The effects of bladder outflow obstruction on the innervation of the bladder were studied using a rabbit animal model. Partial occlusion of the bladder neck was obtained by the placement of a silk ligature at that level; control animals underwent a sham procedure. After a 3-month period, the presence of outflow tract obstruction was confirmed using urodynamic studies. We report a case of congenital hypomyelination neuropathy presenting at birth. The infant had generalized hypotonia and weakness. There was decreased respiratory effort along with a right phrenic nerve and left vocal cord paralyses. Tongue fasciculations were present. The procedure known as rhizolysis has been claimed to denervate the lumbar zygapophyseal joints. Analysis of the surgical anatomy of the incisions used in the procedure reveals that theoretically they do not interrupt the nerve supply to the zygapophyseal joints. The procedure was performed at six levels in each of three cadavers. Subsequent dissection revealed that at no level was the nerve supply to the zygapophyseal joints interrupted. The writers conclude that denervation of the Zygapophyseal joints cannot be held to be the mechanism of any pain relief that rhizolysis may achieve.

Spastic hypertonus (muscle over-activity) often develops after spinal cord injury or stroke. Chemodenervating agents such as Botulinum toxin A (BtA) and phenol are often used to treat this condition. The extensor digitorum brevis and abductor digiti minimi muscles were examined bilaterally with electromyography in 53 healthy subjects. In 72% of the subjects either fibrillation potentials, positive sharp waves or fasciculation was seen in at least one muscle examined. These slight, usually symmetric neuropathic signs are believed to be associated with normal aging and to some extent also with external trauma to the nerves and muscles in the distal parts of the foot.
