Short- and Long-Term Neurodevelopmental Outcomes of Very Preterm Infants with Neonatal Sepsis: A Systematic Review and Meta-Analysis.

Sepsis is commonly experienced by infants born very preterm (<32 weeks gestational age and/or <1500 g birthweight), but the long-term functional outcomes are unclear. The objective of this systematic review was to identify observational studies comparing neurodevelopmental outcomes in very preterm infants who had blood culture-proven neonatal sepsis with those without sepsis. Twenty-four studies were identified, of which 19 used prespecified definitions of neurodevelopmental impairment and five reported neurodevelopmental outcomes as continuous variables. Meta-analysis was conducted using 14 s…

Study of Risk Factors, Causative Organisms & Their Sensitivity Pattern in Neonatal Sepsis in a Community Based Tertiary Level Hospital.

Neonatal sepsis is one of the most common reasons for admission to neonatal units in developing countries. It is also a major cause of mortality in both developed and developing countries. The type and pattern of organisms that cause neonatal sepsis changes over time and vary from one hospital to another hospital, even in the same country. In addition the causative organisms have developed increased drug resistance for the last two decades. Maternal, neonatal and environmental risk factors have contributed for the development of sepsis. To study the risk factors, causative organism and bacteri…

Clinical features and prognostic factors of early-onset sepsis: a 7.5-year experience in one neonatal intensive care unit.

PURPOSE: In this study, we investigated the clinical features and prognostic factors of early-onset sepsis (EOS) in neonatal intensive care unit (NICU) patients. METHODS: A retrospective analysis was conducted on medical records from January 2010 to June 2017 (7.5 years) of a university hospital NICU. RESULTS: There were 45 cases of EOS (1.2%) in 3,862 infants. The most common pathogen responsible for EOS was group B Streptococcus (GBS), implicated in 10 cases (22.2%), followed by Escherichia coli, implicated in 9 cases (20%). The frequency of gram-positive sepsis was higher in term than in

Risk factors and clinical outcomes for carbapenem-resistant Gram-negative late-onset sepsis in a neonatal intensive care unit.

BACKGROUND: Carbapenem-resistant (CR), Gram-negative (GN), late-onset sepsis (LOS) is a serious threat in the neonatal intensive care unit (NICU). AIM: To assess the prevalence of CR-GN-LOS in NICU patients and to identify the risk factors and outcomes associated with its acquisition. METHODS: Neonates with carbapenem-susceptible (CS)-GN-LOS were compared with those with CR-GN-LOS in a two-year observational study. FINDINGS: A total of 158 patients had GN-LOS; 100 infants had CS-GN-LOS and 58 infants had CR-GN-LOS. The incidence rate of CR-GN-LOS was 6.5 cases per 1000 patient-days. The mos

Risk factors and prognosis for neonatal sepsis in southeastern Mexico: analysis of a four-year historic cohort follow-up.

BACKGROUND: Neonatal sepsis is a worldwide public health issue in which, depending on the studied population, marked variations concerning its risk and prognostic factors have been reported. The aim of this study was to assess risk and prognostic factors for neonatal sepsis prevailing at a medical unit in southeastern Mexico. Thus, we used a historic cohort design to assess the association between a series of neonates and their mothers, in addition to hospital evolution features and the risk and prognosis of neonatal sepsis (defined by Pediatric Sepsis Consensus [PSC] criteria) in 11,790 newbo…

Neonatal early onset Escherichia coli sepsis: trends in incidence and antimicrobial resistance in the era of intrapartum antimicrobial prophylaxis.

BACKGROUND: Although intrapartum antimicrobial prophylaxis has lowered the incidence of early onset group B Streptococcus (GBS) sepsis, there are concerns that the increased use of antibiotics may raise the incidence of non-GBS antimicrobial-resistant infections. The objective of this study was to determine trends in the incidence and antimicrobial resistance of early onset sepsis caused by Escherichia coli in the era of antimicrobial prophylaxis. METHODS: All neonates with early onset E. coli infection who were born at La Paz Hospital, Madrid, from January 1, 1992, through December 31, 2002,

Maternal colonisation with group B streptococcus and effectiveness of a culture-based protocol to prevent early-onset neonatal sepsis.

This study was conducted to find out the group B streptococcus colonisation of pregnant women in Kocaeli, Turkey. A culture plus individualised high-risk-based antibiotic prophylaxis was compared with high-risk-based approach alone. The screening of women was performed via vaginal and anal cultures for group B streptococcus (GBS). The maternal GBS colonisation rate was found to be 6.5%. All colonised women or preterm labours with unavailable culture results until delivery received prophylactic antibiotics. Neonatal colonisation rate and early-onset neonatal sepsis due to GBS was 1/200. The uns…

Different cytokine expression in cord blood mononuclear cells after stimulation with neonatal sepsis or colonizing strains of Streptococcus agalactiae.

Streptococcus agalactiae (group B streptococcus, GBS) is the major pathogen of neonatal sepsis. In some newborns, GBS sepsis may have a severe course, including septic shock with a high mortality rate, whereas other newborns are colonized with GBS on their surfaces without any clinical signs of bacterial infection. The reason for this discrepancy is far from clear. We sought, in this study, to compare cytokine expression in cord blood mononuclear cells after stimulation with GBS strains isolated from newborns with sepsis, and strains isolated from newborns without any symptoms of invasive infe…

Unusual occurrence of an epidemic of type Ib/c group B streptococcal sepsis in a neonatal intensive care unit.

An epidemic of late-onset sepsis due to type Ib/c group B Streptococcus (Ib/c-GBS) occurred in a neonatal intensive care unit (NICU). During a seven-week period, five very low birth weight infants (index cases [ICs]) more than four weeks of age became bacteremic. Bacteriologic surveillance of neonates revealed persistent colonization in three ICs and identified three asymptomatic carriers (ACs). All ICs and one AC acquired Ib/c-GBS nosocomially, whereas the other two ACs were colonized at birth. Among nursery personnel, 39% carried GBS, but only two harbored Ib/c-GBS. Although phage typing of

Nomogram to predict risk of neonatal mortality among preterm neonates admitted with sepsis at University of Gondar Comprehensive Specialized Hospital: risk prediction model development and validation.

BACKGROUND: Mortality in premature neonates is a global public health problem. In developing countries, nearly 50% of preterm births ends with death. Sepsis is one of the major causes of death in preterm neonates. Risk prediction model for mortality in preterm septic neonates helps for directing the decision making process made by clinicians. OBJECTIVE: We aimed to develop and validate nomogram for the prediction of neonatal mortality. Nomograms are tools which assist the clinical decision making process through early estimation of risks prompting early interventions. METHODS: A three year r

Systemic immune inflammation index and neutrophil-to-lymphocyte ratio as predictors of neonatal sepsis in low birth weight infants.

BACKGROUND: The systemic immune inflammation index (SII) and neutrophil-to-lymphocyte ratio (NLR) are hematological parameters calculable from standard laboratory assessment. Despite extensive adult medicine implementation, neonatal applicability remains incompletely characterized. Early identification of sepsis in neonates remains challenging and sepsis is a significant global health problem associated with high morbidity and mortality, particularly among vulnerable low birth weight (LBW) infants. This study aimed to evaluate the predictive value of SII and NLR for neonatal sepsis in LBW infa…

Diagnostic utility of neutrophil CD64 for early detection of neonatal sepsis in southwestern Nigeria: a comparative case-control study.

BACKGROUND: Neonatal sepsis remains a major global health challenge due to its contribution to infant morbidity and mortality. Early diagnosis is often difficult because the clinical presentation is nonspecific. Despite extensive research, no biomarker has yet achieved the required sensitivity, specificity and minimal blood volume to avoid hospital-acquired anaemia in newborns. Identifying a reliable early marker could improve therapeutic decision making and reduce unnecessary antibiotics use in neonates with suspected infection. METHODOLOGY: We conducted a hospital-based comparative case-con…

Investigating neonatal sepsis: anti-Infectives, diagnostics and Guidelines used in Health sysTems across sub-Saharan Africa - The INSIGHTS study.

BACKGROUND: Sepsis is a leading cause of neonatal mortality in sub-Saharan Africa (SSA), where microbiological diagnostic capacity and antibiotic access are limited. High antimicrobial resistance (AMR) rates limit the effectiveness of current treatment guidelines, with concern that available antibiotics are rarely adequate treatment for neonatal sepsis in the region. METHODS: A cross-sectional online survey was electronically distributed in English, French and Portuguese to neonatal clinicians across SSA between April and June 2025. Questions focused on the management of neonatal sepsis inclu…

Genome admixture analysis of 1,030 Ugandan infants with neonatal sepsis and hydrocephalus demonstrates geographical stratification of population disease risk.

BACKGROUND: Neonatal disorders such as post-infectious hydrocephalus exhibit a higher incidence in Africa, where the intricate relationships between genetic ancestry, environmental exposures, and other risk factors likely contribute to the increased incidence. METHODS: To start to characterize the common genetic architecture of Ugandan infants, we analyzed genome sequencing data from 1,030 Ugandan infants recruited from studies targeting neonatal sepsis and hydrocephalus. We employed genetic admixture analysis and integrated geospatial data to examine the relationships between genetic backgro…

Vaccines preventing neonatal sepsis: focus on Group B Streptococcus, Klebsiella pneumoniae and Escherichia coli to combat rising antimicrobial resistance.

INTRODUCTION: Neonatal sepsis, a systemic infection occurring in infants within their first 28 days of life, is a leading cause of mortality globally. The burden is especially severe in low- and middle-income countries (LMICs), where incidence and mortality rates are higher than in high-resource settings. Antimicrobial resistance, driven by multidrug-resistant pathogens prevalent in LMICs, further complicates its effective management. Vaccination offers a promising strategy to prevent infections caused by common neonatal sepsis pathogens, potentially reducing sepsis incidence and mitigating re…
