B-acute lymphoblastic leukemia/lymphoblastic lymphoma.

OBJECTIVES: This session of the 2013 Society of Hematopathology/European Association for Haematopathology Workshop was dedicated to B-acute lymphoblastic leukemia (B-ALL)/lymphoblastic lymphoma (LBL) with recurrent translocations and not otherwise specified. METHODS: In this review, we summarize the cases discussed during the workshop, review the pertinent and most recent literature on the respective topics, and provide a few key points that may aid in the workup of patients with B-ALL/LBL. RESULTS: Many of the submitted cases showed interesting diagnostic, immunophenotypic, or clinical aspe…

HAUSP compartmentalization in chronic myeloid leukemia.

INTRODUCTION: PTEN plays an essential role in the pathogenesis of chronic myeloid leukemia. Recently, we have shown that BCR-ABL promotes PTEN nuclear exclusion, through the modulation of HAUSP activity. OBJECTIVES: Here, we investigate HAUSP cellular compartmentalization in primary CML samples. RESULTS: While in normal CD34 positive cells HAUSP is expressed mostly in the nucleus, in CML CD34 cells HAUSP is expressed both in the nuclear bodies and in the cytoplasm. CONCLUSIONS: This observation suggests that HAUSP behaves as a shuttling protein in CML. It can bind to BCR-ABL in the cytosol,…

[Adult T cell leukemia-lymphoma (ATL)].

Adult T-cell leukemia-lymphoma(ATL) is a distinct malignancy of CD4+/CD25+/ CCR4+/FoxP3+ or - Treg/TH2 cells etiologically associated with human T-cell lymphotropic virus type I (HTLV-1). ATL is a single HTLV-1 disease entity with diverse molecular features. Also, the clinical features and prognosis are diverse leading to subtype-classification into acute, lymphoma, chronic, and smoldering types defined by organ involvement, and LDH and calcium values. In case acute, lymphoma or unfavorable chronic subtypes (aggressive ATL), and favorable chronic or smoldering ATL (indolent ATL), intensive che…

New developments in acute lymphoblastic leukemia.

Acute lymphoblastic leukemia (ALL) occurs in both children and adults. Significant improvements in survival outcomes have been realized over the last decade for all age groups with de novo ALL. Frontline treatment incorporates a tailored approach, based on factors such as the patient’s age and the disease subtype. Children, adolescents, and young adults are likely to receive intensifying or deintensifying chemotherapy regimens using standard chemotherapeutics (eg, anthracyclines, vincristine, asparaginase) based on risk stratification. Older adults appear to benefit from reduced-intensity chem…

Adoptive T-cell therapy for Leukemia.

Allogeneic stem cell transplantation (alloSCT) is the most robust form of adoptive cellular therapy (ACT) and has been tremendously effective in the treatment of leukemia. It is one of the original forms of cancer immunotherapy and illustrates that lymphocytes can specifically recognize and eliminate aberrant, malignant cells. However, because of the high morbidity and mortality that is associated with alloSCT including graft-versus-host disease (GvHD), refining the anti-leukemia immunity of alloSCT to target distinct antigens that mediate the graft-versus-leukemia (GvL) effect could transform…

Autoimmune cytopenias in chronic lymphocytic leukemia.

Chronic lymphocytic leukemia (CLL) is frequently complicated by secondary autoimmune cytopenias (AIC) represented by autoimmune hemolytic anemia (AIHA), immune thrombocytopenia (ITP), pure red cell aplasia, and autoimmune granulocytopenia. The distinction of immune cytopenias from cytopenias due to bone marrow infiltration, usually associated with a worse outcome and often requiring a different treatment, is mandatory. AIHA and ITP are more frequently found in patients with unfavorable biological risk factors for CLL. AIC secondary to CLL respond less favorably to standard treatments than thei…

Overcoming resistance in chronic myelogenous leukemia.

Resistance in chronic myelogenous leukemia is an issue that has developed in parallel to the availability of rationally designed small molecule tyrosine kinase inhibitors to treat the disease. A significant fraction of patients with clinical resistance are recognized to harbor point mutations/substitutions in the Abl kinase domain, which limit or preclude drug binding and activity. Recent data suggest that compound mutations may develop as well. Proper identification of clinical resistance and prudent screening for all causes of resistance, ranging from adherence to therapy to Abl kinase mutat…

Ever-advancing chronic myeloid leukemia treatment.

Treatment of chronic myeloid leukemia (CML) has been drastically changed by the emergence of the ABL tyrosine kinase inhibitor (TKI), imatinib mesylate. However, resistance and intolerance have frequently been reported, particularly in patients with advanced-stage disease. Point mutations within the ABL kinase domain that interfere with imatinib binding are the most critical cause of imatinib resistance. To overcome this resistance, four second-generation ATP competitive ABL TKIs, dasatinib, nilotinib, bosutinib and bafetinib, have been developed. Dasatinib and nilotinib also demonstrated high…

Extracellular vesicles in chronic lymphocytic leukemia.

Extracellular vesicles (EVs) are membrane-enclosed nanoparticles 30 to 1000 nm in size and represent a novel mechanism of cell communication. By transferring RNA and protein from their cell of origin, they can reprogram target cells and thus are involved in changes within the cellular microenvironment - a key player in CLL pathogenesis. In the current study, we were able to isolate EVs of 20 to 300 nm from blood plasma of CLL patients as well as from supernatant of primary CLL cells in culture. Further, proteomic profiling by Coomassie staining of SDS-PAGE gels and by mass spectrometry reveale

Williams-Beuren Syndrome and Burkitt Leukemia.

Williams-Beuren Syndrome (WBS) is associated with constitutional deletion of 7q11.23, which includes the elastin gene. Cytogenetic abnormalities of chromosome 7 are frequently described in several human malignancies. Here, we report Burkitt Leukemia in an 8-year-old boy with WBS. In this patient, constitutional deletion of chromosome 7q11.23 including BCL7B was confirmed. WBS may predispose patients to Burkitt Leukemia.

Chronic myelomonocytic leukemia: myelodysplastic or myeloproliferative?

Chronic myelomonocytic leukemia (CMML) is a clonal disease of the hematopoietic stem cell that provokes a stable increase in peripheral blood monocyte count. The World Health Organisation classification appropriately underlines that the disease combines dysplastic and proliferative features. The percentage of blast cells in the blood and bone marrow distinguishes CMML-1 from CMML-2. The disease is usually diagnosed after the age of 50, with a strong male predominance. Inconstant and non-specific cytogenetic aberrations have a negative prognostic impact. Recurrent gene mutations affect mainly t…

Autoimmune cytopenias in chronic lymphocytic leukemia.

The clinical course of chronic lymphocytic leukemia (CLL) may be complicated at any time by autoimmune phenomena. The most common ones are hematologic disorders, such as autoimmune hemolytic anemia (AIHA) and immune thrombocytopenia (ITP). Pure red cell aplasia (PRCA) and autoimmune agranulocytosis (AG) are, indeed, more rarely seen. However, they are probably underestimated due to the possible misleading presence of cytopenias secondary to leukemic bone marrow involvement or to chemotherapy cytotoxicity. The source of autoantibodies is still uncertain, despite the most convincing data are in f…

Biology of childhood acute lymphoblastic leukemia.

Acute lymphoblastic leukemia (ALL) is the most common malignancy in childhood, accounting for almost 30% of pediatric cancers. Despite the high rate of cure, ALL is one of the leading causes of death in children with tumor. For this reason, there is a keen interest in identifying genetic and biological features that influence the pathogenesis of ALL and the risk of treatment failure. The application of standard diagnostic technologies such as a conventional karyotype and polymerase chain reaction methodologies, together with gene expression profiling and genome-wide analyses, allows us to gene…

Aleukemic leukemia presenting with paranoid psychosis.

Mast cells not only synthesize and release serotonin, but also express and are activated through multiple serotonin receptors. Low blood serotonin level might define a specific subset of patients with systemic mastocytosis (SM) who are more likely to present with neurologic and gastrointestinal complaints. Mast cell leukemia (MCL) is a rare and aggressive type of systemic mastocytosis, and psychiatric manifestations in its course have not been well-characterized. We describe herein a unique patient with a KIT D816V mutation positive a leukemic leukemia variant of systemic mastocytosis with gas…

Musculoskeletal problems in pediatric acute leukemia.

Acute leukemia (AL) in children can mimic several orthopedic pathologies at presentation, with a variable delay in the correct diagnosis. This is a major problem, which may result in fractures, loss of mobility, and deformity, with resultant adverse effects on quality of life. Here, we studied the clinical and radiological musculoskeletal manifestations in children with AL. We reviewed 328 children [208 boys (62%), median age 7.2 years] with acute lymphoblastic (279, 85%) or myeloid (49, 15%) leukemia, treated between January 1982 and December 2003 by the Paediatric Oncology Service, Second Un…

Leukemic hypopyon in acute myeloid leukemia.

PURPOSE: Leukemic hypopyon uveitis in acute myeloid leukemia (AML) is a very rare condition. We report this case of an unusual finding in a 2½-year-old boy in second remission after chemotherapy for relapsed AML. METHODS: A young patient with an AML FAB M5 in second remission developed pain, photophobia, and conjunctival injections in the right eye. Recent bone marrow aspiration showed no blast increase and recent peripheral blood sampling presented no evidence of relapse. RESULTS: The patient showed a viscous hypopyon and a myotic pupil in the right eye that was refractory to corticosteroid…

Treatment options for chronic myeloid leukemia.

INTRODUCTION: The bcr-abl tyrosine kinase inhibitors (TKIs) are the cornerstone treatment for chronic myeloid leukemia (CML). However, there are many topics related to therapy that remain debated. AREAS COVERED: The aim of this paper is to give the reader a comprehensive review of how to treat CML at diagnosis, how to monitor the disease and a brief read of special populations and case scenarios. It describes the first-line (imatinib) and second-line (nilotinib and dasatinib) TKIs currently used for the treatment of CML, including landmark studies proving their efficacy, side effect profile, …
