Evaluation of Sinus Membrane Perforation in Osteotome Sinus Floor Elevation With and Without Grafting.

The purpose of this study was to evaluate, using cone-beam computerized tomography (CBCT), the rate of sinus membrane perforation in osteotome sinus floor elevation (OSFE) performed with and without a graft material. Thirty patients with 52 OSFE sites were included in the study. Patients were divided into the control group (OSFE performed without graft material) and test groups (OSFE performed with autograft or xenograft). The autograft was harvested from the maxillary tuberosity using bone forceps. The xenograft was a commercial product originating from bovine bone. Graft volume was measured

Glue embolisation of a bleeding pseudoaneurysm related to surgically-assisted rapid palatal expansion.

Vascular injuries after orthognathic surgery are rare, and mainly occur in young adults after Le Fort I osteotomies. We report the case of a 14-year-old girl who presented with life-threatening epistaxis one week after a surgically-assisted rapid palatal expansion (SARPE) followed by activation of a transpalatal distractor. Definitive treatment was superselective, catheter-directed, glue-embolisation of a bleeding bilobar pseudoaneurysm, which was located at an end branch of the left sphenopalatine artery.

A peculiar complication in Le Fort I osteotomy.

In two cases of routine Le Fort I osteotomy in secondary cleft palate surgery, a swelling noted in the antral mucosa was biopsied. In both cases, histological examination resulted in a diagnosis of odontogenic myxoma. After analysis of the radiographs, it was concluded that the biopsies were taken from displaced tooth germs. The close histological similarity between myxoma on the one hand and dental papilla tissue, a thickened tooth follicle and a thickened or polypoid antral mucosal lining on the other is discussed.

Computer Aided Design/Computer Aided Manufacturing-Guided Scapular Tip Free Flap Reconstruction for Complex Maxillofacial Defects.

BACKGROUND: Reconstruction of maxillary and mandibular defects following oncologic resection remains challenging due to their three-dimensional complexity and critical role in function and aesthetics. The scapular tip free flap (STFF) provides reliable vascularity, substantial bone stock, and chimeric versatility. Computer-Aided Design and Manufacturing (CAD/CAM) has been widely applied in reconstructive surgery to optimize resection accuracy and flap insetting, but its role in STFF supine harvest and inset has never been reported. This report aims to describe the advantages of application of

Epistaxis in dental and maxillofacial practice: a comprehensive review.

The lifetime incidence of epistaxis in dental and maxillofacial practice has been reported to be as high as 60% and can be caused by dental implant placement, Le Fort I osteotomy, intranasal supernumerary tooth, odontogenic tumors, blood disorders and maxillofacial trauma. Most epistaxis cases are minor and easily managed with direct compression on the nares for 10 minutes. For more significant or recurrent epistaxis, other techniques might include electrocautery, anterior or posterior nasal packing, or Foley catheter balloon. For patients with refractory epistaxis, cauterization of the spheno

Three-year analysis of tapered screw vent implants placed into maxillary sinuses grafted with mineralized bone allograft.

With an increase in the number of patients presenting for dental implant treatment, it is becoming more common for clinicians to encounter inadequate bone volume. Several surgical techniques have been advocated for placing dental implants in the posterior maxilla, including the lateral window sinus elevation technique. This article reports the results of implants placed into maxillary sinuses grafted with particulate mineralized cancellous bone allograft alone or in combination with resorbable hydroxyapatite over a 3-year period. A total of 56 sinuses were grafted, and 136 dental implants were

Immediate replacement of single upper posterior teeth: a report of cases.

BACKGROUND: The immediate replacement of upper molar teeth with implants poses a significant challenge to the clinician because of its sinus involvement. PURPOSE: The purpose of this article is to report a protocol in which single molar teeth are replaced immediately with implants, minimizing the need for other surgical procedures. MATERIALS AND METHODS: This protocol has been employed in eight cases. Briefly, the teeth are extracted atraumatically, the sockets evaluated, an osteotomy performed following the direction of the socket, and a wide diameter implant placed using the socket walls a

Intracranial migration of halo fixation pins: a complication of using an extraoral distraction device.

OBJECTIVE: Distraction osteogenesis is a well-accepted technique in the treatment of patients with hypoplastic craniofacial components. Complications of distraction osteogenesis are well described in the literature. We describe a complication of using an external distraction device in a 9-year-old girl with Pfeiffer. INTERVENTION AND RESULTS: A modified Lefort III osteotomy was performed for maxillary hypoplasia with application of an external distraction halo device by a pediatric neurosurgeon. A postoperative computed tomography (CT) scan showed 0.5-cm skull penetration of the cranial pins.

Surgical alternatives for treatment of obstructive sleep apnoea: review and case series.

Obstructive sleep apnoea (OSA) affects up to 24% of middle aged males, resulting in excessive daytime sleepiness, cognitive dysfunction, hypertension, increased risks for acute coronary events, stroke and auto accidents. At special risk are individuals with BMI greater than 29, neck circumference greater than 43 cm (17 inches), macroglossia and retrognathia. Although non-surgical nasal positive air pressure (nCPAP) and mandibular advancement oral appliances are often effective and are the most widely used therapy, they are not definitive for more severe OSA and patients show poor long-term com

Radical surgical treatment of bisphosphonate related osteonecrosis of the jaw.

We describe a case of a 64-year-old female diagnosed with multiple myeloma in 2001. She was treated with pamidronate and subsequently zolodronic acid before developing spontaneous bisphosphonate related osteonecrosis (BRONJ) of the left maxilla in December 2008. Over the next two years the BRONJ was treated conservatively but gradually became more symptomatic. About three years after her last dose of zolodronic acid with her symptoms increasing, she underwent radical surgical excision of all diseased bone and flap reconstruction. The patient is now six months postoperative and symptom free.

Central giant cell granuloma of the jaws.

Central giant cell granuloma was classified by the World Health Organization in 2005 as a rarely aggressive idiopathic benign intraosseous lesion that occurs almost exclusively in the jaws. It occurs most frequently in young women (aged <30 y). This osteolytic lesion histologically consists of proliferation of fibrous tissue, hemorrhagic focuses, hemosiderin deposits, osteoclast-like giant cells, and reactive bone formation. Differential diagnosis has to be made with other osteolytic neoformations of the jaws, both unicystic and multicystic (odontogenic tumors, fibrous dysplasia, cysts, etc).
