BACKGROUND: The medial patellofemoral ligament (MPFL) is the most frequently injured soft tissue structure following acute lateral patellar dislocation. MPFL reconstruction has become a popular option to restore patellar stability following lateral patellar dislocation due to the high incidence of recurrent instability following conservative management. Anatomic reconstruction of the MPFL minimizes graft length changes during full knee range of motion and restores patellar stability.
MATERIALS & METHODS: Four fresh frozen cadaver specimens underwent biomechanical testing in a materials testin

In a 51-year-old woman with a history of fractures and dislocations after low intensity trauma in childhood, intensive blue sclera, short stature, and hearing loss, the diagnosis of osteogenesis imperfecta (OI) was suspected. She was referred to our clinic with hand deformities and left knee pain and stiffness. She had difficulty in walking and reported a history of immobilization for 6 months because of knee pain. She had bilateral flexion contracture of the elbows which occurred following dislocations of the elbows in childhood. She had Z deformity of the first phalanges, reducible swan-neck

A case of Larsen's syndrome in a boy is presented. The syndrome is characterized by flattened facies, multiple congenital dislocations and foot deformities as a result of connective tissue maldevelopment during gestation. The importance of early intensive conservative therapy is emphasized. In the case reported, conservative therapy led, in the first stage, to the management of subluxation of both hips and improvement in left foot position and in the state of both knees. Subsequently, dislocations of the right and the left knee joint were surgically treated at 16 and 20 months, respectively, b

BACKGROUND: The concepts of lateral deviation and lateral inclination of the patella, characterized as shift and tilt, have been applied in combination to evaluate patellar malalignment in patients with patellar dislocation. It is not reasonable, however, to describe the 3-dimensional (3D) positional relation between the patella and the femur according to measurements made on 2-dimensional (2D) images.
PURPOSE: The current study sought to clarify the relation between lateral deviation and inclination of the patella in patients with recurrent dislocation of the patella (RDP) by redefining them

Patellar osteochondral fractures with no dislocation are uncommon and usually affect the centromedial facet of the patella. We present the case of a 10 year-old, overweight, female patient. She was seen in the emergency room after suffering an accidental fall, assessed as an osteochondral fracture-dislocation of the right patella with upper-outer displaced free fragments. By patient interview, she referred to no previous episodes of patellar dislocation. To complete the study, we performed an MRI which showed a medial facet patellar fracture, with two osteochondral fragments located in the sub

AIM: Patellar dislocations are a common injury of the knee joint. During patella dislocations injuries of soft-tissue structures can occur that can destabilise the patella and lead to recurrent dislocations. There are also congenital pathologies that predispose to patella dislocations. In the current literature, diagnostics and treatment of patellar dislocations are frequently discussed. Therefore the aim of our survey was to analyse and summarise actual diagnostic and therapeutic strategies regarding primary and recurrent patella dislocations.
METHODS: An online questionnaire form was sent t

PURPOSE: Synovial plicae and their relation to pain and disability have been reported in the orthopedic literature in association with the knee and other extremity joints. However, the occurrence of synovial plicae in the temporomandibular joint (TMJ) have rarely been reported. This report describes the surgical appearance, distribution, and histologic findings of synovial plicae in patients with TMJ recurrent dislocation and internal derangement.
MATERIALS AND METHODS: Twenty consecutive patients, 16 with recurrent dislocation and 4 with internal derangement, who underwent open TMJ surgery b

INTRODUCTION: Peritalar dislocation is a rare injury involving simultaneous disruption of the talocalcaneal and talonavicular joints, usually following high-energy trauma. Early recognition and management are crucial to prevent chronic instability, stiffness, or post-traumatic osteoarthritis. We report a case of peritalar dislocation in a 48-year-old patient.
CASE REPORT: A 48-year-old male sustained an isolated medial peritalar dislocation of the right foot after a road traffic accident. Clinical examination revealed medial displacement with a prominent talar head palpable through a submalle

Nontraumatic haemarthrosis secondary to anticoagulant therapy is a recognized clinical problem. The involved joints are usually large weight-bearing joints, particularly the knee. We describe a case of spontaneous haemarthrosis in a 53-year-old female patient, who had been started on anticoagulant drugs after a pulmonary embolism in the past (international normalized ratio on admission: 2.4). The patient presented with acute onset of severe shoulder pain with no history of trauma. Her shoulder showed radiological signs of anterior dislocation. Attempts to relocate her shoulder under intravenou

A total of 109 consecutive total condylar knee replacements in 73 patients were assessed at one to eight years postoperatively. The average age at surgery was 66.9 years and the range was from 52 to 84 years. The primary disease was rheumatoid arthritis in 12 and degenerative osteoarthritis in 97 knees. Out of the 109 prostheses; 101 were Miller-Galante kinematic condylar type I or II prostheses, seven were Insall-Burstern type I or II and one was a Whiteside prosthesis. The fixation technique was hybrid (only the tibial prosthesis cemented) in 63, both components uncemented in two and both co

INTRODUCTION: Larsen syndrome (LS) is a rare genetic disorder affecting mainly the connective tissues. It is characterized by characteristic facial anomalies, cervical kyphosis, cardiorespiratory disorders, and multiple joint dislocations. We present a case of a 15-year-old male with unstable neuropathic knee joint instability in a known case of LS. The paucity of literature on the management of this rare condition puts an orthopedician in dilemma regarding the optimal treatment.

BACKGROUND: Charcot neuroarthropathy is a systemic disease that generates pathological changes in the musculoskeletal system, causing instability, dislocations, and deformities. Charcot neuroarthropathy of the knee, due to either diabetes mellitus or syringomyelia, is anecdotally reported with the epidemic of the diseases. However, idiopathic sensory peripheral neuropathy can inflict osteoarticular structures directly, inducing a dysfunctional Charcot neuroarthropathy. An early diagnosis and effective relief of the symptomatic deformity is essential for the treatment.

Anterior knee pain is a common complain in all ages athletes. It may be caused by a large variety of injuries. There is a continuum of diagnoses and most of the disorders are closely related. Repeated minor trauma and overuse play an important role for the development of lesions in Hoffa's pad, extensor mechanism, lateral and medial restrain structures or cartilage surface, however usually an increase or change of activity is referred. Although the direct relation of cartilage lesions, especially chondral, and pain is a subject of debate these lesions may be responsible of early osteoarthrosis

BACKGROUND: Patellar instability is a common pediatric knee injury. The purpose of this study was to analyze patient-reported outcome measures (PROMs) after nonoperative treatment following a first-time pediatric patellar dislocation.
METHODS: In this single-center, retrospective study, patients <18 years old between January 2012 and December 2019 with a first-time patellar dislocation were evaluated. From this cohort, we included all those patients who initially underwent nonoperative management and followed up for at least 1 year. Measures of patellar height and patellofemoral dysplasia wer

An ergonomic upright body posture is maintained by the alignment of the spine, pelvis, and lower extremities, and the muscle strength of body trunk and lower extremities. The posture varies with age because of the degenerative changes in the involved structures and the weakening of the muscles. The compensatory mechanisms underlying these changes have recently been evaluated, and the loss of lumbar lordosis results in spinal kyphosis, pelvic retroversion, hip extension, knee flexion, and ankle dorsiflexion. These mechanisms are referred to as the hip-spine and knee-spine syndromes. The spine,

In spite of continuing progress, total knee arthroplasty still presents many problems. In this clinic 25 arthroplasties have been performed in 22 patients over the last 4 1/2 years. Guépar hinges were used in 21 knees and Marmor modular knees in 4. Acrylic cement was used in all cases. The mean age of the patients was 69 years. 13 had osteoarthritis, 5 rheumatoid disease and 4 other conditions. There were no serious general complications or infections in the early postoperative period. The commonest local complication was malalignment of the patella (17 cases); 2 of these were complete but red
