Cardiogenic shock is the most common cause of death in patients hospitalized with acute myocardial infarction and is associated with a poor prognosis. More than 75% of cases are due to extensive left ventricular infarction and ventricular failure. Other causes include right ventricular infarction and papillary muscle rupture with acute severe mitral regurgitation. Activation of neurohormonal systems and the systemic inflammatory response worsens shock. We describe our experience in treating a 69-year-old man with spinal progressive muscular atrophy (SPMA), who underwent a mitral valve replacement. BACKGROUND: Veno-arterial extracorporeal membrane oxygenation (VA-ECMO) is a commonly used in refractory cardiogenic shock (CS) but may exacerbate left ventricular (LV) loading conditions and impair cardiac energetics. In the extreme, patients may present with cardiogenic shock (CS) requiring mechanical circulatory support (MCS). BACKGROUND: Heart transplantation (HTx) candidates supported by Impella (Abiomed, Danvers, MA) or intraaortic balloon pump (IABP), who demonstrate evidence of cardiogenic shock, may qualify for waitlist status 2 without exception under the new donor heart allocation system. However limited data comparing Impella versus IABP as a bridge to HTx exist.
