B Cell Tolerance and BCR Signaling Dysregulation in NF155-Mediated Autoimmune Nodopathies.

OBJECTIVE: Autoimmune nodopathies (AINs) are a group of rare, acquired autoimmune neuropathies with distinct clinical features and the presence of circulating autoantibodies - often of the immunoglobulin G4 (IgG4) subclass - targeting proteins at the node of Ranvier. Defects in B cell tolerance checkpoints have been implicated in several autoimmune diseases. Prior work identified defective B cell tolerance-reflected by a high frequency of self-reactive naïve B cells-in patients with MuSK-positive myasthenia gravis (MG), mediated by IgG4 autoantibodies. Here, we investigated whether tolerance d…

Partial Response to Nivolumab and Ipilimumab in a Patient With Malignant Pleural Mesothelioma and Pre-Existing Myasthenia Gravis Without Severe Flares or Immune-Related Adverse Events: A Case Report.

Detailed clinical data on the combination immune checkpoint inhibitor (ICI) therapy in patients with myasthenia gravis (MG) remain limited. We report a case of malignant pleural mesothelioma with previously undiagnosed ocular MG. Owing to hepatic dysfunction, reduced doses of nivolumab and ipilimumab were administered before confirmation of anti-acetylcholine receptor (AChR) antibody positivity. MG was diagnosed based on subtle ocular symptoms and serological tests. Prophylactic intravenous immunoglobulin and an acetylcholinesterase inhibitor were administered; combination therapy was disconti…

Acquired Myasthenia Gravis in a Juvenile Australian Shepherd Dog and Efficacy of Leflunomide Treatment.

Myasthenia gravis is a disorder of neuromuscular transmission characterized by muscle weakness and exercise intolerance that is rarely diagnosed in dogs younger than 6 months. We report a case of juvenile-onset myasthenia gravis in a 5-month-old, intact male Australian Shepherd with onset of clinical signs at 4 months of age. The dog presented with a 4-week history of progressive weakness and regurgitation. Diagnosis was confirmed by an elevated serum acetylcholine receptor antibody concentration. Treatment with pyridostigmine provided only partial improvement. Leflunomide was added to achieve…

Safety and effect of topical neostigmine ophthalmic solution in animal models.

The study aimed to evaluate the safety and effect of various dosages of topical neostigmine ophthalmic solution (TNOS) in animal models and establish a recommended dosage for further studies on myasthenia gravis (MG) diagnosis. A placebo-controlled, sequential ascending dose study was conducted in healthy rabbits. Eighteen eyes were randomized to receive one of three concentrations (1.0, 1.5, and 2.5 mg/mL) of TNOS. Pupillary sizes over time were compared with control group that received normal saline solution (NSS). Mean pupillary size of the 1.5 mg/mL group was significantly smaller than con…

Machine Learning in Myasthenia Gravis: A Systematic Review of Prognostic Models and AI-Assisted Clinical Assessments.

Background: Myasthenia gravis (MG), a chronic autoimmune disorder with variable disease trajectories, presents considerable challenges for clinical stratification and acute care management. This systematic review evaluated machine learning models developed for prognostic assessment in patients with MG. Methods: Following PRISMA guidelines, we systematically searched PubMed, Embase, and Scopus for relevant articles published from January 2010 to May 2025. Studies using machine learning techniques to predict MG-related outcomes based on structured or semi-structured clinical variables were inclu…

Acupuncture Treatment for Individuals With Myasthenia Gravis - A Pilot Randomized Clinical Trial.

BACKGROUND: Myasthenia gravis (MG) is a rare autoimmune disease resulting in muscle weakness. Although pharmacologic treatments are typically effective at managing symptoms, prolonged use is associated with safety issues. Acupuncture treatment may have a significant positive effect in treating MG symptoms and may enhance the efficacy of medications. OBJECTIVE: The overall goal was to examine the feasibility and safety of a pilot randomized controlled trial of acupuncture treatment in individuals with MG. METHODS: Participants were randomized into 2 arms: (1) immediate start and (2) delayed s…

Congenital Myasthenic Syndrome in Pregnancy: A Case Report and Literature Review on Peripartum Management Strategy.

Congenital myasthenic syndromes (CMS) are a group of rare inherited neuromuscular disorders caused by defects in neuromuscular transmission. Unlike autoimmune myasthenia gravis, CMS typically presents in childhood with variable severity and symptoms, and does not involve autoantibodies. This report presents the management of a primigravida diagnosed with CMS who was generally stable on pyridostigmine therapy but experienced a severe exacerbation during pregnancy. The diagnosis of CMS was made on clinical grounds, but the genetic testing for confirmation of the exact mutation was not available.…

Myasthenia gravis (MG) is an autoimmune condition affecting the neuromuscular junction. Juvenile MG (JMG) is a subtype of pediatric MG and is reported to be a rare neurological disorder with variable presentation and difficult diagnosis. MG has been described as a great neurological mimicker having resemblance with a wide spectrum of neurological disorders. The present case is a distinctive and compelling example of JMG that manifested at a young age with a disguised presentation. Bilateral ptosis was the principal presenting symptom with mild facial droop which simulated Bell's palsy initiall…
