Onset of multiple sclerosis (MS) in childhood and adolescence has now been well-recognized over the past 10-15 years. There have been significant advances in the understanding of the disease course of pediatric MS, which is highly inflammatory, with a higher relapse rate than adult MS. High-efficacy, disease-modifying treatments are recommended for the management of pediatric MS, and these treatments have been studied in open-label and retrospective studies, several of which are outlined here. Fingolimod (Gilenya) is approved for pediatric MS in the United States and many world regions. Severa…

BACKGROUND: Aggressive multiple sclerosis (MS) causes early, rapid accumulation of disability with frequent and severe relapses, incomplete recovery from relapses, and/or high T2 lesion burden. Severe relapses are typically treated with high-dose IV corticosteroids followed by plasmapheresis. Patients who do not respond to initial treatment lack universally accepted second-line options. Cyclophosphamide is used in clinical practice for MS patients that do not respond to initial relapse therapy. Despite its use, there is no conclusive data demonstrating its efficacy.
OBJECTIVE: To review the U…

Multiple sclerosis (MS) is a chronic, inflammatory disorder of the central nervous system (CNS) characterized by overlapping relapsing and progressive pathologies. Although they exist along a continuum, pathology in relapsing MS (RMS) is primarily driven by the peripheral adaptive immune system, while progressive MS (PMS) pathology is underpinned by innate immune activity and other pathologic processes compartmentalized within the CNS. While currently approved therapies are highly effective in targeting aspects underpinning relapsing pathology, they have limited efficacy in PMS due to either i…

Multiple sclerosis (MS) typically presents in early to middle adulthood, but owing to advancements in health care, many individuals with MS now live a normal lifespan, and approximately half of the people currently living with MS are ≥50 years of age. As people living with MS age, their diagnosis, treatment and disease management become more complex owing to…

Multiple sclerosis (MS) affects 2.9 million people. Traditional classification of MS into distinct subtypes poorly reflects its pathobiology and has limited value for prognosticating disease evolution and treatment response, thereby hampering drug discovery. Here we report a data-driven classification of MS disease evolution by analyzing a large clinical trial database (approximately 8,000 patients, 118,000 patient visits and more than 35,000 magnetic resonance imaging scans) using probabilistic machine learning. Four dimensions define MS disease states: physical disability, brain damage, rela…

BACKGROUND: Previous studies on Benign Multiple Sclerosis (BMS) relied mostly on the Expanded Disability Status Scale (EDSS) for classification. In addition, the underlying mechanisms of BMS are not yet fully understood. Therefore, we analyzed the frequency of BMS when parameters beyond EDSS are included, and BMS neuroimaging correlates.
METHODS: BMS (EDSS ≤ 3.0 after 10 years of onset) and traditional MS (EDSS > 3.0 after the same period) individuals underwent assessment of physical disability, fatigue, depression, and cognition. BMS were further classified according to a stringent EDSS-base…
