophysitis. Rare endocrine irAEs are diabetes insipidus, hypoparathyroidism, thyrotoxic crisis and hypogonadism. We report a case of hypoparathyroidism induced by ICI treatment with durvalumab, which has not previously been described. LEARNING POINTS: Treatment wit…

A case of severe rhabdomyolysis associated with secondary adrenal insufficiency and autoimmune hepatitis.

Rhabdomyolysis is a serious and life-threatening condition which has many established causes including endocrine disturbances. Of those, thyroid, adrenal and pituitary deficiencies are the most commonly seen. Most cases of rhabdomyolysis with adrenal insufficiency that have been reported have been primary. Here, we report an encounter with a patient who presented with her second case of severe rhabdomyolysis in the setting of secondary adrenal insufficiency. The cause for corticotropic suppression was most likely autoimmune hypophysitis given the presence of other autoimmune comorbidities incl…

Immunoglobulin G4-Related Disease: An Update.

Immunoglobulin G4-related disease (IgG4-RD) is an increasingly recognized immune-mediated condition comprised of a collection of disorders that share specific pathological, serological, and clinical features. IgG4-RD is a fibroinflammatory condition with a tendency to form tumors with inflammatory infiltrate with IgG4 rich plasma cells and elevation of serum IgG4, which may affect virtually every organ and tissue. IgG4-related ophthalmic disease may present as dacryoadenitis, myositis, or involvement of other orbital tissue. Hypophysitis or pachymeningitis may manifest as cranial neuropathies.…

Sellar inflammatory mass with inflammatory bowel disease.

Inflammatory bowel disease may be associated with different intracranial disorders. An inflammatory sellar mass is very rare but includes a variety of noninfectious causes including lymphocytic hypophysitis, granulomatous inflammation and Wegener's granulomatosis. A 32-year-old man was diagnosed with an inflammatory sellar mass associated with an extensive colonic inflammatory process clinically characteristic of Crohn's disease. The concurrent onset of these inflammatory disorders in distinctly separate sites may reflect their common embryological origin or represent an unusual form of metast…

Lymphocytic infundibulo-neurohypophysitis: An unusual cause of recurrent optic neuropathy in a child.

Lymphocytic infundibulo-neurohypophysitis (LINH), a rare autoimmune disease, is distinct from lymphocytic hypophysitis and is characterized by lymphocytic and plasma cell infiltration of the posterior lobe of the pituitary and the pituitary stalk.(1) We report the case of a young boy who presented with recurrent, steroid-responsive optic nerve dysfunction and diabetes insipidus. T1-weighted magnetic resonance imaging scans of the brain showed an isointense soft tissue mass within the sella turcica and a thickened pituitary stalk. Tissue specimen resected at transsphenoidal surgery revealed chr…

MR imaging of central diabetes insipidus: a pictorial essay.

Central diabetes insipidus (DI) can be the outcome of a number of diseases that affect the hypothalamic-neurohypophyseal axis. The causes of the condition can be classified as traumatic, inflammatory, or neoplastic. Traumatic causes include postoperative sella or transection of the pituitary stalk, while infectious or inflammatory causes include meningitis, lymphocytic hypophysitis, and granulomatous inflammations such as sarcoidosis and Wegener's granulomatosis. Various neoplastic conditions such as germinoma, Langerhans cell histiocytosis, metastasis, leukemic infiltration, lymphoma, teratom…

Pituitary apoplexy within a prolactin-secreting macroadenoma in a child: A rare clinical entity with radiological correlation.

Pituitary apoplexy is an uncommon but potentially fatal syndrome of the pituitary gland, caused by ischemia, hemorrhage, or necrosis that can occur with or without a pre-existing pituitary neuroendocrine tumor (PitNET), as reported during pregnancy and in lymphocytic hypophysitis. We present in this paper a case of an 11-year-old girl with pituitary apoplexy. Our patient experienced sudden deterioration in her left eye vision and a headache. The ophthalmological examination reported a divergent strabismus. Brain imaging demonstrated an intra-suprasellar mass with hemorrhagic areas. This paper …
