The carotid cavernous fistula has historically been associated with extensive facial trauma as a result of direct or indirect forces. Most fistulas of traumatic origin develop as a result of a fracture through the base of the skull, which produces a force causing laceration of the internal carotid artery in the region where it approximates the cavernous sinus. We report a case in which apparently minimal head trauma resulted in the development of a carotid cavernous fistula. The rather innocuous presentation of this complication requires particular attention by the caregiver in assessing the p

OBJECTIVE: We describe 2 patients with surgical injury to the lateral femoral cutaneous nerve (LFCN) after bone harvesting from the iliac crest for autologous bone grafting.
DESIGN: A case-series of 2 patients and literature study of all anatomical variants of the LFCN in relation to the anterior superior iliac spine and inguinal ligament.
SETTING: A teaching hospital in The Hague, the Netherlands.
PATIENTS: Two patients with surgical injury to the LFCN after bone harvesting from the iliac crest for autologous bone grafting.
RESULTS: All 9 known anatomical variations of the LFCN in the lit

BACKGROUND: Ectopic eruption of a tooth or osteoma rarely occurs within the maxillary sinus. Coexistence of these two rare entities in the maxillary sinus has never been reported in the English literature.
CASE REPORT: Herein, we present a 21 year-old male patient with osteoma and ectopic tooth in the left maxillary sinus treated with the removal of the ectopic tooth by endoscopic sinus surgery and excision of the osteoma by the Caldwell-Luc procedure.
CONCLUSION: Unique coexistence of two different entities in the maxillary sinus is most likely due to pediatric facial trauma. Pediatric pati

PURPOSE: This study compares the usefulness of axial computed tomography (CT) with two-dimensional (2D) reformats and three-dimensionally (3D) rendered CT data in the treatment planning and management of patients.
PATIENTS AND METHODS: Twenty-nine patients who had been studied with CT were grouped into the following four clinical problems: 1) temporomandibular joint assessment (n = 8), 2) growth and development assessment (n = 4), 3) posttrauma and postoperative assessment (n = 13), 4) "other" (n = 4). The clinicians who treated these patients were surveyed for their opinions of the usefulnes

Maxillofacial fractures in the pediatric population are generally managed by conservative approaches such as soft diet and medication or semi-invasive procedures namely inter-maxillary fixation or circum-mandibular wiring. These approaches are preferred over any invasive treatment to minimize injury to the growing skeleton and tooth germs. Displaced fractures that cause functional problems such as restricted mouth opening, malocclusion or impaired breathing, mandate open reduction and internal fixation. However, surgical management is associated with morbidity related to general anesthesia, ri

The aim of this study is to evaluate the incidence of ophthalmic complications following midfacial fractures and investigate its relation to surgical or nonsurgical treatment. This article is a retrospective study, describing the spectrum and incidence of ophthalmic injury in 106 patients presenting with midfacial fractures at the Department of Oral and Maxillofacial Surgery of the University Hospitals Leuven over a period of 16 months (January 2013 to April 2014). The mean age of the patients was 45.6 years with a gender distribution of 68 men and 38 women. The main cause of trauma was road t

Maxillofacial traumatology requires mastering complex three-dimensional relationships that remain challenging to teach with conventional supports. Cadaveric dissection, still considered the gold standard, is increasingly restricted by tightening legislation on the academic use of human bodies. Radiological images and textbooks remain limited to two-dimensional projections. Three-dimensional (3D) printing provides tangible, reproducible and accessible models that can enhance spatial understanding. While 3D-printed anatomical models have already been reported in medical education, no permanent l

OBJECTIVE: Early treatment of fractures of the cranio-maxillofacial complex (CMFC) is challenging and likely to result in craniofacial deformity. Multidisciplinary team (MDT) care has developed very rapidly and has recently been accepted in cancer treatment. Therefore, the authors explored the application of MDT care with digital technology in CMFC fractures.
STUDY DESIGN: A 29-year-old man presented for treatment of CMFC fractures and bone defects. An MDT of oral surgeons, ophthalmic surgeons, neurological surgeons, and other experts was convened. After CT scan and three-dimensional reconstr

Appropriate medical care for a patient with a facial fracture can not only optimize aesthetic outcomes but also prevent the potential morbidity and mortality of delayed treatment. In this article, we focus on the clinical presentations, physical examination findings, diagnostic imaging, consultations, and follow-up that patients with facial fractures need related to their emergency department management. Specifically, we address the nuances of evaluating frontal, orbital, nasal, maxillofacial, and mandibular fractures.

A national sample of 1,775 Canadian dentists, representing 13.2 per cent of all Canadian dentists, were evaluated via a mailed questionnaire about their knowledge, awareness of, and experiences with victims of elder abuse and/or neglect. In all, 83.3 per cent of respondents were aware, to varying extents, of elder abuse or neglect. Of the dentists observing a patient suspected to have been a victim of neglect or abuse, 40.6 per cent reported incidents of neglect, while physical abuse was seen 59.4 per cent of the time. The types of neglect most frequently observed included: neglect of personal

The types of midfacial fractures and their complexity were evaluated in admissions to the Maryland Institute of Emergency Medical Service Systems (MIEMSS) during the years of 1984 to 1988. Two hundred and sixty-eight LeFort fractures were treated and followed (3.2 percent of admissions). One half (50 percent) had skull fractures and 40 patients (15 percent) had LeFort, skull and mandibular fractures. Isolated nasoethmoidal fractures were observed in 176 patients and in 107 patients (39 percent) of patients with LeFort fractures. Isolated mandibular fractures were observed in 321 patients and i

A case of a traumatic defect of the maxilla is described. The patient sustained an injury in the middle third of his face that involved loss of the anterior portion of the maxilla, the alveolus from the right molar to the left premolar region, and the anterior two thirds of the hard palate with comminuted fractures of the adjacent bones. The resulting defect was successfully closed by means of a skin-tube pedicle and cartilage graft. The discomfort and difficulties in swallowing and speech as well as masticatory disturbance were much improved. The grafted skin took on an appearance similar to

Dysgnathia (jaw and tooth anomaly, tooth misspacing, macillodento-facial anomaly) influences to a certain degree the development of defects of articulation especially sigmatism. Other factors, such as mild motor system inco-ordination and hearing defects, besides dysgnathia, are frequently observed. Speech therapy is necessary preceding as well as together with the maxillofacial treatment. The dental, prosthodontic and maxillofacial prostheses may likewise interfere with enunciation, but this will disappear spontaneously within 2 weeks or will be compensated for. If necessary the prosthesis wi
