Reversible conduction failure is distinct from neurophysiological patterns of recovery in mild demyelinating Guillain-Barré syndrome.

Axonal Guillain-Barré syndrome (GBS) can present with rapid resolution of nerve conduction failure also referred to as early reversible conduction failure. It is unclear if this reversible conduction failure represents successful immunotherapy or part of the natural evolution of mild axonal disease, or whether it is in fact recovery from mild demyelination. Here we describe two patients with axonal and demyelinating GBS with favorable outcomes without immunotherapy. One was a 32-year-old pregnant woman with axonal GBS. She presented with acute limb weakness, areflexia and mild sensory symptoms

Anti-glycolipid GM2-positive Guillain-Barre syndrome due to hepatitis E infection.

BACKGROUND: Hepatitis E infection is most prevalent in developing countries with poor sanitation, but can also occur apparently sporadically in more developed areas. CASE: We here report a second European case of Guillain-Barre syndrome due to hepatitis E infection in association with anti-glycolipid GM2 antibody. INTERPRETATION: This is likely to be a specific association involving molecular mimicry, and further European cases can therefore be expected.

Intraneural blood flow analysis during an intraoperative Phalen's test in carpal tunnel syndrome.

Phalen's test has been one of the most significant of clinical signs when making a clinical diagnosis of idiopathic carpal tunnel syndrome (CTS). However, it is unknown whether intraneural blood flow changes during Phalen's test in patients with CTS. In this study, an intraoperative Phalen's test was conducted in patients with CTS to observe the changes in intraneural blood flow using a laser Doppler flow meter. During Phalen's test, intraneural blood flow showed a sharp decrease, which lasted for 1 min. Intraneural blood flow decreased by 56.7%-100% (average, 78.0%) in the median nerve relati

Infantile Sandhoff's disease with peripheral neuropathy.

Sandhoff's disease is a rare autosomal-recessive disorder of sphingolipid metabolism that results from a deficiency of lysosomal enzyme beta-hexosaminidase A and B. The resultant accumulation of GM2 gangliosides within both grey matter and the myelin sheath of white matter results in essential, severe neurodegeneration. We describe a 14-month-old boy with seizures and severe neurodegeneration. His diagnosis was confirmed by neuroimaging and enzyme assay. In addition to the classic features of Sandhoff's disease, the child's clinical features were suggestive of neuropathy as supported by nerve

[Two patients with congenital insensitivity to pain with anhidrosis showing marked prolongation of central conduction time in short latency somatosensory evoked potential].

We examined the evoked potentials in 2 patients, a 6-month-old girl and a 3-year-old boy, with congenital insensitivity to pain with anhidrosis (CIPA). While auditory brainstem response (ABR) in both patients showed normal latencies, flash visual evoked potential (FVEP) revealed delayed latency of wave IV (P100), and short latency somatosensory evoked potential (SSEP) demonstrated marked prolongation of the central conduction time (CCT; N13-N20 interval). The boy had West syndrome and his prolonged CCT might have been influenced by abnormal cortical activities. The girl did not have epilepsy a

A practical electrophysiological guide for non-surgical and surgical treatment of carpal tunnel syndrome.

In the present study, we illustrate the use of an electrophysiological classification as a guide to the treatment of carpal tunnel syndrome (CTS). A total of 113 CTS patients were assessed with symptom severity scores, hand functional scores and electrophysiological studies. By setting criteria of electrophysiological tests, 179 hands in 113 patients were classified into mild, moderate and severe degrees of CTS. Of these, the 41 hands with severe CTS were referred for surgery. The 58 hands with moderate CTS and 80 hands with mild CTS received conservative treatment. The improvement ratios in t

HIV infection presenting with motor axonal variant of Guillain-Barré Syndrome.

OBJECTIVES: We present a case of acute motor axonal neuropathy in a patient with previously unrecognized human immunodeficiency virus (HIV) infection. METHODS: A 46-year-old male graduate student from Mali, Africa presented with a 3-week history of progressive weakness that began during a visit home from Utah. The symptoms stabilized by 6 weeks and gradually improved without treatment. RESULTS: Electrodiagnostic studies revealed widespread fibrillation potentials and positive sharp waves with normal sensory amplitudes and no demyelinating features. Cerebrospinal fluid pleocytosis and elevate

Clinical, electrophysiologic, and pathologic evidence for sensory abnormalities in ALS.

BACKGROUND: Amyotrophic lateral sclerosis (ALS) is a progressive degenerative disease of upper and lower motor neurons. Reports of the nature and frequency of sensory nerve involvement in ALS have varied. METHODS: We reviewed the Emory University motor neuron disease registry between 1997 and 2004 to identify 103 patients with ALS without coexisting diseases that might cause sensory abnormalities and for whom electrodiagnostic studies were available for review. Neurophysiologic studies were interpreted based on age-adjusted normative data from our laboratory. Twelve control biopsies were eval

Prognostic values of electrodiagnostic studies in traumatic radial neuropathy.

It is important to have strong predictors of outcome in traumatic neuropathies so that appropriate management can be instituted early. Our objective in this study was to evaluate the prognostic value of electrodiagnostic studies in traumatic radial neuropathy. In this retrospective study, 33 of 67 subjects with traumatic radial neuropathy met the inclusion criteria. Good outcome was defined as grade 3 or higher strength on the Medical Research Council scale in wrist extensors. Compound muscle action potential (CMAP) responses from extensor indicis proprius (EIP) predicted prognosis: 92% of sub

Fasciculation potentials in high-density surface EMG.

Fasciculation potentials (FPs) are observed in healthy individuals, but also in patients with neurogenic disorders. The exact site of origin and the clinical relevance in distinguishing, for example, amyotrophic lateral sclerosis (ALS) from other neurogenic diseases based on specific characteristics of the FPs is still a matter of debate and needs further exploration. This report describes the use of high-density surface EMG (HD-sEMG), with multiple electrodes in a compact grid to noninvasively record FPs. The technique provides both temporal and spatial information of fasciculations. Examples

Multifocal motor neuropathy with and without conduction block: a single entity?

OBJECTIVE: To assess if multifocal motor neuropathy (MMN) with and MMN without conduction block (CB) are similar or distinct diseases. METHODS: The authors reviewed the clinical features and responses to IV immunoglobulin (IVIg) treatment of patients with MMN with and without CB at diagnosis, after 4 years of follow-up and at the last examination. They included all patients showing clinical features of MMN who had been followed for at least 4 years: All had asymmetric purely motor weakness with a peripheral nerve distribution, without any sensory, bulbar, or respiratory signs and without any

Natural neural output that produces highly variable locomotory movements.

We recorded fast extensor tibiae motor neuron activity during single-legged treadmill walking in the stick insect, Carausius morosus. We used this activity to stimulate the extensor muscle motor nerve, observed the resulting extensor muscle contractions under isotonic conditions, and quantified these contractions with a variety of measures. Extensor contractions induced in this manner were highly variable, with contraction measures having SDs of 12 to 51%, and ranges of 82 to 275%, when expressed as percentages of the means, an unexpectedly wide range for a locomotory pattern. Effect of tension on force of contraction of muscle and nerve conduction velocity of the repaired nerve in a rat model.

To investigate the effect of tension on the contractive force of muscle and nerve conduction velocity of the repaired nerve, this study was designed. Fifty-four Sprague-Dawley (SD) rats were randomly divided into 3 groups. The left gastrocnemius muscles of the rats were dissected with the neurovascular pedicles intact; the tibial nerves were cut and immediately repaired by epineurial suture. Then the Achilles tendons were isolated and treated accordingly; the Achilles tendon was lengthened by 0.5 cm in lengthened group, shortened by 0.5 cm in shortened group and left alone in normal (control)

Sensory-motor index is useful parameter in electroneurographical diagnosis of carpal tunnel syndrome.

It was performed electroneurographic (ENG) studies with surface electrodes and examined nervus medianus (NM) in 60 patients (38 females), average age of 50,28 years (X+/-SD=50,28+/-11), with clinical diagnosis of carpal tunnel syndrome (CTS) and at least one border or discrete abnormal value of conventional electrophysiological tests. It was also examined 57 healthy individuals (33 females) as control group, average age of 45,65 years (X+/-SD=45,65+/-9,68). The sensitivity and specificity of sensory-motor index (SMI), terminal latency index (TLI) and residual latency (RL) were calculated and c

The effect of carpal tunnel release on median nerve flattening and nerve conduction.

Carpal tunnel syndrome (CTS) is the most common entrapment neuropathy and extensive surveys have been given on the time course of electrophysiological findings pre- and postoperatively. In patients with clinical and electrophysiological confirmed diagnosis of CTS surgical decompression of the carpal tunnel is a first line treatment and has proven to be successfull in 70 to 90% of all cases. The objective of this work was to study the morphological changes of the median nerve after endoscopic release of the carpal tunnel. We used high resolution ultrasound to quantify flattening of the median n
