Scanning and transmission EM of rat kidney following low dose mercuric chloride administration.

The development of necrosis and the subsequent regeneration of the proximal tubular epithelium located in the cortex and the outer medulla of the rat kidney following subcutaneous injection of 2 mg of mercuric chloride per kilogram of body weight was observed utilizing light micromicroscopy (LM), transmission electron microscopy (TEM), and scanning electron microscopy (SEM). The cortical proximal tubules were largely free of injury. In the medullary proximal tubules, the residual, undamaged cells underwent a change in phenotype appearance involving loss of the microvillus border, a decrease in

Importance of Hypomagnesemia in Primary Hyperparathyroidism: A Turkish Nationwide Retrospective Cohort Study.

Patients with primary hyperparathyroidism (PHPT) are predisposed to hypomagnesemia as well as hypophosphatemia. In the current literature, scarce data was available on the clinical significance of hypomagnesemia in PHPT. The present study aimed to investigate the prevalence of hypomagnesemia and its association with complications of PHPT in a large nationwide cohort. A nationwide population-based retrospective study was conducted using anonymized data from the Turkish Ministry of Health National Electronic Database (E-nabız). The International Statistical Classification of Diseases and Related

Insights into the pathological mechanisms of p85α mutations using a yeast-based phosphatidylinositol 3-kinase model.

In higher eukaryotes, cell proliferation is regulated by class I phosphatidylinositol 3-kinase (PI3K), which transduces stimuli received from neighboring receptors by local generation of PtdIns(3,4,5)P3 in cellular membranes. PI3K is a heterodimeric protein consisting of a regulatory and a catalytic subunit (p85 and p110 respectively). Heterologous expression of p110α in Saccharomyces cerevisiae leads to toxicity by conversion of essential PtdIns(4,5)P2 into futile PtdIns(3,4,5)P3, providing a humanized yeast model for functional studies on this pathway. Here, we report expression and function

Six cases with severe insulin resistance (SIR) associated with mutations of insulin receptor: Is a Bartter-like syndrome a feature of congenital SIR?

Biallelic insulin receptor (INSR) gene mutations cause congenital syndromes of severe insulin resistance (SIR) known as Donohue syndrome (DS … agnosed with RMS (patients 3 and 4). The fifth patient with congenital SIR died when 14 months old. Nephrocalcinosis, hyperaldosteronism, hyperreninemia, and hypokalemia, in the absence of hypertension, were discovered in patients 3 and 5 when 24 and 4 months old, respectively. Patient 3, now 3 years/3 months old, still shows hyperreninemic hyperaldosteronism requ …

[Primary hyperparathyroidism in children].

Primary hyperparathyroidism in children is an extremely rare condition. Slightly more than a hundred cases have been published in children or adolescents aged under 16. Forms with a neonatal presentation, the most rare, should be viewed apart since they rapidly become life-threatening. They involve hyperplasia of the chief cells of the parathyroid glands. Treatment is always surgical and should be rapid, consisting of total parathyroidectomy with autotransplantation. Primary hyperparathyroidism in older children more closely resembles that seen in adults. Sporadic forms are most often due to a

A Case Report of Familial Primary Hyperoxaluria Type 1 Nephropathy with c.781C>G Gene Mutation and Literature Review.

OBJECTIVE: Primary hyperoxaluria type 1 (PH1), the most severe form of autosomal recessive hyperoxaluria, is caused by AGXT gene mutations t … enotypic heterogeneity, ranging from asymptomatic cases to recurrent or occasional stone formation, nephrocalcinosis, and ESRD, often leading to missed or delayed diagnosis. Genetic testing is the gold standard. Early diagnosis allows for interventions such as hydration, pyridoxine therapy, RNAi treatments, or liver transplantation to slow renal function decline, …

Etiology and outcomes of primary renal tubular acidosis.

BACKGROUND: This study investigates the etiology, outcomes, and genotype-phenotype correlations in patients with renal tubular acidosis (RTA … uent in patients with biallelic compared to heterozygous SLC4A1 variants (50% vs. 12.5%; P = 0.18). Nephrocalcinosis and metabolic control were similarly prevalent across genetic categories of distal RTA. Sensorineural hearing loss was more common with ATP6V1B1 than with ATP6V0A4 variants (61.5% vs. 22.2%, P = 0.099) and did not vary by metabolic control. At media …

An interesting case of unintentional vitamin D toxicity in an infant due to erroneous supplement concentration: a case report.

UNLABELLED: Despite the rare occurrence of vitamin D toxicity in infants, increased use of vitamin D formulations as well as incorrect suppl … tion (3.7 pg/ml), which was the major concern to the clinicians. On ultrasonographical examination, nephrocalcinosis was observed. Further evaluation unveiled that the vitamin D supplement administered to the infant constituted a deucedly high dose of 42 000 IU instead of the recommended dose of 0.5 ml of 800 IU. CLINICAL DISCUSSION: The patient developed vitamin …

Biallelic CYP24A1 variants presenting during pregnancy: clinical and biochemical phenotypes.

INTRODUCTION: Inactivating mutations in CYP24A1, encoding vitamin D-24-hydroxylase, can lead to an accumulation of active vitamin D metabolites and consequent hypercalcaemia. Patient (infantile and adult) presentation is varied and includes mild-severe hypercalcaemia, hypercalciuria, nephrocalcinosis and nephrolithiasis. This study aimed to characterize the clinical and biochemical phenotypes of a family with two CYP24A1 missense variants. METHODS: The proband and seven family members underwent detailed clinical and biochemical evaluation. Laboratory measurements included serum calcium, intac…

Proteomic Analysis of Urinary Extracellular Vesicles Reveals a Role for the Complement System in Medullary Sponge Kidney Disease.

Medullary sponge kidney (MSK) disease is a rare and neglected kidney condition often associated with nephrocalcinosis/nephrolithiasis and cystic anomalies in the precalyceal ducts. Little is known about the pathogenesis of this disease, so we addressed the knowledge gap using a proteomics approach. The protein content of microvesicles/exosomes isolated from urine of 15 MSK and 15 idiopathic calcium nephrolithiasis (ICN) patients was investigated by mass spectrometry, followed by weighted gene coexpression network analysis, support vector machine (SVM) learning, and partial least squares discri…

Treatment of hypophosphataemic rickets in children remains a challenge.

INTRODUCTION: Hypophosphataemic rickets (HR) is a rare hereditary disease characterised by hypophosphataemia, defects in bone mineralisation … 7). Episodes of secondary hyperparathyroidism were seen in 87%, and one patient developed transient nephrocalcinosis. CONCLUSION: The current medical treatment for HR is insufficient. The rarity of the disease and the treatment difficulties of HR call for centralised management. International multi-centre trials including novel treatment options are warranted.
