Clinical ethics for the treatment of children and adolescents: a guide for general psychiatrists.

Clinical work--psychotherapy, assessment, consultation, or research--with emotionally ill children and adolescents is challenging, complex, and exceedingly rewarding. Psychiatrists must remain mindful of children's vulnerabilities but also remain respectful of their rights, regardless of any legal barriers or developmental limitations that exist. To varying degrees, youths are afforded the rights of integrity, autonomy, informed consent or assent, protection of health care information, and participation in research. All physicians who work with children and adolescents are obliged to possess t

Mood profile evaluation by families, young physicians and experienced psychiatrists in an actual clinical practice setting.

The purpose of this study was to explore differences in mood profile evaluation between raters with different backgrounds. Special emphasis was placed on expertise factor, influence of mood categories, and patient heterogeneity in a clinical setting. We administered Profile of Mood Scale (POMS) to patients' families, residents, and experienced psychiatrists as well as the patients, asking them to make an evaluation based on their image of the patients' subjective mood experience. Subscale scores and within-case across-item agreement indices among the raters were submitted for analysis. Analysi

Clinical and ethical implications of impaired executive control functions for patient autonomy.

The authors identify the clinical and ethical implications of impaired executive control functions for patient autonomy. Executive control functions are processes that orchestrate relatively simple ideas, movements, or actions into complex goal-directed behavior, and impairments in these functions are becoming more common as the population ages. The authors examine difficulties that individuals with impaired executive control functions may have in making treatment decisions and describe a practical, ethically justified framework for making treatment decisions for patients with impairments in t

Psychiatric aspects of sexual abuse involving persons with developmental disabilities.

OBJECTIVE: To consider psychiatric issues that relate to the prevention and management of sexual abuse involving persons with developmental disabilities as either abusers or victims. METHOD: Seven case descriptions illustrate the clinical challenges raised by allegations of sexual abuse. RESULTS: Mental disorders and communication problems often coexist with the cognitive impairment that is primary in developmental disabilities. Pedophilic behaviour prompts allegations of abuse, and posttraumatic stress disorder often follows abuse. Difficulties communicating with others may preclude the det

"Together at school"--a school-based intervention program to promote socio-emotional skills and mental health in children: study protocol for a cluster randomized controlled trial.

BACKGROUND: Schools provide a natural context to promote children's mental health. However, there is a need for more evidence-based, high quality school intervention programs combined with an accurate evaluation of their general effectiveness and effectiveness of specific intervention methods. The aim of this paper is to present a study protocol of a cluster randomized controlled trial evaluating the "Together at School" intervention program. The intervention program is designed to promote social-emotional skills and mental health by utilizing whole-school approach and focuses on classroom cur

Cognitive and functional impairment in Chinese elderly with late-onset depression.

OBJECTIVES. To investigate cognitive and functional impairment in Chinese elderly subjects with late-onset depression. METHODS. Subjects with late-onset depression and who were clinically non-demented were recruited. Their cognitive and functional scores were compared with those of cognitively normal elderly controls and elderly persons with mild cognitive impairment. Functional ability was assessed by the Disability Assessment for Dementia score. Various cognitive domains were assessed including global cognitive function, delayed episodic memory, working memory, and categorical verbal fluency

Unfitness to plead in England and Wales: Historical development and contemporary dilemmas.

Fitness to plead refers to a criminal defendant’s ability to participate at trial. The purpose of fitness-to-plead laws is to protect the rights of vulnerable individuals who are unable to defend themselves in court and to preserve natural justice in the legal system while balancing the needs to see justice served and protection of the public. Early legal systems treated mentally disordered defendants with leniency, but over time those found unfit to plead have been subjected to indefinite incarceration, breaching their right to liberty while protecting their right to a fair trial. Conversely,

A Rare PSEN1 (Leu85Pro) Mutation Causing Alzheimer's Disease in a 29-Year-Old Woman Presenting as Corticobasal Syndrome.

Dementia is not just a disease of old age. Early-onset dementia affects people younger than 65 and its differential diagnosis is broader than in older people. Nevertheless, although young people are considerably more liable to develop a rare form of dementia, Alzheimer's disease (AD) remains the most common diagnosis. The aim of this article is to report on an early-onset AD patient associated with the rare pathogenic variant PSEN1 (Leu85Pro) presenting as a possible corticobasal syndrome with asymmetric limb apraxia, parkinsonian signs, and myoclonus.

Preservation of the capacity to appoint a proxy decision maker: implications for dementia research.

CONTEXT: Research involving persons with impaired decision-making capacity (such as persons with Alzheimer disease [AD]) remains ethically challenging, especially when the research involves significant risk. If individuals incapable of consenting to research studies were able to appoint a research proxy, it would allow for an appointed surrogate (rather than a de facto surrogate) to represent the subject. OBJECTIVE: To assess the extent to which persons with AD retain their capacity to appoint a research proxy. DESIGN: Interview study. SETTING: Academic research. PARTICIPANTS: One hundred

Diminished emotionality and social functioning in schizophrenia.

There is indirect evidence to suggest that a subgroup of patients with schizophrenia exhibit a diminished capacity to experience both positive and negative emotions. To date however, no studies have focused specifically on this diminished emotionality (DE). The main objective of the present project was to determine whether patients with self-reported DE differed from other patients in level of social functioning, physical and social anhedonia, and negative/deficit symptoms. Seventy-three state hospital patients with DSM-IV diagnosed schizophrenia and 22 nonpsychiatric controls were examined. R

Self-Disclosure and Psychological Well-Being in Chinese Adolescents: Exploring the Role of Self-Perception.

INTRODUCTION: Self-disclosure is a fundamental aspect of human communication, particularly important for adolescents. As adolescents navigate the gradual transition from parental reliance to individual autonomy, they increasingly turn to their peers as trusted confidants. Although self-disclosure has been associated with positive psychological outcomes among adolescents, research remains limited, inconsistent, and outdated, particularly in the Majority World. METHODS: This study examined the direct relations between self-disclosure and psychological well-being among 192 Chinese adolescents an

Frequency and Timing of Depression and Anxiety Diagnoses Following Cancer Diagnosis: A Multi-Center Cohort Study.

BACKGROUND: Depression and anxiety are common among individuals with cancer and are associated with adverse outcomes; however, the frequency, timing, and disparities of anxiety and depression diagnoses following cancer diagnosis remain incompletely characterized. METHODS: We conducted a retrospective, multi-center cohort study of 1,161,370 adults diagnosed with one of 10 common cancers between 2015 and 2025 within the University of California Health system using electronic health record data. Patients with depression or anxiety diagnoses in the year prior to cancer diagnosis were excluded. In

Defense Mechanisms of Pregnant Mothers Predict Attachment Security, Social-Emotional Competence, and Behavior Problems in Their Toddlers.

OBJECTIVE: For at-risk (single parent, low income, low support) mothers, healthy adaptation and the ability to manage stress have clear implications for parenting and the social-emotional well-being of their young offspring. The purpose of this longitudinal study was to examine associations between defense mechanisms in pregnant women and their toddlers' attachment security, social-emotional, and behavioral adjustment. METHOD: Participants were 84 pregnant women during their last trimester of pregnancy, recruited from community agencies primarily serving low-income families. Women were follow

Lived experiences of self-care among older physically active urban-living individuals.

BACKGROUND: Promoting physical activity is a public health priority in most industrial countries, and physical function is an important factor when taking into consideration older people's self-care and health. Despite the increasing challenges associated with urbanization and the aging population, urban life appears to be positive in many ways for urban dwellers. However, the manner in which older people live in urban settings and how this influences their ability to take care of themselves should be considered important knowledge for health professionals and politicians to acquire. The aim o

Prospective study of central nervous system function in amateur boxers in the United States.

Active amateur boxers from six US cities were studied in 1986-1990 to determine whether changes in central nervous system function over a 2-year interval (as evaluated by tests of perceptual/motor function, attention/concentration, psychomotor speed, memory, visuoconstructional ability, and mental control, measures of ataxia and brain-stem auditory evoked potentials, and electroencephalography) were associated with degree of participation in amateur boxing. A total of 484 participants were examined at baseline; 393 (81.2%) were examined 2 years later. At baseline, 22% of the participants had n

Evolutionary medicine and psychiatry in medical education: a scoping review.

BACKGROUND: Evolutionary medicine (EvMed) and evolutionary psychiatry (EvPsych) apply principles of evolutionary biology to deepen understanding of disease vulnerability, mental health, and clinical reasoning. Despite growing conceptual maturity and relevance across biomedical domains, the extent and nature of their integration into medical and psychiatric education remain unclear. OBJECTIVE: This scoping review aims to map the education-focused literature on EvMed and EvPsych across undergraduate, postgraduate, and continuing professional training contexts, and to characterize the types of e

Motivational climate in school sport and adolescent mental health: a systematic review.

BACKGROUND: Motivational climate in school-based physical education and sport represents a potentially modifiable context for adolescent mental health; however, its associations with psychological outcomes have not been systematically synthesised. This review aimed to synthesise evidence on perceived motivational climate and adolescent mental health outcomes in school settings. METHODS: Databases were searched from the inception to May 2026 following PRISMA guidelines. Inclusion criteria for studies included sampling of adolescents (10-19 years of age), assessment of motivational climate in s

NoiBene, a Group Intervention for Promoting Mental Health Among University Students: A Study Protocol for a Randomized Controlled Trial.

University students' mental health has become a public health issue since increasingly students report high levels of psychological distress. Mental health difficulties influence students' lives, such as academic performance, relationships satisfaction, and quality of life. Moreover, different kinds of obstacles often hinder help-seeking behavior. Such evidence strongly suggests the need to implement prevention and promotion strategies to increase health and well-being in educational contexts. This article presents a study protocol for implementing and evaluating NoiBene, an evidence-based gro
