[Idiopathic orthostatic hypotension (Shy-Drager syndrome). Results of regional cerebral blood flow measurements (author's transl)].

Using a gamma camera and the stochastic method, regional cerebral blood flow was measured in a patient with the diagnosis of idiopathic orthostatic hypotension. Blood pressure was altered by tilting the patient in supine position head-up and head-down. A partial defect in the autoregulation and CO2 sensitivity was found. Sensitivity to Norepinephrine was checked by intravenous infusion of 0.02 mug/min/kg body weight. An immediate marked rise in blood pressure was observed. Clinical symptoms, therapeutic guidelines, and histopathological findings are discussed.

Brain neoplasms with development similar to transient brain ischaemia.

Diagnostics of brain tumours still remains a serious problem in neurologist's career. Difficulties in proper diagnosing refer mainly to endocranial tumours with atypical clinical course. Frequency of brain tumours' vascular manifestation is assessed as 1-4%, and in most cases it is the kind of manifestation which corresponds with intracerebral bleedings and more seldom with ischaemic strokes. Reports on brain tumours with developments similar to transient brain ischaemia (1, 2, 3, 4) are published occasionally. In Neurology Clinic of the Medical Academy in Lublin, we conducted observation of t

Sciatic tension signs and lumbar disc herniation.

STUDY DESIGN: Sciatic stretch maneuvers designed to elicit signs of nerve root compressions secondary to lumbar disc herniation were evaluated in a prospective manner to correlate intraoperative evaluation of surgical pathology regarding level and anatomic location and proximity of the herniated disc to the nerve root. OBJECTIVES: To study the correlation between specific diagnostic maneuvers of sciatic stretch and anatomic location or level of lumbar disc pathology. SUMMARY OF BACKGROUND DATA: Maneuvers of sciatic tension, such as the straight leg lift, the well leg lift, Lasègue's sign, an

One-year cognitive and neurologic outcomes in survivors of paediatric extracorporeal cardiopulmonary resuscitation.

OBJECTIVE: To describe one-year cognitive and neurologic outcomes among extracorporeal cardiopulmonary resuscitation (ECPR) survivors enroll … rmance measures (Mullen Scales of Early Learning or Wechsler Abbreviated Scale of Intelligence) and neurologic examinations were obtained 12 months post-arrest. VABS-II and cognitive performance measures were transformed to standard scores (mean = 100, SD = 15) with higher scores representing better performance. Only children with broadly normal pre-arrest function (VAB …

Neurologic, neuropsychologic, and computed cranial tomography scan abnormalities in 2- to 10-year survivors of small-cell lung cancer.

In order to evaluate the relationship between neurologic function and cranial irradiation, 20 patients treated on National Cancer Institute … one were institutionalized. Fifteen patients (75%) had neurologic complaints, 13 (65%) had abnormal neurologic examinations, 12 (60%) had abnormal mental status examinations, 13 (65%) had abnormal neuropsychologic testing, and 15 (75%) had abnormal CCT scans. Compared with those given low-dose maintenance chemotherapy during PCI using 200 to 300 rad per fraction, patien …

Angiostrongylus cantonensis infection in captive Geoffroy's tamarins (Saguinus geoffroyi).

CASE DESCRIPTION: 3 Geoffroy's tamarins (Saguinus geoffroyi) in a zoo in east central Alabama developed neurologic signs shortly after a tamarin kept in the same enclosure was found dead. CLINICAL FINDINGS: Neurologic abnormalities varied among animals and were progressive. One female tamarin with a head tilt, nystagmus, mild ataxia, and paresis of a thoracic limb had gram-positive cocci present in an ear canal with otitis media and interna suspected. Another female with mild ataxia attributed to previous tail amputation developed seizures, and a male tamarin with tail tip trauma also develop

Linear scleroderma en coup de sabre with associated neurologic abnormalities.

Linear scleroderma represents a unique form of localized scleroderma that primarily affects the pediatric population, with 67% of patients diagnosed before 18 years of age. When linear scleroderma occurs on the head, it is referred to as linear scleroderma en coup de sabre, given the resemblance of the skin lesions to the stroke of a sabre. Here we describe 3 pediatric patients with linear scleroderma en coup de sabre who presented with neurologic abnormalities before or concurrent with the diagnosis of their skin disease. Our patients' cases highlight the underrecognized relationship between

[The diagnosis and forensic psychiatric expertise in infectious cerebral arachnoiditis].

Neurologic and psychiatric examination of 32 patients with infectious cerebral arachnoiditis was performed. Psychopathic-like syndrome and intellectual deficiency were diagnosed in those patients and the correlation between high intracranial pressure and severity of mental disorders was established. Characteristics of forensic psychiatric examination of this group of patients are presented.

Noninvasive bedside neuromonitoring in acute brain injury. A narrative review.

Clinical neurological examination remains the gold standard to detect, diagnose, and follow-up responses to treatment in acute neurological conditions in the critical care setting. However, in patients with severe neurological deficits at baseline or those requiring sedatives, detecting neurological deterioration can be challenging. In this scenario, noninvasive bedside neuromonitoring as a part of multimodal strategies can be useful in the avoidance of secondary brain injury and in the selection of which patient with acute brain injury would benefit from invasive neuromonitorization.

A New Neurological Screening Approach for Diagnosing Brainstem Infarction Using the Calling Method and Familiar Voices.

This report proposes a new approach to assess dysarthria in patients with brainstem infarction by involving familiar individuals. Collaboration provides valuable insights compared to subjective traditional methods. A man in his 70s presented with resolved positional vertigo. Standard neurological tests showed no abnormalities, and inquiries with the patient's friend did not reveal voice changes. While inquiring about voice changes with family, friends, and acquaintances is a common practice in clinical settings, our approach involved the patient calling out to his friend from a distance. [Finger drop variant of Guillain-Barré syndrome: a case report].

We report the case of a 31-year-old man with a finger drop variant of Guillain-Barré syndrome (GBS). The patient visited a neurological clinic with complaints of difficulty in extending the fingers, which occurred seven days after he had fever and diarrhea. The physician who first saw the patient suspected posterior interosseous nerve palsy and referred him to our hospital. Neurological examination 35 days after the onset revealed distal weakness of the upper extremities, particularly in the bilateral extensor digitorum (Medical Research Council [MRC] scale 1/1 [right/left]). The left triceps

Classification statistics of the Montreal Cognitive Assessment (MoCA): Are we interpreting the MoCA correctly?

OBJECTIVE: The Montreal Cognitive Assessment (MoCA) is a common cognitive screener for detecting mild cognitive impairment (MCI). However, previously suggested cutoff scores of 26/30 and above is often criticized and lacks racial diversity. The purpose of this study is to investigate the potential influence of race on MoCA classification cutoff score accuracy. METHOD: Data were obtained from the National Alzheimer's Coordinating Center (NACC) Uniform Data Set and yielded 4,758 total participants. Participants were predominately White (82.8%) and female (61.7%) with a mean age of 69.3 years (S
