Artificial Intelligence-Guided Artificial Nutrition in Critical Illness: Integrating Indirect Calorimetry and BIVA for Metabolic Precision.

Background: Critical illness is characterized by profound and rapidly evolving metabolic derangements driven by systemic inflammation, hyper … addressed indirect calorimetry, BIVA-derived parameters, or AI-based metabolic modeling applied to nutritional support. Given the heterogeneity of study designs and outcomes, findings were synthesized qualitatively. Results: Predictive equations showed substantial inaccuracy in unstable metabolic states, with errors frequently exceeding ±20-40%. Indirect calorimetry …

Effects of FSMP on nutrition status and sarcopenia among nutritional risk cancer patients: A randomized, double-blind, placebo-controlled study.

BACKGROUND AND OBJECTIVES: Cancer patients at nutritional risk have a worse prognosis, but this can be improved by nutritional support. Food for special medical purposes (FSMP), as a new form of nutritional support, needs to be further evaluated for its safety and efficacy in these patients. Our study elucidate the impact of FSMP on nutritional status and sarcopenia among nutritional risk cancer outpatients by assessing the NRS2002 score, exercise performance, muscle mass, and inflammatory factors pre- and postintervention. METHODS AND STUDY DESIGN: We conducted a single-centre, double-blind,…

Current practices in nutrition therapy in cardiac surgery patients: An international multicenter observational study.

BACKGROUND: Cardiac surgery patients with a prolonged stay in the intensive care unit (ICU) are at high risk for acquired malnutrition. Medical nutrition therapy practices for cardiac surgery patients are unknown. The objective of this study is to describe the current nutrition practices in critically ill cardiac surgery patients worldwide. METHODS: We conducted a prospective observational study in 13 international ICUs involving mechanically ventilated cardiac surgery patients with an ICU stay of at least 72 h. Collected data included the energy and protein prescription, type of and time to

Review of evolution and current status of protein requirements and provision in acute illness and critical care.

Nutrition therapy, by enteral, parenteral, or both routes combined, is a key component of the management of critically ill, surgical, burns, and oncology patients. Established evidence indicates overfeeding (provision of excessive calories) results in increased risk of infection, morbidity, and mortality. This has led to the practice of "permissive underfeeding" of calories; however, this can often lead to inadequate provision of guideline-recommended protein intakes. Acutely ill patients requiring nutritional therapy have high protein requirements, and studies demonstrate that provision of ad

Management of gastroparesis-associated malnutrition.

Gastroparesis (GP) is a chronic debilitating dysmotility characterized by unrelenting nausea, vomiting, bloating, early satiety, postprandial fullness and abdominal pain. Patients with GP experience other associated conditions, including gastroesophageal reflux disease, gastric bezoars and small bowel bacterial overgrowth. Furthermore, GP is associated with poor quality of life, increased emergency room visits, hospitalizations and subsequent increased healthcare costs. Currently, the managements of GP consist of glycemic control, antiemetics, prokinetics and the use of gastric electrical stim

[What is the benefit of the new European nutritional guidelines for dialysis?].

Nutritional guidelines are scarce for patients receiving maintenance hemodialysis. In this review, we address the recent European guidelines released in 2007. They include diagnosis and follow-up criteria for malnutrition to be performed routinely. They provide new values for protein and energy requirements, based on physical activity. Vitamins and trace elements requirements are proposed. The renutrition therapies are addressed, i.e., oral supplement, enteral and intradialytic parenteral nutrition. Finally, androgens, quotidian dialysis and acidosis treatment are also considered.

Refeeding syndrome.

Refeeding syndrome can occur when nutrition is reinstituted in a starved patient and, if abnormalities are not corrected, can be lethal. This article describes metabolic adaptations to starvation, risk factors for the development of refeeding syndrome, and causes as well as symptoms of refeeding syndrome. Adverse consequences of hypophosphatemia, hypomagnesemia, and hypokalemia are described. Nursing implications to prevent refeeding syndrome are outlined.

Malnutrition and clinical outcomes: the case for medical nutrition therapy.

Malnutrition is not a new or a rare problem. In studies involving more than 1,327 hospitalized adult patients, 40% to 55% were found to be either malnourished or at risk for malnutrition, and up to 12% were severely malnourished. Surgical patients with likelihood of malnutrition are two to three times more likely to have minor and major complications as well as increased mortality; and their length of stay can be extended by 90% compared with the stay of well-nourished patients. Hospital charges are reported to be from 35% to 75% higher for malnourished patients than for well-nourished patient

A holistic perspective on malnutrition in older adults: Towards an integrated clinical nutrition research guiding framework.

Malnutrition is a multifactorial and complex condition with significant consequences for recovery, functional outcomes, and healthcare systems. Research in malnutrition is often limited by single-component interventions, heterogeneous study designs, and variable outcome measures. This perspective paper introduces a practical guiding framework for clinical nutrition research, emphasizing interdisciplinary, multifactorial approaches, co-designed interventions, and pragmatic, adaptive study designs. Evidence from several trials demonstrates that individualized nutritional support delivered by mul…

Use of Home Enteral Nutrition in Malnourished Post-Bariatric Surgery Patients.

BACKGROUND: Bariatric surgery is by far the most effective treatment option available for successfully achieving and maintaining weight loss in the obese population, but it can also be associated with complications that lead to malnutrition. There is limited data on how enteral nutrition (EN) can be used to provide nutrition support in such cases. METHODS: Retrospective review of our Home Enteral Nutrition (HEN) database was conducted from February 2013 to April 2018 to identify patients who received HEN because of bariatric surgery-related complication. RESULTS: During the study period, 72

Nutritional counseling and nutritional supplements: a cornerstone of multidisciplinary cancer care for cachectic patients.

PURPOSE OF REVIEW: The challenge with cancer cachexia is that it is not fully reversed by nutrition support. The purpose of this review is to provide an opinion on the nutritional management of cancer cachexia based on the most recent available evidence. RECENT FINDINGS: There continues to be a paucity of nutrition intervention studies in patients with cancer cachexia. In patients with cancer undergoing radiotherapy, there is strong evidence that nutrition counseling increases dietary intake, body weight, nutritional status and quality of life with some suggestion that dietary counseling may

Involuntary weight loss and the nonhealing wound.

Wounds elicit a hypermetabolic, net catabolic state that can precipitate profound involuntary weight loss, particularly in the lean tissue compartment. To promote anabolic conditions more conducive to wound healing, an integrated treatment approach is warranted. Effective management includes optimal wound care, treatment or prevention of infection, nutritional intervention (e.g., protein supplementation), moderate exercise, and adjunctive anabolic treatment in patients whose involuntary weight loss has reached at least 10% of usual body weight.

[Recent advances in the nutritional management of pediatric surgical patients].

The prognosis of pediatric surgical patients is significantly improved by perioperative nutritional support. However, the morbidity and mortality rates from intestinal failure, including short gut syndrome and allied disorders of Hirschsprung's disease, remain high. Recent advance in nutrition support focus on the management of intestinal failure. A central venous catheter is commonly used for long-term total parenteral nutrition (TPN) in intestinal failure, although the incidence of catheter-related complications is still high. A nutrition support team approach might reduce such complications…
