PURPOSE: To analyze the effectiveness of various methods of treating dizziness with fistulas of the labyrinth (FL) of inflammatory genesis. The incidence of autoimmune inner ear disease (AIED) is difficult to determine: probably it is a rare disease, accounting for <1% of all cases of hearing impairment or dizziness. A 6-year review of complications of mastoid surgery between June 1995 and June 2001 revealed five cases with serious iatrogenic complications from mastoid surgery, of which four were facial nerve palsy and two were labyrinthine fistula. Calculating the SP/AP ratio in electrocochleography requires the interpreting audiologist to unequivocally identify the peak of the SP wave form. If different points are selected, different SP/AP ratios will result. To investigate this effect, 50 electrocochleographic tracings were sent to 10 different audiologists (500 tracings). Hantavirus-induced acute viral infection of the inner ear of guinea pigs is reported. Guinea pigs were inoculated directly into the inner ear through the round window, or intraperitoneally with hantavirus (B-1 strain), and the cochlea was examined using an immunohistochemical technique for the localization of viral antigens. Virus-specific antigens were detected prominently in the stria vascularis and to a lesser degree in other inner ear structures of the infected guinea pigs. The case is reported of a 47-year old female suffering from the exploding head syndrome. This syndrome consists of a sudden awakening due to a loud noise shortly after falling asleep, sometimes accompanied by a flash of light. The patient is anxious and experiences palpitations and excessive sweating. Most patients are more than fifty years of age. Further investigations do not reveal any abnormality. The pathogenesis is unknown, and no therapy other than reassurance is necessary.

A series of 79 otic capsule fistulae occurring in 792 consecutive cases of chronic mastoiditis undergoing mastoidectomy was analyzed. The incidence (10%) is the same as in other large series, the primary difference being the location of the fistula. In all series cited, 90% of fistulae were in the lateral semicircular canal whereas in this series the comparable incidence was 75%.
