Extrasinus Zygomatic Implants for the Immediate Rehabilitation of the Atrophic Maxilla: 1-Year Postloading Results From a Multicenter Prospective Cohort Study.

PURPOSE: The use of zygomatic implants is an alternative to major grafting procedures for the treatment of the atrophic posterior maxilla. However, only few studies reported solid and correct data about the use of extrasinus zygomatic implants. Starting from these considerations, this study aims to investigate the 1-year survival and success rates of extrasinus zygomatic implants immediately loaded in patients with severe atrophy of the posterior maxilla. METHODS: Twenty-three consecutive patients fully edentoulus or with failing dentition in the upper arch and with a severe atrophy of the po…

Primary intraosseous CD9-positive B-cell lymphoblastic lymphoma of the maxilla affecting a pediatric patient: Immunohistochemical and in situ hybridization analysis.

Lymphoblastic lymphoma (LBL) is a clinically aggressive disease, representing approximately 2% of all non-Hodgkin lymphoma cases. In the oral and maxillofacial (OMF) region, approximately 39 cases, diagnosed as LBL, acute lymphoblastic leukemia (ALL) or ALL/LBL, have been reported to date. Noteworthy, the CD9 expression, which indicates a poor outcome in ALL, has not been reported in LBL and lymphoblastic neoplasms of the OMF region. Herein, we report an additional maxillary intraosseous B-cell LBL, affecting a 14-year-old girl, which also showed positivity for CD9, Bcl-6 and MUM1/IRF4. Aiming…

Likelihood of needing facial bone augmentation in the anterior maxilla of Chinese Asians: A cone beam computed tomography virtual implant study.

BACKGROUND: The perfect position and axis of an implant is often limited by the morphology of the alveolar ridge, especially in Asians. PURPOSE: The aim of this study was to investigate the frequency at which ideal implant placement position in the anterior maxilla of Chinese Asians would require facial bone augmentation given by an initial facial bone profile. MATERIALS AND METHODS: Implants were virtually placed in 303 sites without tooth loss in the anterior maxilla. The incidence and characters of implant facial plate fenestration were analyzed. If there was no facial fenestration, the t…

Biomechanical Interaction Between Endosseous and Patient-Specific Subperiosteal Implants in Full-Arch Rehabilitation of the Atrophic Maxilla: A Finite Element Analysis.

PURPOSE: To compare the biomechanical behavior of a hybrid maxillary full-arch reconstruction supported by conventional endosseous implants and a patient-specific subperiosteal implant with a reconstruction supported by bilateral patient-specific subperiosteal implants. MATERIALS AND METHODS: Three-dimensional finite element models of a severely atrophic maxilla were constructed from computed tomography data. Two full-arch prosthetic configurations were analyzed: a hybrid model (three endosseous implants on one side and a patient-specific subperiosteal implant on the contralateral side) and a…

Effectiveness of Two Different Flap Advancement Techniques for Vertical Bone Augmentation in the Anterior Maxilla: A Comparative, Split-Mouth Cadaver Study.

INTRODUCTION: Achieving tension-free flap closure is a critical challenge in vertical ridge augmentation of the anterior maxilla, where anatomical constraints limit flap mobility and soft tissue advancement. This study compares the split-thickness flap (control) and sub-orbicularis full-thickness flap (test) in cadaveric models to evaluate their impact on soft tissue thickness, flap integrity following periosteal release, and mucogingival junction (MGJ) displacement. The goal is to characterize and optimize surgical approaches for vertical bone augmentation in the anterior maxilla. MATERIALS …

Atrophic Maxilla Rehabilitation Through Dental Implants Using the V-4 Strategy Associated With Guided Surgery: A Case Report and Technique Description.

Atrophic maxilla rehabilitation through dental implants is always a challenging procedure. However, alternative approaches such as guided surgery and the installation of short implants are progressively supplanting more invasive bone regeneration procedures. A V-4 technique described in 2016 facilitates the installation of dental implants in patients with atrophic maxilla; however, its authors recommend incision, flap opening, and elevation of the anterior area of the maxillary sinuses. This case report describes a less invasive proposal for modifying the technique through the association of g…

Functional Endoscopic Sinus Surgery for Paranasal Sinusitis Originating From a Peri-implantitis-Triggered Infection in the Augmented Maxilla: A Case Report.

The aim of this case report was to report the course of treatment for advanced paranasal sinus infection triggered by peri-implantitis, managed using functional endoscopic sinus surgery (FESS), with outcomes. A nonsmoking male patient received sinus augmentation with implant placement on his left posterior maxilla 15 years ago. Possibly due to noncompliance to maintenance, peri-implantitis developed and progressed into the augmented bone area in the maxilla. Eventually, maxillary sinusitis occurred concomitantly with a spread of the infection to the other paranasal sinuses. Implant removal and…

Clinical manifestations of diffuse large B-cell lymphoma that exhibits initial symptoms in the maxilla and mandible: a single-center retrospective study.

BACKGROUND: Diffuse large B-cell lymphoma (DLBCL) is the most common type of lymphatic tumor; however, extranodal DLBCLs that exhibit initial symptoms in the maxilla and mandible are rare. Moreover, DLBCL is clinically classified as a moderate to highly malignant lymphatic tumor that can progress rapidly; therefore, early diagnosis is crucial. However, diagnosis is difficult as the disease causes a diverse range of clinical symptoms with no characteristic imaging findings. We conducted a clinical investigation to clarify the clinical characteristics of DLBCL that exhibits initial manifestation…
