A review of the evidence for a natalizumab exit strategy for patients with multiple sclerosis.

Natalizumab is an effective treatment for relapsing-remitting multiple sclerosis (RRMS), but treatment for >2 years is associated with an increased risk of opportunistic infection and progressive multifocal leukoencephalopathy (PML). For this reason, patients and physicians may consider discontinuing natalizumab therapy. This article reviews the evidence for the various therapeutic approaches that may be taken in such patients. Stopping therapy altogether is unlikely to be appropriate for most patients, as it is associated with a high rate of relapse or rebound. Continuing natalizumab therapy

Clinical Evaluation and Management of Inflammatory Myopathies.

The idiopathic inflammatory myopathies are classified into four distinct subtypes: dermatomyositis, polymyositis, inclusion body myositis, and immunomediated necrotizing myopathy. These subtypes are clinically, histologically, and pathogenically distinct. Dermatomyositis, polymyositis, and necrotizing myopathy generally respond to immunotherapy, whereas inclusion body myositis does not. In this review, the authors focus on the clinical evaluation and management of idiopathic inflammatory myopathies and highlight recent therapeutic studies and the evolving role of myositis-specific antibodies.

Infant botulism: review and clinical update.

Botulism is a rare neuromuscular condition, and multiple clinical forms are recognized. Infant botulism was first identified in the 1970s, and it typically occurs in infants younger than 1 year of age who ingest Clostridium botulinum spores. A specific treatment for infant botulism, intravenous botulism immunoglobulin (BIG-IV or BabyBIG®), was developed in 2003, and this treatment has substantially decreased both morbidity and hospital costs associated with this illness. This article will review the pathogenesis of infant botulism as well as the epidemiology, clinical manifestations, diagnosis

Mast cell specific immunological biomarkers and metabolic syndrome among middle-aged and older Chinese adults.

The main aim of this study is to explore whether these mast cell specific immunological biomarkers [immunoglobulin E (IgE), chymase and tryptase] is an independent risk factor of MetS and whether the combined action of these biomarkers increased the associations with MetS. Three mast cell-specific immunological biomarkers were measured using enzyme linked immunosorbent assay (ELISA). One-way analysis of covariance and logistic regression models were used for analyzing the associations between immunological biomarkers with MetS. A total of 340 participants, 82 (24.1%) individuals had diabetes m

Temporal profile of magnetic resonance angiography and decreased ratio of regulatory T cells after immunological adjuvant administration to mice lacking RNF213, a susceptibility gene for moyamoya disease.

Moyamoya disease (MMD) is a chronic, occlusive cerebrovascular disease with an unknown etiology and is characterized by an abnormal vascular network at the base of the brain. Recent studies identified the RNF213 gene (RNF213) as an important susceptibility gene for MMD; however, the mechanisms underlying the RNF213 abnormality related to MMD have not yet been elucidated. We previously reported that Rnf213-deficient mice and Rnf213 p. R4828K knock-in mice did not spontaneously develop MMD, indicating the importance of secondary insults in addition to genetic factors in the pathogenesis of MMD.

Understanding and Leveraging Implementation Science Within a Clinical Informatics Framework for Allergy/Immunology.

The integration of research discoveries into routine clinical care is often slow and resource-intensive, particularly in allergy and immunology, where advances in biologics, immunotherapy, precision diagnostics, and guideline-driven management continue to evolve rapidly. Implementation science provides a structured approach to understand and address the gaps between research discovery and real-world patient care. In asthma, food allergy, atopic dermatitis, drug allergy, and primary immune disorders, variation in clinician knowledge, health system infrastructure, reimbursement models, patient d

Identification of a circulating immunological signature as a liquid biopsy approach for the diagnosis of endometriosis.

Endometriosis is a chronic inflammatory disease characterized by ectopic endometrial tissue growth, causing pain and infertility. Diagnosis is often delayed due to reliance on imaging and invasive methods, highlighting the need for non-invasive biomarkers for early detection. We have performed a case-control study with 78 endometriosis patients and 48 healthy controls undergoing salpingectomy. Blood samples were analyzed for soluble cytokines and immune checkpoints using a bead-based flow cytometry assay. Logistic regression models combining immunological markers with demographic and clinical

Permanent detrimental effect of nonimmunological factors on long-term renal graft survival: a parsimonious model of time-dependency.

PURPOSE: We attempted to model and test the pattern of effects of prognostic factors on renal graft survival during the posttransplantation time course. PATIENTS AND METHODS: Patients who received a cadaveric kidney-only transplant between January 1990 and December 1995 in Eurotransplant, who received cyclosporine as induction therapy, and who had a complete follow-up at the time of analysis were included in the study (n= 10614). An index summarizing all covariate information was calculated and used for modeling the time-dependent effects with relation to graft failure. RESULTS: The immunolo

Impact of Induction Therapy on Preventing Early Acute Kidney Allograft Rejection: A Single-Center Experience Study.

OBJECTIVE: Acute rejection infrequently occurs among immunologically low-risk recipients within the first few weeks after transplantation, and the role of induction treatment in the frequency of acute rejection and graft loss remains debatable. MATERIALS AND METHODS: This retrospective study analyzed 208 kidney transplant recipients with low immunological risk, defined by living donor transplantation, no prior transplantation history, absence of preformed anti-HLA antibodies, and a negative lymphocyte crossmatch prior to transplantation. Demographic data, immunologic characteristics, and graf

Elevated α-Ketoglutaric Acid Concentrations and a Lipid-Balanced Signature Are the Key Factors in Long-Term HIV Control.

Long-term elite controllers (LTECs) are a fascinating small subset of HIV individuals with viral and immunological HIV control in the long term that have been designated as models of an HIV functional cure. However, data on the LTEC phenotype are still scarce, and hence, the metabolomics and lipidomics signatures in the LTEC-extreme phenotype, LTECs with more than 10 years of viral and immunological HIV control, could be pivotal to finding the keys for functional HIV remission. Metabolomics and lipidomics analyses were performed using high-resolution mass spectrometry (ultra-high-performance l

Murine Q Fever Vaccination Model Reveals Sex Dimorphism in Early Phase Delayed-Type Hypersensitivity Responses.

Delayed-type hypersensitivity (DTH) responses to microbial vaccines and related components are a major roadblock for widespread licensing of whole cell vaccines such as that of Q fever. Q fever is a zoonotic disease caused by the intracellular bacterium Coxiella burnetii. The only currently licensed vaccine, Q-Vax®, is a whole cell inactivated formulation that is associated with a potentially severe dermal post vaccination DTH response in previously sensitized individuals. To investigate the underlying immunologic mechanisms of this response and better represent the early-phase DTH response ob

Interplay between ESKAPE Pathogens and Immunity in Skin Infections: An Overview of the Major Determinants of Virulence and Antibiotic Resistance.

The skin is the largest organ in the human body, acting as a physical and immunological barrier against pathogenic microorganisms. The cutaneous lesions constitute a gateway for microbial contamination that can lead to chronic wounds and other invasive infections. Chronic wounds are considered as serious public health problems due the related social, psychological and economic consequences. The group of bacteria known as ESKAPE (Enterococcus faecium, Staphylococcus aureus, Klebsiella pneumoniae, Acinetobacter baumannii, Pseudomonas aeruginosa and Enterobacter sp.) are among the most prevalent

Immune biomarkers in older adults: Role of physical activity.

Aging is associated with a decline in the normal functioning of the immune system. Several studies described the relationship between immunological alterations, including immunosenescence and inflammation, and aging or age-related outcomes, such as sarcopenia, depression, and neurodegenerative disorders. Physical activity is known to improve muscle function and to exert a number of benefits on older adult health, including reduced risk for heart and metabolic system chronic diseases. However, the positive influence of physical activity on the immune system has not been elucidated. In order to

Immunoplacental therapy, a potential multi-epitope cancer vaccine.

The field of tumor immunology has made great advancements in recent years. A retrospective analysis of previous vaccine strategies combined with present knowledge may provide additional insight in this treatment modality. This article provides a review of immunoplacental therapy (IPT), a cancer vaccine consisting of chorionic villi extractions from the human placenta after a live full-term delivery. This therapy was first introduced in the 1970s by Valentin I. Govallo, M.D., Ph.D., who noted the immunological similarities between pregnancy and cancer. The goal of cancer immunotherapy, accordin

Identification of a novel role for the immunomodulator ILRUN in the development of several T cell subsets in mice.

Inflammation and lipid regulator with UBA-like and NBR1-like domains (ILRUN) is a protein-encoding gene associated with innate immune signaling, lipid metabolism and cancer. In the context of innate immunity, ILRUN inhibits IRF3-mediated transcription of antimicrobial and proinflammatory cytokines by inducing degradation of the transcriptional coactivators CBP and p300. There remains a paucity of information, however, regarding the innate immune roles of ILRUN beyond in vitro analyses. To address this, we utilize a knockout mouse model to investigate the effect of ILRUN on cytokine expression
