Muscle phosphofructokinase deficiency in two generations.

Phosphofructokinase (PFK) is the key regulatory enzyme of glycolysis. Patients lacking the muscular isoform of PFK typically present with myopathy and compensated hemolysis (glycogenosis type VII or Tarui's disease). Since 1965 about 30 cases of muscular PFK deficiency have been reported. In most cases family history suggests a recessive inherited trait. We describe a family of Ashkenazi Jewish origin with two members in subsequent generations suffering from muscular PFK deficiency. The propositus, a 19-year-old male patient presented with weakness, myalgias and exercise intolerance since earl…

New-onset panic, depression with suicidal thoughts, and somatic symptoms in a patient with a history of lyme disease.

Lyme Disease, or Lyme Borreliosis, caused by Borrelia burgdorferi and spread by ticks, is mainly known to cause arthritis and neurological disorders but can also cause psychiatric symptoms such as depression and anxiety. We present a case of a 37-year-old man with no known psychiatric history who developed panic attacks, severe depressive symptoms and suicidal ideation, and neuromuscular complaints including back spasms, joint pain, myalgias, and neuropathic pain. These symptoms began 2 years after being successfully treated for a positive Lyme test after receiving a tick bite. During inpatien…

Mixed cryoglobulinemia and secondary membranoproliferative glomerulonephritis associated with ehrlichiosis.

Ehrlichiosis is a tick-borne disease with diverse clinical presentations, ranging in severity from a flu-like illness with fever and myalgias to a serious systemic disease with multisystem organ failure. Nephrotic syndrome has been reported previously in two cases of human ehrlichiosis. A kidney biopsy revealed minimal change disease in one of those patients. Herein, we present the case of a 40-year-old man with ehrlichiosis who developed nephrotic syndrome, cryoglobulinemia, and secondary membranoproliferative glomerulonephritis (MPGN). The patient originally presented with shortness of breat…

99mTechnetium pyrophosphate scintigraphy in the detection of skeletal muscle disease.

We aimed to assess the specificity and sensitivity of (99m)technetium pyrophosphate muscle scintigraphy in the diagnostic workup of patients with suspected myopathy. We reviewed the charts of 166 patients; 52% of the subjects had myalgias, 36% had muscle weakness, 45% had an elevated serum creatine kinase (CK), and 49% had an increased C reactive protein (CRP). Scintigraphy was positive in 34 patients (20%). The test was more sensitive in the presence of muscle weakness, elevated CK, or increased CRP. The presence of myalgias did not influence the odds. Sensitivity was 60% in patients with the…

Prospective rheumatological study of muscle and joint symptoms during Trichinella nelsoni infection.

One hundred and fifty patients were assessed during an outbreak of Trichinella infection in Northern Italy. The aetiological agent was proved to be T. nelsoni. A detailed prospective rheumatological study was carried out. Myalgia was observed in 88 per cent, muscle weakness in 60 per cent, myositis in 62 per cent and arthralgia in 20 per cent of the patients. The arthralgia score was strictly related to myalgias-myositis. The degree of myositis correlated with the degree of hypereosinophilia. Muscle damage in tissue samples was often observed in areas where eosinophils were the major cell popu…

LOCALIZED MYOSITIS AND TRANSIENT ENCEPHALOPATHY AS PRESENTING SYMPTOMS IN MURINE TYPHUS.

Murine typhus in the pediatric population has increased substantially in recent years. The most common clinical presentation of murine typhus includes fever, rash, headaches and myalgias. Murine typhus presenting with predominant myositis and/or encephalopathy is rare. It is important to recognize unusual clinical manifestations of murine typhus in children for early diagnosis and treatment.

My back hurts! Did you have COVID-19 infection?

Pain may be an early symptom of COVID-19 infection, most commonly seen as myalgia and headache. However, atypical presentations such as abdominal pain and leg pain can also be observed. We present seven cases of COVID-19 treated for pain. Our aim is to draw attention to low back, leg, and back pains that develop after COVID-19 infection.

[Acute polyarthritis after a stay in the Caribbean].

A 59-year-old female patient presented with acute polyarthritis after a holiday in the Caribbean. In addition, constitutional symptoms as well as myalgia and arthralgia were reported. Imaging demonstrated synovitis of the wrist and fingers without erosive changes. Immunoserological findings were normal with no evidence of autoimmune disease or vasculitis. Further evaluation revealed serological evidence of chikungunya virus infection.

Acute kidney injury associated with rhabdomyolysis in a patient with COVID-19.

Rhabdomyolysis is defined as the breakdown of skeletal muscle leading to the release of muscle contents into the extracellular fluid. Patients with rhabdomyolysis can be asymptomatic or have myalgia symptoms, weakness, myoglobinuria with dark urine, significant electrolyte imbalance, and acute kidney injury. Here we describe a case on acute kidney injury associated to rhabdomyolysis in a patient with COVID-19.

Plasmodium vivax exflagellated microgametes in human peripheral blood: a potential diagnostic dilemma.

Both malaria and relapsing fever Borrelia are infectious diseases characterized by fever, headache, myalgia, hepatosplenomegaly and tendency to relapse. Exflagellation of microgametocyte in malarial parasites is seen only in the definitive host, i.e., mosquitoes. Here we report an unusual case of a 23-year-old man who presented Plasmodium vivax infection with multiple exflagellated microgametes in the peripheral blood smear.

Recurrent rhabdomyolysis caused by carnitine palmitoyltransferase II deficiency, common but under-recognised: Lessons to be learnt.

We discuss two adult siblings who presented with symptoms of myalgia and rhabdomyolysis following exercise with myoglobinuria; genetic testing confirmed carnitine palmitoyltransferase II deficiency and resulted in institution of appropriate crisis management and dietary advice. We explore the phenotypic variability of this commonest fatty oxidation defect that remains under-diagnosed in the adult population and provide clues for early recognition and diagnosis.

[Delayed diagnosis of Trichinelliasis focused in the Poznan Province].

The study compares chronologically the weeks in which Trichinella spiralis infected meat was consumed. The first symptoms appeared and clinical diagnosis of trichinellosis was made in 37 patients. Several factors were responsible for the delay in final diagnosis of trichinellosis: widely distributed infected meat, contemporary common respiratory viral infections in population with fever and myalgia, diagnostic difficulties in the first index case.
